Statement of Work For Optical Comparator Preventive Maintenance, Calibration, and Emergency Repair

1. SCOPE: Provide preventive maintenance, calibration, on-site emergency repair service for four (4) each Government owned Optical Comparators Two (2) Model Numbers QL-30, and two

(2) Model Number FC-300. Period of performance is for 1 base year plus four (4) option years.

The machines are located at Anniston Army Depot, 7 Frankford Ave, Anniston, Al. 36201- 4199.

2. REQUIREMENTS: The estimated price shall cover the cost of providing all things necessary to provide on-site emergency repairs and calibration services for the Optical Comparators, including labor, diagnostic and calibration, equipment, special tools, documents, transportation, schematics, common tools of the trade, etc. Contractor personnel must be manufacture certified with hands-on experience in operating, servicing, calibrating, and maintaining the Optical Comparator, and shall be capable to perform all services necessary to bring this equipment to 100% production capable readiness.

3. PRINCIPAL PERIOD OF MAINTENANCE (PMM): PMM shall consist of 9 hours on weekdays Monday – Thursday, 7:00 am – 12:00pm and 12:30pm – 4:30pm, and 8 hours on alternating Fridays 7:00 am – 12:00pm and 12:30pm – 3:30pm. No hours on alternating off Fridays, Saturdays, Sundays, or Government observed holidays.

4. CALIBRATION SERVICES: Each Optical Comparator shall be calibrated once a year in accordance with procedures outlined by the Original Equipment Manufacturer (OEM). Contractor personnel will be required to perform preventive maintenance checks in conjunction with calibration services and shall consist of all things necessary to reduce equipment maintenance down town including inspection for worn or damaged parts, cleaning, and adjusting when needed.

Upon completion of services, contractor personnel shall perform equipment operational checks/self-test procedures designed to and verify that the equipment is 100% operational and ready for government use. All equipment used to calibrate the Optical Comparators shall be certified with standards traceable to the National Bureau of Standards (NBS). A certificate of compliance for each Optical Comparator must be issued to the Government upon completion of calibration services.

5. EMERGENCY REPAIRS: The contractor shall perform on-site emergency repairs for the Optical Comparators on a high priority basis during the PPM (Ref. Para 3). All associated costs including travel. Labor, mileage, repair parts, etc., must be invoiced to the Government and shall be consistent with the contractor’s quote and service rates. Only new OEM repair parts shall be used to repair the Optical Comparators. All replacement parts or components shall remain the property of the Government.

6. CONTRACTOR’S POINT OF CONTACT: The Contractor shall provide the Contracting Officer with a point of contact, e-mail address and a telephone number for the Government to report service calls, and shall make arrangements for qualified service personnel (e.g. qualified technician) to receive notification and respond to the Government within twenty-four hours. Telephone recording devices and/or voice-mail systems are acceptable. The Contracting Officer shall supply the Contractor with the name and telephone number of his/her designated technical representative. The COR is authorized to request maintenance support services. The Contractor shall not proceed with any verbal or written change(s) to the scope of work from sources other than the Contracting Officer, who will provide changes in writing.

7. CONTRACTOR MANPOWER REPORTING:

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Anniston Army Depot via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil/, and then click on “Department of the Army CMRA” or the icon of the DoD organization that is receiving or benefitting from the contracted services.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. 8. Access and General Protection/Security Policy and Procedures:

A. This standard language is for Contractor employees with an area of performance within an Army-controlled installation, facility, or area. The Contractor and all associated subcontractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services, or Security office. The Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA, and/or local policy. In addition to the changes otherwise authorized by the change clause of this contract, should the Force Protection Condition http://www.ecmra.mil/

(FPCON) at any individual facility or installation change, the Government may require changes in Contractor security matters or processes.

B. For Contractors that do not require a Common Access Card (CAC), but require access to a DOD facility or installation. Contractor and all associated subcontractor employees shall comply with adjudication standards and procedures using the National Crime Information Center interstate identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014- 05/AR 190-13), applicable installation, facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

C. If necessary, Contractor shall acquire and return all issued U.S. Government Common Access Cards, installation badges, and/or access passes.
