About this file
This document is a Past Performance Questionnaire issued by the Michael E. Debakey VA Medical Center for solicitation number 36C25625Q1366. The questionnaire is designed for a reference to evaluate an offeror's past performance across multiple dimensions, including overall quality/satisfaction, delivery performance, quality of service, problem resolution, and contractor personnel quality. The form uses a 1-5 rating scale, with 5 representing performance that almost always exceeds contract standards, and includes additional questions about contract terminations, performance issues, and willingness to award another contract.
The questionnaire must be completed by an evaluator with familiarity of the offeror's performance and returned by 09/03/2025 at 12:00 CST via email to Oney.kelly@va.gov. The document requires detailed information about the contract's period of performance, dollar value, and services provided, with space for the reference to attach supporting documentation. Key evaluation areas include assessing the offeror's ability to deliver services, resolve problems, and maintain high-quality personnel, with the ultimate goal of determining the contractor's suitability for future contract awards.
