STATEMENT OF WORK (SOW)

FOR

SERVICES NON-PERSONAL

AERIAL HERBICIDE APPLICATION

19 November 2019

1. Description of Services.

1.1 This services non-personal requirement is to control 80% of local grasses, shrubs and small trees on approximately (~) 334.1+- acres located on Moody AFB, GA in three General Zones. Zone A is approximately 114.1+- acres and is located at the south end of the runway (South EOR). Zone B is approximately 156.1+- acres and is located in the Grand Bay Weapons Range Impact Area. Zone C is approximately 63.9+- acres and is located near the Bemis Field ULZ Area on Grand Bay Weapons Range. This contract is to cover any aerial herbicide spraying needed over a five year period. Tentatively, the sprayings are scheduled for Zone A in 2021 and 2024, and Zones B and C in 2020, 2022, and 2024, but with the option to delay any spraying by one year depending on funding availability, weather and access constraints, as well as the results of the previous treatment, etc.

1.1. Work consists of applying herbicide via helicopter. Contractor shall furnish all labor, materials, equipment (including safety equipment, spill kit capable of containing 110% of the largest possible chemical spill, and a spill containment system of appropriate size), tools, chemicals, transportation, storage and disposal, supervision and perform all work required to apply herbicide using aerial spraying for different cover types. Water is available via fire hydrant at the Range shop or at the entomology shop, but the contractor can initially provide water for convenience. If/when filling up from fire hydrants on the range or entomology shop, a backwash protection system or an air gap protection system must be employed.

1.2. The treatment area associated with this SOW consists of ~ 334.1+- acres (see attached Zone Maps) of local grasses, shrubs and small trees on Moody AFB and Grand Bay Weapons Range. The areas to be sprayed contain mostly wetlands and all of the required herbicides listed in this statement of work are appropriately labeled for aquatic use.

1.2.1 Aerial spraying application shall be by helicopter ONLY. This work shall be accomplished between 23 June - 30 September in whatever year the spraying is scheduled.

Minor timeframe adjustment may be required for any or all of the three (3) zones indicated due to funding or weather issues. The available days for aerial herbicide applications must be closely coordinated with the Range Manager and Airfield Manager, and generally will be when the airfield or range is closed on weekends, holidays, no-fly days, or down days like Fridays before a Monday holiday. Three long holiday weekends that may be a good target time for spraying would be Memorial Day weekend, July 4th weekend, and Labor Day weekend. The range areas have a bit more flexibility in scheduling spraying in coordination with the range manager. Timing of spraying should be coordinated, if possible, to avoid times of severe drought.

1.3. Objective. The purpose of aerial spraying is to control and eliminate undesirable vegetation, in order to meet specific program objectives. Kill back or control most (80%) of the vegetation, especially brush and small trees. The vegetation as it gets taller produces a habitat appropriate for numerous bird species that are creating a Bird Aircraft Strike Hazard, limits the visibility for safety observers, and poses a potential fire hazard.

1.4 Description and Location. The area to be treated is on the south end of the airfield on Moody AFB, GA (see attached map of Zone A), the impact area of Grand Bay Bombing

Range (see attached map of Zone B), and the Bemis Field ULZ area (see attached map of Zone C). Zones B and C are about 3-3.5 miles northeast or east of Zone A.

1.5 Boundaries. The areas to be sprayed are clearly indicated on the attached maps of Zones A, B, and C. Shape files produced using ESRI ArcGIS ArcMap 10.3.1 software data can be provided by the airfield or range managers if necessary. The acres shown for each area were estimated using aerial imagery and ESRI ArcGIS ArcMap 10.6.1 software.

1.6 Contractor Qualifications. (1) The Contractor shall have a Georgia Commercial Pesticide Contractors License in categories 23 (Forestry), 26 (Aquatic), and 34 (Aerial Equipment); and shall adhere to all herbicide label directions. The actual applicator who flys the helicopter shall have a Georgia Pesticide Applicators License in categories 23 (Forestry), 26 (Aquatic), and 34 (Aerial Equipment). A copy of the applicator’s license should be submitted to the Base Pest Management Supervisor or alternate at least one week prior to spray application.

2. GENERAL INFORMATION

2.1. Coordination. The Contractor shall coordinate closely with the government representative who in turn, will coordinate with the Airfield Manager and the Grand Bay Range Manager concerning items such as security, access to work site, work schedules, etc.

The Contractor shall notify the government representative who in turn, shall notify the airfield and range managers as well as Air Traffic Control Tower and 23 WG Scheduling Office at least two (2) days prior to flying on base.

2.2. Work Hours. Work hours will be during daylight hours when weather conditions are favorable and when not in conflict with the range schedule or the airfield flying schedule preferably between 0630 and 2000 (6:30 am and 8:00 pm) in coordination with the government representative for the zone sprayed and either the Airfield Manager, Range Manager or Base Pest Management Supervisor (or their designee) present to inspect the operation at time of application.

2.3. Safety and Health. The Contractor shall comply with all applicable requirements in OSHA Parts 1910 and 1926. The contractor is responsible for providing a safety plan that identifies a point of contact in the event of any accident at the time that scheduling occurs. The initial point of contact in the plan should state that 911 is the initial call to be made in the event of an emergency and to tell the operator you are on Moody Air Force Base in order to be transferred to the Moody Dispatch Center (also called the Moody Emergency Control Center).

The plan should also include a description of on-site emergency procedures the Contractor plans to follow in case of an accident. The plan shall identify the insurance company providing coverage and the pertinent group coverage number. It is the responsibility of the Contractor to identify all hazards within the work area. All appropriate precautions shall be taken by the Contractor.

2.4. Technical Specifications.

2.4.1. Aerial Spraying for Zones A. Contracted services will involve a single aerial helicopter application of the herbicides Alligare Imazapyr 4 SL (EPA Reg. No. 81927-24, 52.6% Imazapyr) at a rate of 1 quart per acre, Trycera (EPA Reg. No. 5905-580, 29.4% Triclopyr) at a rate of 3 quarts per acre, and Alligare Glyphosate 5.4 (EPA Reg. No. 81927-8, 53.8% Glyphosate) at a rate of 3 quarts per acre. The herbicides shall be applied in accordance with the label guidelines by a certified applicator. Herbicides shall be diluted with 15 gallons of water per acre. The helicopter shall be equipped with nozzles that produce a course spray to minimize drift and achieve uniform coverage. A methylated seed oil (MSO) or vegetable-based seed oil concentrate (labeled for use on aquatic applications) shall be added last to the spray mixture at a rate in accordance with the label directions to enhance control and will be agitated as needed to prevent separation or follow specific tank filling information on the herbicide label. Herbicides shall not be applied if winds exceed eight (8) MPH, not during a temperature inversion or when ground fog is present, not when heavy dew is present, and not within 3 hours of predicted oncoming precipitation. Contractor shall provide a completed Pesticide Application Records (PAR) for each treated site. A GIS shapefile or spray map showing treated areas is required.

Approved Chemical Substitutions: Arsenal (EPA Reg. No. 241-346, 27.8% Isopropylamine salt of Imazapyr) at 2 quarts per acre can be substituted for Alligare Imazapyr 4 SL at one quart per acre. Either Aqua Neat aquatic herbicide (EPA Reg. No. 228-365) or Rodeo (EPA Reg. No.

62719-324) at 3 quarts per acre can be substituted for Alligare Glyphosate 5.4 at 3 quarts per acre.

There are no suitable substitutions for Trycera. Trycera is an acid formulation of Triclopyr that has an aquatic label. Compared to other aquatic labeled herbicides containing Triclopyr which are amine formulations, at higher rates, Trycera has shown to be less antagonistic when mixed with herbicides containing Imazapyr.

2.4.2. Aerial Spraying for Zones B and C. Contracted services will involve a single aerial helicopter application of the herbicides Alligare Imazapyr 4 SL (EPA Reg. 2.5. Environmental and Cultural Resources.

2.5.1. Mixing Site. For Zone A, all mixing shall be done, either on the Hot Cargo Pad southwest of the control tower or on Taxiway Alpha (located on the south end of the airfield, between the two runways). Any alternative mixing-site on base must be approved by the Base Pest Management Supervisor (CES/CEOIE). For Zones B and C, all mixing shall be done, near the target material holding area near the Range Operations Office at the northeast corner of the Grand Bay Range or on the ULZ on Bemis Field. Any alternative mixing-site must be approved by the Base Pest Management Supervisor. The Contractor shall provide a spill kit and secondary containment and have it on site for all mixing. A Material Safety Data Sheet (MSDS) for each chemical product shall be onsite.

2.5.2. Protection of Streams, Lakes, and Reservoirs. The Contractor shall use every precaution necessary to prevent pollution of streams, lakes, and reservoirs by fuels, oils, and other chemicals or other harmful materials. Any waste of said material shall be disposed of in accordance with EPA regulations. All streams and grounds will be kept free of Contractor-generated debris. Contractor shall take all empty herbicide containers with them when leaving the base and dispose of them according to label directions at a location off of Moody

AFB.

2.5.3. Protection of Soil. The Contractor shall use every precaution necessary to prevent the contamination of soils by spills of petroleum products (oil and fuels), ethylene glycol (coolant), and other chemicals or harmful materials. The Contractor shall also be responsible for reporting all spills of the aforementioned materials immediately to the Contracting Officer (CO) or the Base Pest Management Supervisor.

2.5.4. Pre-Performance Required Documentation.

2.5.4.1. The Contractor shall complete and submit the following forms a minimum of thirty

(30) days, prior to conducting flight operations on Moody AFB. Contact the Airfield Manager for details.

2.5.4.2. DD Form 2400, Civil Aircraft Certificate of Insurance

2.5.4.3. DD Form 2401, Civil Aircraft Landing Permit

2.5.4.4. DD Form 2402, Hold Harmless Agreement

2.5.4.5. Georgia Commercial Pesticide Contractors License

2.5.4.6. Georgia Commercial Pesticide Apllicators License

2.5.5. Inspection and Acceptance.

2.5.5.1. Inspection Procedures. Inspection by the Government of the work performed under this contract shall be made as the spraying progresses.

2.5.5.2. The Pest Management Supervisor, along with the government representative or alternate will check activities to ensure herbicide applications are made within acceptable weather parameters, as described in sections 2.4.1. and 2.4.2., and that applications are made at the specified volume(s) and concentration(s).

2.5.6. Contractor Inspection and Record Keeping.

2.5.6.1. The Contractor shall provide and maintain a system of records covering the services under this contract utilizing DD Form 1532 “Pest Management Report and DD Form 1532-1 “Pest Management Maintenance Record” or company service ticket containing as a minimum:

2.5.6.1.1. Applicators name and certification number and expiration date

2.5.6.1.2. Total area treated and map of sprayed area or shape file

2.5.6.1.3. Common name and amount of pesticide concentrate applied

2.5.6.1.4. Amount of final solution applied

2.5.6.1.5. Concentration of final solution

2.5.6.1.6. Rate of application (volume/unit area)

2.5.6.1.7. Complete records of all work performed by the Contractor shall be maintained and provided to the Base Pest Management Supervisor upon completion of the spraying.

2.7. Measurement and Payment.

2.7.1. Measurement. The acreage listed in the bid schedule is considered an estimate of the acreage to be sprayed and has been determined by base personnel using aerial imagery and ESRI ArcGis ArcMap 10.3.1 software. For the purpose of determining the payment amount, these acreage amounts determined will be used to the nearest one tenth (1/10) acre as shown on the maps of Zones A, B, and C. If for some reason, some of the areas shown on the maps of Zone, A, B, or C are not sprayed, base personnel will use aerial imagery, ESRI ArcGis software, as well as a handheld GPS unit if needed to clarify any reductions in the total acres of sprayed areas once the vegetation has changed in color enough to see what has been sprayed. If the contractor has a helicopter- based mapping system, which differs in the estimated acres from the government maps of Zones A, B, and C, the contractor should not spray more than the total number of acres listed for any individual zone.

2.7.2 Basis for Payment. Payment will be made in accordance with contracted procedures.

Payments will be made in accordance with each issued task ordered and paid via contractor submitted invoices in WAWF.

2.8. Records Management Requirement.

2.8.1. All records, files, documents, regardless of media (e.g., paper, electronic, etc.), as described in the SOW, that are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract. The Contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, Air Force Manual (AFMAN) 33-364, paragraph 2.10, and the Air Force Records Disposition Schedule which may be acquired by contacting the Grand Bay Range Manager or the Base Pest Management Supervisor. Records shall be turned over to the Government (Base Pest Management Supervisor) upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.

2.8.1.1. All records, files, and documents, regardless of media (e.g., paper, electronic, etc.), as described in this PWS, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.

2.8.2. The service provider shall ensure all records (according to Title 44, United States Code, Public Printing and Documents, Section 3301, include all books, papers, maps, photographs, machine readable materials, or other documentary materials, regardless of physical form or characteristics), files, documents and working papers provided by the Government, and/or generated for the Government in the performance of this contract, remain Government property and are maintained in accordance with established Federal Records Act (Public Law 81-754), Freedom of Information Act (Public Law 90-23), Privacy Act of 1974 (Public Law 93-529), (DoD), and Air Force directives listed in this SOW.

2.9. Conformance with Environmental Management Systems.

2.9.1. The Contractor shall perform work under this contract consistent with the relevant policy and objectives identified in the 23 Wing’s Environmental Management System (EMS) applicable for this contract. The Contractor shall perform work in a manner that conforms to all appropriate EMS and operational controls identified by the agency, organizational, or facility EMS, and provide monitoring and measurement information as necessary for the organization to address environmental performance relative to the environmental, energy, and transportation management goals. In the event an environmental non-compliance associated with the contracted services is identified, the Contractor shall take corrective and/or preventative actions.

2.9.2. In the case of a noncompliance, the Contractor shall respond and take corrective action immediately. In the case of a nonconformance, the Contractor shall respond and take corrective action based on the time schedule established by the EMS Site Coordinator. In addition, the Contractor shall ensure that their employees are aware of the roles and responsibilities identified by the EMS and how these requirements affect their work performed under this contract.

2.9.3. Prior to the day of actual spraying, the on-site Contractor shall contact the 23d Wing Environmental Coordinator either in person or by phone at 229-560-6200 or 229-257-4654 for an environmental briefing which will include initial EMS awareness training. Individual documentation processes will be established by the EMS Coordinator or a designated appointee.
