Montana Roller
Our mouse system is located beneath the floor in the air conditioning closet in the hallway that leads to the janitorial closet and bathrooms in the West end of the building. The trap is accessible through a trap door in the floor. The trap door swings up and can be held in place by a chain and hook on the back wall of the closet. The trap is a 5-gallon bucket that has been fitted with a dowel through a plastic coke bottle.
The coke bottle has a coating of peanut butter which the office will provide and is available in the janitor closet. The peanut butter is out of date and should only be used to bait the trap. There should always be about 3 inches of water in the bottom of the bucket. The trap is set in the crawl space so mice can access the wooden dowel, crawl along it to the peanut butter on the coke bottle and then the bottle spins dropping the mouse into the bottom of the bucket. There is a rope attached to the handle of the bucket, so it is not necessary to climb down into the crawl space to clean out any mice or service the bucket. The bucket can be lifted up and out of the space and then replaced after servicing. It is important to replace the bucket in a position where the mice can access the wooden dowel. Please see the photo above. For servicing, precautions should be taken as with any mouse trap to wear gloves, mask, and any other necessary protection associated with mouse traps and pest removal. There is a scoop available to remove mice from the bucket. The bucket should be checked and serviced weekly to make sure the water level is sufficient and remove any dead mice.
2. Change and install all interior light bulbs or tubes whenever necessary. Government will supply all light bulbs. Used bulbs are to be disposed of in recycle kits provided by the government.
3. Spot clean all stains on the carpet or floors.

11.0 Performance Requirement Summary
Insert Performance Requirement Measures Here
• Calculating Acceptable Quality Level, AQL: Acceptable quality levels AQL will be calculated by dividing the number of acceptable occurrences observed by the number of possible occurrences observed. For example:
Paragraph 3.2 in month one the COR observed 20 janitorial services. Of the 20 observed services, 18 were satisfactory. 18 satisfactory observations divided by 20 possible observation equals 0.9 or 90%.
• Payment deduction is generally a remedy for non-performance, not as an incentive. Non-Monetary incentives may include award term contract arrangement (contractor is awarded periods of performance) or Past Performance for quality level of performance (CPARS).
• Below is an example of Performance Requirement Measures and Surveillance and may be tailored to individual requirements and needs of the program:
