STATEMENT OF WORK (SOW)

A. Scope of Work: DLA Distribution Warner Robins has a requirement for nine (9)

Follett Model 110FB425A-S ice and water dispensers to have preventative maintenance performed on them twice a year to ensure they are maintained and operational year-round. This contract also will include the performance of corrective maintenance for repairs, and a parts allowance. The Distribution Center is located in Middle Georgia with the city of Warner Robins adjacent to the base and the city of Macon within 25 miles of the base. Both passenger and cargo air services are provided by either the Middle Georgia Regional Airport in Macon, Georgia or Hartsfield-Jackson International Airport in Atlanta, Georgia. DDWG supports both local and worldwide customers and provide immediate support to industrial and war-fighting customers. We also provide direct support for various mission units assigned to Robins Air Force Base.

1. Tasks to be Performed:

Task 1: DLA Distribution Warner Robins has a requirement for nine (9) Follett Model 110FB425A-S ice and water dispensers to have Preventative Maintenance (PM) performed on them twice a year to ensure they are maintained and operational year round. Each PM will include a deep cleaning to both the machine and dispenser and replacement of the water filters. After each cleaning an inspection should be performed to trouble shoot the machine and to test for any unserviceable parts.

Task 2: Remedial/Corrective Maintenance (RM) includes all work necessary to diagnose equipment malfunctions and restore the equipment to normal operating condition. The Contractor shall submit a Service Call Work Authorization form for ANY work that is to be done as RM. The need for RM service by definition is service that is not identified on the maintenance and testing schedule and is therefore unscheduled.

The notification of an RM service requirement shall, in most cases, be initiated through a service call. The Government will advise the Contractor by phone of any service call requests received during regular working hours. The Contractor shall provide a description of the problem or requested work, date and time received, location, classification, and other appropriate information on a Service Call Work Authorization form by the Contractor. A copy of this form shall be forwarded to the Government for written approval. There may be instances where the Contractor identifies an RM requirement as a result of his inspection or testing activities. In these cases, the Contractor shall complete the Service Call Work Authorization form and submit the completed form to the DDWG designated representative for approval prior to initiating any work on this Service Call Work Authorization form.

The following (9) machines are to be serviced on the contract:

Period of Performance: Work should begin within 60 days after the contract being awarded. Base contract with option years. This is a 5-year contract; Base Year and Four (4) Option Years.

Estimated Periods of Performance:

Base Year: 12 November 2021-11 November 2022

Option Year 1: 12 November 2022-11 November 2023 Option Year 2: 12 November 2023-11 November 2024 Option Year 3: 12 November 2024-11 November 2025 Option Year 4: 12 November 2025-11 November 2026

2. Place of Performance: All work is to be performed at DDWG and Robins AFB, Georgia in multiple buildings.

3. Invoicing: The Contractor will submit an itemized invoice to the Contract Administrator and DDWG designated representatives upon completion of the contract. The invoice will be certified by a DDWG designated representative prior to payment. The amount claimed to be due for services rendered will be indicated by Contract Line Item Numbers (CLIN).

The invoice will contain the following minimum information:

a. Name of Contractor

b. Contract number

c. Contract Line Item Number (CLIN)

d. Description of Work

e. Number of units

f. If applicable, other costs incurred and allowable under the contract and identification of such costs

g. Date service was provided

4. Government Management Oversight: Government management will provide general instructions on limitations and deadlines. Additional instructions will be provided for any unusual assignments or those that vary from established procedures. The Contractor’s employees will independently carry out the assignments. Completed work will be spot-checked by Government management for adherence to procedures, accuracy, and completeness.

5. Certification and Acceptance: The DDWG designated representative is designated as the point of final inspection and acceptance by the Government of all items and services required by the contract.

a. Inspections: The DDWG designated representative is responsible for technical oversight of the Contractor’s performance. The DDWG designated representative duties will include, but are not limited to, assurance of the completion of work in a quality manner, resolution of quality concerns, and payment for services rendered. This responsibility includes the determination of adequacy of services performed by the contractor in accordance with the terms and conditions of the contract.

The DDWG designated representatives will perform all inspections on behalf of the Government and confer with the Contractor regarding any problems encountered in the performance of the work.

b. Deficiencies: Once an inspection has been conducted, the Contractor will be notified via electronic mail of its results. All inspections will be documented on a pass/fail basis. When a failed inspection occurs, the Contractor will be allowed the time to correct all noted deficiencies. The Contractor should resolve the deficiencies as documented by the DDWG designated representative in the failed inspection report. The DDWG designated representative will perform a re-inspection and further assess the area or item of concern once again on a pass/fail basis.

c. Re-Performance of Work: When the Contractor re-performs services as a result of the inspections defined in this section, and the contractor do not meet the standards identified in this SOW, the Government may desire that the work to be re-performed prior to the completion of the delivery order. If this is the case, the Contractor will perform the service at no additional cost to the Government.
