STATEMENT OF WORK

TO

PROVIDE AND INSTALL NEW SPREADERS AND PLOWS FOR SNOW EQUIPMENT AT

JOINT BASE MDL, NEW JERSEY

DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, tools, materials, supervision and other items necessary to install (2) SaltDogg SHPE6000 Spreaders on (2) snow removal vehicles. The contractor shall also provide all personnel, equipment, tools, materials, supervision and other items necessary to install (1) Henke Model 36R11IS Reversible Snow plow and (1) Western 10 foot Pro Plus snow plow on (2) snow removal vehicles. The contractor will provide and install all new components. Black is the preferred color of choice for the new equipment. Vehicles will be delivered to the contractor’s facility by the customer, and picked up upon completion. As a result, the contractor’s facility must be within a 50 mile radius of JB MDL (McGuire), if possible.

1. Snow Equipment Installation Breakdown

1.1. (2) SaltDogg SHPE6000 Spreader on (2) vehicle: – 2020 INTERNATIONAL DUMP TRUCK / model (MV607 4X2 and HV507 4X4), Install new SaltDogg SHPE6000 Spreader or equal.

1.2. (1) Henke Model 36R11IS, 11 FT Reversible Snow Plow on (1) vehicle: - 2020

INTERNATIONAL DUMP TRUCK / model (MV607 4X2) or equal

1.3. (1) Western 10 FT Pro Plus Heavy Duty Snow Plow on (1) vehicle: - 2020

INTERNATIONAL DUMP TRUCK / model (HV507 4X4) or equal

2. Minimum Requirements

2.1. Electric powered motor must be included and installed.

2.2. Spreader assembly and controls must be easily detachable for truck use during the summer months.

2.3. Spreader hopper must be constructed out of poly material, in an inverted “V” design. With a 6 cu yd capacity.

2.4. Must handle bulk salt.

2.5. Spreader must have hopper vibrator, top screen, tie downs, remote bearing grease kit and tarp or hard cover installed.

2.6. Independent auger and spinner controls in cab.

ADDITIONAL RESPONSIBILITIES

Any adjacent areas disassembled, damaged, or removed by the contractor during installation shall be properly repaired by the contractor at no expense to the Government. Any and all debris / trash generated by this project shall be removed and properly disposed of off base by the contractor. The contractor or his representative (QAP) shall be on site during duty hours or the hours of work being accomplished. The contractor shall coordinate the time that work will be accomplished with the points of contact listed below. The contractor shall be responsible for daily clean up at the work site and complete restoration of the area after completion of the work. All work shall be accomplished in accordance with all Federal, state, and local laws and regulations, applicable building codes and standards, NEC, NFPA, BOCA, & CABO, and OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) codes. NOTE: Where more than one standard is applicable, the higher standard shall be adhered to.

SPECIAL REQUIREMENTS: The work site shall remain clean and clutter free since this area is an active employment site. All dumpsters or other containers for trash and debris shall be located in areas designated by the building manager and shall be subject to removal and / or relocation at the building manager’s request. These requirements shall not impact the performance period.

The Contractor Manager or an alternate shall be available within sixty (60) minutes notice during normal duty hours, 7:30am to 4:15pm, to meet with Government personnel to discuss any problems encountered at the work site. Contractor will supply Government Quality Assurance Personnel (QAP) with phone number for contacting purposes.

BASE ENTRY: The contractor shall obtain the necessary passes and /or identification for entry into McGuire Air Force Base and Fort Dix for all employees prior to commencement of work. The Government reserves the right to refuse to issue an entrance pass to any employee for any reason deemed valid by the Government. The contractor shall be required to provide the Security Forces with vehicle registration(s), proof(s) of insurance, driver license(s), and a picture ID(s) to obtain a pass for entry onto the base. The Pass and Registration building is located inside the twenty-four (24) hour gate right off of Wrightstown / Cookstown Rd.

SECURITY FORCE AND FIRE PROTECTION: The Base Security Police telephone number is 609- 754-2001 and the Base Fire Department telephone number is 911 for reporting a fire. General information telephone number is 609-754-3326.

PERFORMANCE PERIOD: All work shall be completed within a 30 calendar day period, excluding holidays. Provide material lead times to CE repetitive for scheduling and planning purposes.

INSPECTION / ACCEPTANCE OF PROJECT COMPLETION: A final inspection will be scheduled upon completion of all work. Contact the Quality Assurance Evaluator two (2) days prior to completion for a site visit.
