STATEMENT OF WORK FOR
PURCHASE OF MATTRESSES FOR MISSILE ALERT FACILITIES
BACKGROUND. This Statement of Work (SOW) describes the requirements for the purchase of four hundred sixty five (465) mattresses, four hundred sixty five (465) bed bug encasements and the pick-up and disposal of all old mattresses from fifteen (15) missile alert facilities. Thirty one (31) mattresses with bedbug encasements shall be delivered to each of the missile alert facilities. Old mattresses will be available for pick-up 24 hours after delivery of new mattresses and will be picked up within five (5) days of being available.
SCOPE. This contract is for the purchase of 465 mattresses with bedbug encasements with the below requirements, to be delivered to fifteen (15) missile alert facilities, unloaded to the on-site vehicle garage, and then to return within five days after notification to pick-up old mattresses for disposal.
Mattress requirements include:
• 2-stage twin extra-long foam mattress with dimensions of 39”x80”x11”
• FR barrier fire retardant material in accordance with all state and federal fire qualifications
• Ticking, breathable material, anti-bacterial, anti-fungal, anti-static, bedbug resistant, anti-microbial, crack resistant, stain resistant, non-allergic, waterproof, fire retardant and odor free
• Softer feel with a rating of 4/10 in firmness
• Bedbug encasement
• Made without ozone depleters
• Made without PBDEs, TCDPP or TCE (“Tris”) flame retardants
• Made without mercury, lead and other heavy metals
• Made without formaldehyde
• Made without phthalates regulated by the Consumer Product Safety Commission 11
• Low VOC (volatile organic compound) emissions for indoor air quality (less than 0.5 parts per million)
OBJECTIVE. To purchase mattresses with bed bug encasements to be delivered to each of the 15 missile alert facilities and on a separate day have the old mattresses picked up and disposed of.
General Information.
The Contractor is responsible for delivery of new mattresses with bed bug encasements and pick-up and disposal of all old mattresses. Appendix A includes a map indicating the location of each Missile Alert Facility and Appendix B indicates their GPS coordinates and the approximate distance from Minot Air Force Base, North Dakota, to each Missile Alert Facility. Contractor shall coordinate delivery with MSgt Joshua Lorenz 72 hours prior to delivering and/or picking-up mattresses.
The Contractor is solely responsible for compliance with all Federal and State standards and regulations for the disposal of old mattresses. The Air Force is not responsible for ensuring that the Contractor comply with “personal” safety requirements that do not present the potential to damage government resources.
Environmental Compliance. Contractor shall be knowledgeable of and comply with all applicable Federal, State and local regulations and requirements regarding environmental protection and the disposal of old mattresses.
Government Furnished Property and Services
Government Furnished Utilities. In the event that the Contractor requires utilities, the Government will furnish water and sewage as necessary for accomplishment of work in accordance with this contract unless unavailable.
Telephone Service. The Government shall only provide base and local commercial telephone service.
Long distance service, if desired, shall be at the Contractor’s expense. Contractor shall only be able to use personal cellphones prior to processing through security office to begin delivery or pick up of product.
Security, Fire and Medical Services. The Government will provide police and fire protection. In the event of a medical emergency, commercial ambulance service for transporting an injured employee to a local hospital is available at the cost to the Contractor.
Installation Information
Hours of Operation. The Contractor shall perform the services required under this contract during the hours of 0800 and 1700. The Contractor is not required to perform services on Federal Holidays.
Normal Hours of Contractor Operation. The contractor may find it necessary to deviate from the normal base hours of operation to ensure timely delivery and pick-up under this SOW at no additional cost to the government.
Federal Holidays

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed.
MAF Limited Entry and/or Highway Closure. Deliveries and pick-ups scheduled but not accomplished due to weather, exercise, or actual alert, will be accomplished as soon as possible after reopening the MAF.
Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes.
The Contractor shall comply with all requirements and procedures IAW AFFAR 5353.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted prior to the delivery or pick-up or products.
Pass Requirements. The Contractor shall provide an up-to-date roster of contractor personnel, by employee name, and position title, last four digits of the social security number, valid driver license number (or valid state issued identification card) and level of clearance held who are authorized to work on the installation to the Contracting Officer prior to start of work or when changes occur.
Termination Requirements. Upon delivery and pick-up or all mattresses, the Contractor shall surrender all Government Issued Identification or passes to include, all vehicle identification to the Security Forces at the Visitor Control Center (Building 425).
Heightened Security and FPCONs. During periods of heightened security and Force Protection Conditions (FPCONs), the Contractor shall still be responsible for delivering or picking-up mattresses unless notified by the owning squadron or a designated representative.
During times of heightened security, delays at the entry gates should be expected; also movement on to and around the site will be significantly restricted.
Contractor’s Vehicles and Drivers. All drivers will have in their possession the following documentation: valid driver’s license, vehicle registration and insurance card. Any driver who does not have these items will be turned away and denied access to the installation. Any driver who is not on the Entry Authorization List (EAL) will be turned away and denied access to the installation. All contractor vehicles must be on the Entry Authorization List (EAL) or the vehicle will not be allowed on site.
Information needed to approve vehicle on the Entry Authorization List (EAL) includes: Make, Model, Year, Primary color, and License plate number (State & Number, if no plate then the equipment serial number).
Additional Security Requirements. All employees will be required to be approved for, and on a current Entry Authorization List (EAL) at least two weeks prior to the beginning of delivery and pick-up of mattresses. Information required from each employee includes the following: photocopies of the front and back of a valid form of picture ID (i.e. driver’s license, state ID card, etc.), date of birth (DOB), and social security number (SSN). Employees will not be permitted to bring any electronic devices onto the facility, to include but not limited to cellular phones, laptops, and pagers. Weapons of any kind will not be allowed on site. Employees must be U.S. citizens with at least one valid form of picture identification.
Background checks are required. Specific site entry procedures will be briefed to the delivery/pick-up crew.
