About this file
The document is a Statement of Need from the Montana VA Medical Center (VAMC) for an Extracorporeal Shock Wave Therapy (ESWT) system. The VA identifies a growing need to treat chronic conditions among aging Veterans, including tendinopathy, plantar fasciitis, nerve injuries, and musculoskeletal disorders through a non-invasive treatment modality. The facility seeks to acquire one modular shock wave generator to be shared across podiatry, urology, and orthopedic departments, with the goal of improving patient outcomes and reducing reliance on long-term pain medication.
The related federal contract opportunity (Solicitation Number 36C25925Q0444) is a small business set-aside procurement for one Extracorporeal Shock Wave System, with delivery expected 60 days after order receipt. The system must have specific technical characteristics, including a focused module, integrated compressor, 15.6-inch touch screen, and electromagnetic functionality. Quotes are due by May 30th, 2025 at 14:00 Mountain Time, with all questions to be submitted by May 23rd, 2025 to the Contract Specialist Peyton Wilkerson. The procurement is being conducted by the Department of Veterans Affairs Veterans Health Administration for the Fort Harrison VA Medical Center in Montana.

STATEMENT OF NEED
Montana VAMC -New Contract or Emergency-
REQUIREMENT
-Reason for requirement (SBAR):
**Situation:** The average age of our Veterans is increasing and a large number of them present with chronic issues including achilles and shoulder tendinopathy, plantar fasciitis, carpal tunnel, sciatic nerve injury, fracture nonunions (failure of bone fractures to heal), wound healing, chronic prostatitis, erectile dysfunction and musculoskeletal disorders such as arthritis. Due to these factors, there is an increasing demand for conservative, non-invasive and cost effective processes to reduce inflammation and pain and promote healing. Billings VA would potentially be able to effectively treat several hundred Veterans per year. Many of the diagnoses this treatment serves are currently only managed with long-term pain medication.
**Background:** Emerging technologies in medical devices have significantly enhanced the modalities of treatments available in podiatry, orthopedics and urology. Due to the average age of our Veterans and the medical issues presented, invasive procedures and surgeries are typically not a solution to their pain. The ability to perform an established treatment modality (focused shockwave therapy) that is non-invasive is critical for improving patient outcomes and quality of life and reducing chronic pain for our Montana Veterans. Our department has experienced a steady increase in patient volume, necessitating the adoption of advanced tools that facilitate long term effective treatment strategies that are non operative.
**Assessment:** After a thorough evaluation of our operational needs and patient care objectives, it is clear that incorporating a new and different technology into our practice is imperative. The current inability to effectively treat and manage these chronic issues of our aging Veterans results in increased risks of complications due to over medication and subpar modalities. Additionally, a lack of adequate equipment hampers our training and educational initiatives for medical staff, inhibiting their development and familiarity with cutting-edge techniques now standard in the field. Therefore, investing in a new shock wave system aligns with our commitment to excellence in patient care and continuous professional development.
**Recommendation:** To address these pressing needs, we recommend the acquisition of a high-quality modular shock wave generator that offers a conservative, non-invasive modality which produces long lasting results to our patients. We propose obtaining one unit which can be shared between our podiatry, urology and orthopedic departments. This investment will not only improve patient outcomes and satisfaction but also establish our facility as a leader in patient care practices within the region. Thus, we urge decision-makers to prioritize this purchase to uphold our mission of providing the highest standard of care.
