STATEMENT OF WORK
FOR
350th SPECIAL WARFARE TRAINING SQUADRON (SWTS) Bldg. 5570
JOINT-BASE SAN ANTONIO, LACKLAND
1.0 GENERAL INFORMATION
1.1 SCOPE: The contractor shall provide all products, materials, equipment, labor, warranty, transportation and delivery of equipment to Special Warfare Preparatory Course, 2239 Walker Ave, Building 5570, JBSA-Lackland, TX-78236 to accomplish this project in accordance with the Statement of Work. The technical equipment shall perform in accordance with applicable federal regulations and Air Force instructions and meet the approved use list specifications.
1.2 BACKGROUND: The SWTS requires the acquisition of a dedicated fitness facility to enhancing the operational readiness, human performance optimization, and overall well-being of the 350th SW Personnel. The proposed gym will provide a comprehensive resource for maintaining peak physical fitness essential for the demanding duties of special warfare operators.
Additionally, the facility will foster unit cohesion and morale by offering a shared space for physical training, team-building events, and execute individualize physical therapy events. It also serves as a center for injury prevention and rehabilitation, supporting troop health, and enhancing operational capacity. By prioritizing this facility, we underscore our commitment to a culture of fitness excellence, reflecting our dedication to the physical and mental fortitude of every member of the 350th SW Personnel. The purchase of this gym is ensures immediate access to crucial fitness and wellness resources that bolster readiness, enhance morale, and promote a robust, health-focused military community. The 350th courses and POI’s include exercise events that require the equipment listed below in order to execute the curriculum as required. The production requirements for each course rely on this dedicated facility to accommodate these course needs.

2.1 CONTRACT MANAGEMENT
2.1.1: The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, codes, permits, DoD, AF, and base requirements and instructions required for the performance of the duties in this SOW.
2.1.2: The contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. These policies shall be in compliance with Federal and Installation policies and procedures.
2.1.3: The contractor shall manage ordering, shipment and delivery of all new equipment from manufacturer to final acceptance by the designated Government Acceptor.
2.1.4: All inquiries regarding the status of the project or timeline shall be responded by a contractor representative within two (2) business days.

2.1.5: The contractor shall be responsible for coordination of an escort through any controlled areas through the assigned Government Acceptor.
2.2 WORK HOURS AND SPECIAL CONDITIONS: The project will be completed between the hours of 0730 through 1730 every Monday through Friday until project completion. If the project takes place on a federal holiday the contract can complete the work the following day that doesn’t fall on a federal holiday. Due to the amount of personnel moving around the compound contractor will ensure to be aware of their surroundings to ensure no safety mishaps incur.
2.3 PLACE OF PERFORMANCE: 2239 Walker Ave BLDG 5570 Lackland AFB, TX 78236
2.4 SITE SURVEYS: Site Survey is not required.
2.5 START-UP MEETINGS: Start-up meetings are not required.
3.0 REQUIREMENT
3.1: DELIVERY SERVICES: The contract project manager shall be responsible for on-site coordination of all staging and deliveries of their product, to include access to and operation of any equipment needed for unloading and movement of materials or product. The delivery POC is:

3.1.1: Contractor shall be responsible for determination of the appropriate delivery method and handling, based on site conditions to include the non- existence of loading docks and equipment at any given location. Special circumstances may be identified by and negotiated with the Contracting Officer.
3.2 BASE/INSTALLATION SPECIFICS:
3.2.1 All commercial trucks must enter through Growden Gate which is off of HWY 90 and ACME Rd, for inspection. In accordance with the Installation JBSA-Lackland, 802 SFS base entry procedures must adhere to delivery vehicle process requirements mandated by the Wing. Once at Growden Gate, the driver must be able to show:
• A current Vehicle Registration
• A current Proof of Insurance
• A valid and current Driver's License for the driver
• A valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.
3.3 GATE PROCEDURES: Once at Growden Gate be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination. Arrival at the appropriate gate after the designated allotted time will result in non-admittance to the installation. This procedure will occur each time a commercial truck requests access to the base.

3.3.1: Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).
3.3.2 Personnel should have the following items when entering an installation:
• Current Registration for all vehicles
• Current Proof of Insurance for all vehicles Valid driver’s license for each driver
• Valid driver’s license or other state identification for each passenger
• Depending on the current Force Protection Condition, a government representative may be required to sponsor those people in person at Pass & ID/Visitor Center (Military Drive).
• The sponsor must state the guest's purpose and destination. It is the responsibility of the contractor to validate this need with their government POC prior to their arrival so as to coordinate the arrival time accordingly. Sponsors may request that a pass be applicable for multiple days if needed.
3.3.4 Firearms may not be brought on base under any circumstances. Individuals with criminal records are not allowed on base. Contracted personnel are prohibited from sponsoring guests on the installation.
4.0 CONTRACTOR REQUIREMENT: Coordination and management of all new product, equipment, and people required to create and deliver equipment within an Air Force location. This includes ongoing servicing of any items defective due to manufacturing error as well as for replacement or repair of any damaged item after purchase. Additional information can be found in section 2.1.
4.1 SECURITY REQUIREMENTS: The work being completed at building 5570 will be unclassified. Base access is required which is covered in section 3.3. At building 5570 parking is limited but can be shuffle around to make space for contractor and crew to complete project requirements.
