PERFORMANCE WORK STATEMENT
FOR
POTABLE WATER DELIVERY
SECTION I
Description of Services
1. Contractor shall provide all management, tools, supplies, equipment, labor, and transportation necessary to deliver potable water to the areas listed below and any other area that may require potable water on Eielson AFB, Alaska.
1.1 Contractor shall provide four levels of service: regularly scheduled routine biweekly deliveries, regularly scheduled routine weekly deliveries, emergency deliveries, and emergency fire system deliveries.
1.2 Contractor shall provide regularly schedule routine deliveries during normal business hours, Monday through Friday, 0700 - 1700. The contractor shall develop and follow a schedule that fills all of the listed water storage tanks once every two weeks and supply the COR with scheduled dates and times of deliveries. Contact COR of any changes to the schedule.
1.2.1 Contractor shall provide weekly routine deliveries to Building 6396, 1348 Quarry Road (Arctic Survival) during the months of October through March during normal business hours, Monday through Friday, 0700 - 1700. The contractor shall develop and follow a schedule that fills this water storage tank once every week and supply the COR with scheduled dates and times of deliveries. Contact COR of any changes to the schedule.
1.2.2 Contractor shall provide emergency deliveries on an as needed basis to the fire suppression tank located near Bldg 6421 Quarry Road (CATM Fire tank). The contractor must respond within 3 hours of receiving a call for an emergency delivery to this location.

1.4 For scheduled routine deliveries, the contractor shall fill their delivery vehicle from designated locations on Eielson AFB. For emergency deliveries, the contractor shall provide water from their Alaska Department of Environmental Conservation (ADEC) monitored water source.
1.5 Contractor shall provide the government with a copy of their monthly analytical reports submitted to ADEC.
1.6 Delivery charges will reflect cost per gallon from the designated fill locations on Eielson AFB to the identified delivery destinations.

Government Special Instructions, Terms and Conditions
2. GOVERNMENT FURNISHED EQUIPMENT/PROPERTY: The Government will not furnish any facilities or equipment for any work that needs to be accomplished in this Performance Work Statement. The contractor shall furnish all supplies needed to perform this contract according to all its terms.
2.1. HAZARDOUS MATERIALS: The Contractor shall comply with base regulations regarding Hazardous Material (HAZMAT) for all issues of hazardous material, and ensure procedures meet all applicable policies, publications, and protocols in this contract. Provide Bio- Environmental Engineer Service with a Material Safety Data Sheet (MSDS) on all first time purchases of hazardous items obtained through local sources of supply and obtain approval prior to purchase.
2.2. EMERGENCY MEDICAL SERVICE: The Government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill while at Eielson AFB. Treated personnel will be responsible for reimbursing the government for the cost of medical treatment and patient transportation at the current inpatient or outpatient treatment rate.
2.2.1. VOLUNTARY PROTECTION PROGRAM: The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence. 2.3. GOVERNMENT REMEDIES: The Contracting Officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items(Feb2007), for contractor’s failure to correct nonconforming services.
2.4. HOURS OF OPERATION: Services shall typically be performed Monday through Friday from 7:00 am to 5:00 pm local time in coordination with the Government, unless otherwise specified.
2.5. SECURITY REQUIREMENTS: The contractor shall be responsible for and shall pay for obtaining all necessary clearances, cards, passes, decals, or other items required for access of contractor personnel and vehicles to the base or other areas where work will be performed. The contractor shall be required to apply for vehicle and personnel identification media at the Pass and Registration Section. Upon completion or termination of the contract, the contractor shall ensure all vehicle and personnel identification media are surrendered to the Pass and Registration Section. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contractor employees will be required to obtain and display identification badges. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.
2.5.1. The contractor shall maintain a current listing of employees. The list will include employee names, social security numbers and driver’s license numbers. This list shall be provided to the contracting officer and servicing security forces organization. An updated listing shall be provided upon change of employees. The Contractor must provide all names, social security numbers and duty hours of all personnel who will be on site at start or anytime during this project.
2.6. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT
COMMANDER: During times of crisis or national emergency, the contractor shall continue to provide services as scheduled, with minimal disruption of service if directed by a contracting officer.
2.7. MISCELLANEOUS: Use of cell phones while operating any motor vehicle on base is strictly prohibited. Driving privileges will be suspended for 30 days for the first violation. The use of seatbelts is mandatory.
2.7.1. The on-base speed limit while passing military troops performing physical training is 10 MPH and is strictly enforced.
