About this file
This document is a Contractor Responsibility Verification form that outlines the standards and documentation requirements for determining whether a prospective contractor is responsible and qualified to receive a federal contract award.
The form establishes seven mandatory responsibility criteria that contractors must satisfy: adequate financial resources to perform the contract; ability to comply with required delivery or performance schedules; satisfactory performance record; satisfactory record of integrity and business ethics; necessary organization, experience, accounting and operational controls, and technical skills; necessary production, construction, and technical equipment and facilities; and qualification and eligibility under applicable laws and regulations. For each criterion, the form specifies required documentation or certifications. Financial resources verification requires profit/loss statements, balance sheets, lines of credit, or written confirmation from a financial institution. Delivery capability requires a company certification of ability to meet schedules considering existing commitments. Performance record requires submission of minimum two past performance examples from the last three years, or a capability statement for new entrants, with an exemption for contractors with prior government CPARS and SPRS ratings. Business integrity verification requires certification that SAM.gov responsibility information is accurate and current. Organizational structure and production capabilities require company confirmation of adequate resources and equipment or the ability to obtain them. The form concludes with a certification requirement that a company representative must sign, attesting that all provided information is correct and accurate.
