This questionnaire shall be completed by the Offeror. All entries to this questionnaire shall be typed. The questionnaire will be used to determine the Offeror's understanding of the requirements and acceptability of the proposed equipment. Attach any exceptions to the Statement of work or recommendation/design alternatives as detailed on the last page of this questionnaire.

Provide a summary of relevant experience in providing TV, air cargo systems. The project must have started operational use within the last five years and the offeror must have been the prime contractor or the prime subcontractor for the TV or air cargo system for the project to be considered relevant to this acquisition. As a minimum, include the following information for the three projects proposed as relevant experience:
