Statement of Work (SOW) Veterans Health Care System of the Ozarks (VHSO)
Fayetteville, Arkansas
1) Requirement Title: Boiler Safety Valve Conversion
2) Background: This requirement is needed due to findings related to a VA Boiler Plant Safety Device Report, from March 2023. A copy of this report will be provided for interested parties upon written request, via email to brian.hendren@va.gov.
3) Scope of Work The contractor shall furnish all management, supervision, labor, materials, equipment, parts, supplies, tools, and related services required to provide and install steel safety valves on boilers one, two and three.
Contractor shall remove and dispose existing six (6) threaded brass safety valves and replace with new flanged bolt on steel safety valves. Contractor shall provide all labor and materials to perform all fabrication required to complete the conversion. The contractor shall have the option to purchase approved and certified factory manufactured safety valve adaptors.
4) Deliverables A) Boiler 1:
1) Remove and dispose safety valves Kunkle 6030GFM01AAM and Kunkle 6030HGM01AAM.
2) Install one (1) Kunkle 1-1/2 x 2-1/2 300X150 300LHG01 (KUN300LHG01) safety valve and one (1)
Kunkle 1-1/4 x 1-1/2 300X150 (KUN300LGF01) safety valve.
3) Provide all modifications and fabrication required for installation of new safety valves.
B) Boiler 2:
1) Remove and dispose safety valves Kunkle 6030GFM01-AM and Kunkle 6030HGM01-AM.
2) Install one (1) Kunkle 1-1/2 x 2-1/2 300X150 300LHG01 (KUN300LHG01) safety valve and one (1)
Kunkle 1-1/4 x 1-1/2 300X150 (KUN300LGF01) safety valve.
3) Provide all modifications and fabrication required for installation of new safety valves.
B) Boiler 3:
1) Remove and dispose safety valves Kunkle 6030HBM01AAM and 6030GFM01AAM.
2) Install one (1) Kunkle 1-1/2 x 2-1/2 300X150 300LHG01 (KUN300LHG01) safety valve and one (1)
Kunkle 1-1/4 x 1-1/2 300X150 (KUN300LGF01) safety valve.
3) Provide all modifications and fabrication required for installation of new safety valves.
5) General Requirements A) All modifications must comply with VHA and State of Arkansas policies and regulations.
B) VHSO facility must continue to operate during the contract Period of Performance, including but not limited to, the mechanical and electrical systems.
C) Warranty: Contractor must guarantee workmanship for a period of one (1) year after Contracting Officer
Representative (COR) written acceptance of work. Contractor must furnish a minimum one (1) year parts and labor warranty on equipment, components, and materials furnished plus any manufacturer’s warranty greater than one (1) year.
D) Contractor must coordinate all services with the COR, and communicate all questions, concerns, or issues to the COR. The CO must approve any request, or proposal for changes in project scope or additional requirements, in writing prior to the Contractor proceeding with any new work. Any Contractor changes without prior written approval by the CO are unauthorized.

6) Place of Performance Veterans Health Care System of the Ozarks (VHSO) Main Campus 1100 N. College Avenue, Fayetteville, AR 72703
7) Period of Performance See contract line item schedule.
8) Hours of Operation A) Regular Business Hours: Site work must be scheduled during regular business hours, 0600 to 1600 hours, Monday through Friday.
B) National Holidays are excluded. If a holiday falls on Sunday, the following Monday will be observed as the
National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Scheduled services should be completed on the first business day following any federal holiday.
C) Prior to any service or repair performed outside of regular business hours, these services shall be approved by the COR at a price agreed to by the Contractor and Contracting Officer (CO).
D) Contractor must plan contract work activity that interferes with Medical Center (MC) operations and must not commence without prior COR written approval at no additional expense to the Government.
9) Security A) Contractor must coordinate with VA Police and COR to comply with the VHSO security management program, including but not limited to, obtain onsite access permission by the VA Police, be identified by Contractor and project, and restricted from unauthorized access.
B) While on campus, Contractor must wear a visible VHSO issued identity Badge. Contractor can be subject to inspection of personal effects when entering or leaving the project site and VHSO property. Contractor site superintendent must escort Contractor employees to Engineering Shop (Building 37) to obtain badge(s). Contractor must maintain a Badge Log with Employee Names, Badge ID Numbers, Start Date(s), Departure Date(s), ID Lost Date(s), and ID Return Date(s). Contractor must update Badge Log and furnish to the COR weekly or as directed in writing by COR. Contractor must report lost Badge(s) immediately to the COR and document on Badge Log. Contractor must return Badge(s) immediately on day of departure of employee from project to COR and document on Badge Log. Contractor must return remaining Badge(s) at project closeout to COR. Contractor must reimburse the Government a $100.00 fee for each lost or unreturned Badge.
C) Contractor site superintendent may check out a key to access mechanical and electrical rooms as project requires. Contractor site superintendent must check out and return a key at Engineering (Building 37) daily and the key must not leave VHSO property. Contractor site superintendent must notify the COR when a key is lost. Contractor shall reimburse the Government a $3,500.00 fee for each lost or unreturned key(s).
D) Contractor must be escorted by VHSO personnel to take job site related photos. Contractor must not photograph any patients, personnel, visitors, and others on VHSO property.
10) Information Security **The Certification and Accreditation (C&A) does not apply, a Security Accreditation Package is not required**
11) Other Requirements A) Contractor must comply with applicable Federal, State, Local laws, codes, regulations, and ordinances.
B) If required, the Contractor shall obtain applicable permits and licenses required by Federal, State, Local law, code, regulation, and ordinance(s) at no additional cost to the Government.
C) Contractor shall manage its employees and resolve employee issues included but not limited to: dress code, code of conduct, performance, payroll issues, etc.) Supervision of contractor employees is the responsibility of the contractor.
D) Contractor must make every reasonable effort to protect VHSO property from damage. Any property, including but not limited to, electrical, natural gas, medical gas, communication and utility lines, landscaping, walkways, roads, and curbs damaged by Contractor must be repaired or replaced at no additional cost to the Government. Government property, including but not limited to, walls, ceilings, floors, furniture, must be protected from damage at no additional cost to the Government. Areas must be cleaned after coring, sawing, and other construction activities by the Contractor at no additional cost to the Government. Anything remaining in place that is damaged or defaced by the Contractor must be restored by the Contractor to the pre-existing damaged or defaced condition at no additional cost to the Government.
E) Safety must be the Contractor’s highest priority.
