Statement of Work
Kitchen Equipment at 3 Missile Alert Facilities
29 July 2020
1. GENERAL: The contractor shall provide all labor, materials and equipment to install the new kitchen equipment and remove the old equipment (Reach in Fridges, 2 Door Freezers, Single Door Freezers, Stovetops, Ovens, Fryers, and Sanitizers) at 3 Missile Alert Facilities at Minot, North Dakota. If certain items do not meet the brand specifications but meet the other specifications, then the item(s) can be swapped with another brand. The installation shall be in accordance with manufacturer’s instructions, attached schematics and requirements of the Statement of Work (SOW) as stated in the following paragraphs.
2. PERFORMANCE PERIOD: This contract shall have 30 days performance period and a total of one day at each Missile Alert Facility. Coordinate work area with Contract Management and Using Agency. Work should begin within 60 days after award of contract.
3. GENERAL SCOPE OF WORK: Work includes but is not limited to: removing the existing fridges, freezers, dishwashers, stovetops, ovens, fryers, and placing in the garage on site and install the new product. This will include all electrical and wiring if necessary. Old equipment will be disposed by service contractor. All transportation, equipment, tools, supervision, labor, materials and performing the work in accordance with the specifications, attached equipment standards, standard industry practice, National Fire Prevention Association (NFPA) codes, the National Electrical Code (NEC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), Uniform Building Code (UBC), manufacturer’s recommendations and terms of the contract. The contractor shall delivery the equipment directly to the Missile Alert Facilities and install the new equipment, and dispose of the current equipment in the same day (one day per facility). All proper approvals (EAL), shall be coordinated prior to delivery/install in accordance with paragraph 4 of this Statement of Work.
4. FACILITY ACCESS: Each employee will be required to provide (2) credentialed forms of identification with their photo ID, social security number, full name, birth date. Send through EAL information at least 14-20 days prior to contract performance. Individuals not on the EAL will not be permitted on site. Dan Betz 701-723-4836 for CE Construction otherwise your customer POC will work the approval of the EAL. Coordination with Missile Alert Facility Staff as soon as contract is granted to process paperwork to give access to Missile Alert Facilities access to Contractor. Contractor will contact Missile Alert Facility Staff one day prior to heading out the Missile Alert Facilities. Contractors must provide their own PPE (Gloves, Masks) and sanitize all equipment/surfaces.
5. NOTIFICATION: Contractor shall provide written notification to the Contracting Officer two weeks prior to commencement of work. The Contracting Officer shall notify the Project Inspector who in turn will notify the Using Agency. Initial contact and coordination between the end user and contractor shall take place as soon as the contractor receives receipt of award.
6. DEBRIS: Contractor shall remove all construction rubble from the work site to a Contracted Authorized onsite Construction Dumpster or remove from Missile Alert Facility on a daily basis.
Contractor is responsible for the security of tools and equipment being used at the project site.
7. SITE DAMAGE: Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the government.
8. CONTRACTOR WORK HOURS: Recommended working hours for the Contractor will be Monday - Sunday 0800-1800, weekends will need pre-arrangement with the Missile Alert Facility Chef Manager.
9. DIMENSIONS: Each equipment listed below is made to fit the area, please use the dimensions from the below listed equipment and not exceed their width/length/depth. There is a 2” tolerance width for the fridges/freezers at the site but no more than that.
