Altus Air Force Base

Fire Department

NFPA Testing FY22 w/4 option years

Statement of Work

1. SCOPE OF WORK: The contractor will provide the 97 CES/CEF all labor, tools, equipment, test equipment, personal protective clothing and equipment, materials, transportation and any other incidentals necessary to travel to Altus AFB, OK to perform the NFPA

Testing outlined below.

a. Perform hose testing IAW NFPA 1962 – Standard on Fire Hose.

b. Unload all hose to be tested.

c. Perform a visual inspection of each hose to identify any physical damage.

d. Perform a hydrostatic pressure test.

e. Perform a hydrostatic pressure test on all nozzles and appliances IAW NFPA 1962 during this time as well.

f. Ensure that all hose which fails any part of the inspection is marked and removed from service.

g. Mark both ends of each section of hose with stencils provided by 97 CES/CEF and black spray paint provided by the contractor.

h. Reload all hose to department specifications with the exception of “hotel packs”.

i. Perform ground ladder testing IAW NFPA 1932—Standard on Ground Ladders.

j. Perform vehicle mounted equipment testing on all pumps and pump systems

IAW NFPA 1911.

k. Provide both digital and hard copy of all test results within 10 days of completion to include: Date of test, service test pressures, hose diameter, length, type, identification number, location (which apparatus the equipment is assigned to) and pass/fail for each piece of hose, nozzle, appliance or ground ladder as required per NFPA 1962 and 1932.

l. Provide both digital and hard copy of all test results within 10 days of completion to include: Date of test and all pertinent information as required by NFPA 1911 for vehicle mounted equipment.

2. Standards and Codes: The contractor shall comply with all Federal, State and local regulations pertaining to the required work including, but not limited to:

a. National Fire Protection Association (NFPA)

b. Occupational Safety and Health Administration (OSHA)

3. Delivery Schedule: Testing to be completed in April of each year

a. Base Year in April 2022

b. Option Year 1 in April 2023

c. Option Year 2 in April 2024

d. Option Year 3 in April 2025

e. Option Year 4 in April 2026

4. SECURITY REQUIREMENTS: The Contractor shall be responsible to gain access to Altus

Air Force Base through the contract. The Contractor shall coordinate with the CO and

Fire Department to gain access to Altus AFB.

a. While on Altus AFB, the Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office.

Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.

b. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.

c. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.

Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

d. Anti-Terrorism. Altus AFB has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus AFB, you and/or your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and/or your sub-contractors are expected to report to base security forces (481-7444) any suspicious activities, packages or items you see while conducting work on Altus AFB. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees and sub-contractors about antiterrorism, Altus AFB has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus AFB, Oklahoma.

5. General Provisions:

a. Notification: The contractor shall coordinate all work with the end-user POC. The contactor shall provide notice of five days prior to work starting to allow end-user time to prepare equipment getting tested.

6. Safety: The contractor shall maintain a safe and healthful working condition for all contractor personnel and must ensure conformance with standards developed under United States Public Law 91-596, OSHA standards, American National Standards Institute and NFPA.

7. Mishap Reporting: Report all mishaps involving any person sustaining injury or illness from contractor operations and all mishaps involving Government property or equipment damaged by the contractor to the end-user.

8. Environmental: Contractor is required to comply with all environmental standards as required by Altus AFB 97 CES/CEIE. They can be contacted at 580-481-6221.
