Statement of Work For
Installation of Furniture in a Military Facility Kadena AB, Okinawa, Japan
718 AMXS
20 June 23
1. Description of services - The contractor must provide all personnel, equipment, tools, materials, supervision, transportation, safety equipment, any other items and services necessary to perform installation of shelving as defined in this Performance Work Statement (PWS) in a military hangar located on Kadena Air Base, Okinawa, Japan.
1.1 Existing Shelving
1.1.1 Existing shelving must be destroyed and disposed of properly at an off base location. NOTE: (The government must not be held liable for any expenses that the contractor may incur due to disposing furniture off base.)
1.1.2 Liability for damages – The contractor must be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities and Government owned property.
1.2 New Furniture
1.2.1 The contractor must properly assemble and install all shelving into the government facility and rooms.
1.2.2 Liability for damages – The contractor must be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities, Government owned property and Government purchase furniture.
1.2.3 Contractor is responsible for door to door delivery, unloading, assembly of shelving in the designated room, and removal/disposal of rubbish and old shelving.
1.2.4 Contractor is responsible for processing all custom forms, original customs forms shall be submitted to 18 Contracting Squadron for certification.
1.2.5 Contractor will coordinate schedule with 961 AMU, based on facility construction and beneficial occupancy date. Government will provide storage for shelving should beneficial occupancy date change.
1.3 CLEANING AND PROTECTION
1.3.1 Maintain Walk Through - The contractor must maintain common walk through areas to allow proper evacuation of the building in the case of emergencies.
1.3.2 Remove Rubbish - The contractor must remove rubbish, wrapping paper, salvages and scrap. The contractor must not leave waste material inside or outside of the facility overnight, or for a prolonged period of time, when work is not done. Remove and dispose of waste materials only at authorized off base locations, daily.
1.3.3 Clean Floors - The contractor will clean the government area that they were working at the end of each day.
1.3.4 Final Cleaning - The contractor must remove packing material, boxes, broken items, spots, smears, and other foreign matter.
1.3.5 Liability for damages – The contractor must be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities, Government owned property and Government purchase furniture
2. CONSERVATION OF UTILITIES
2.1 Utilities. The government will provide water and electricity, when available.
2.2 Conservation of Utilities. The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions, which prelude the waste of utilities, which shall include:
2.2.1 Lights shall be used only in areas where and when work is actually being performed.
2.2.2 Water faucets or valves must be turned off after the required usage has been accomplished.
2.2.3 Government or private telephones shall not be used for personal reasons or for any toll or long distance calls.
3. GENERAL INFORMATION.
3.1 HOURS OF OPERATION. The contractor must perform the on- site services required under this contract during the following hours: 08:00 am- 16:30 pm, Monday through Friday, except on U.S. holidays specified in section 3.2.1 of this SOW and PACAF Family days. No on site work will be scheduled or performed on holidays unless previously approved or directed by the Contracting Officer. If the contractor desires to work on non-scheduled workdays, he must submit a written request to the POC for approval.
3.1.1 Holidays - The contractor is not required to provide service, except as noted on the following days:

If holiday fall on a Saturday, it will be observed on the preceding Friday. If it falls on a Sunday it will be observed on the following Monday.
3.2 SECURITY REQUIREMENTS - The contractor must be responsible for safeguarding all contractor controlled government property at the close of each work period.
3.2.1 Identification and Base Passes – The contractor shall obtain identification cards and base passes for all employees and vehicles. Application shall be coordinated through the Contracting Officer or designated representative. Upon termination of employment, the contractor shall return identification cards and base passes to Security Forces within 24 hours.
3.2.2 Safety and Fire - The contractor must comply with applicable government published safety requirements and regulations which will be available upon request by the contractor.
3.3 CONTRACTOR PERSONNEL REQUIREMENTS.
3.3.1 Contract Manager - The contractor must provide a contract manager who will be responsible for the performance of the work. The name of this person, and an alternate(s), who will act for the contractor when the contract manager is absent, must be designated in writing to the CO 5 working days prior to start of projects.
The contract manager or alternate will have full authority to act for the contractor on all contract matter relating to the daily operation of this contract. The contract manager or alternate will be available during normal duty hours with sixty (60) minutes to meet on the installation with the government personnel (designated by the Contracting Officer) to discuss problem areas. manager or alternate must be available within two (2) hours. The contract manager or alternate(s) must be able to read, write, speak, and understand English.
3.3.2 Contractor Personnel - Contractor personnel must present a neat appearance and be easily recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contains the company and employee name in English. Contractor personnel must use protective clothing whenever they are performing tasks that could cause harm to the employees or damage to property.
3.3.3 The contractor must not employ persons to work on this contract if such person is identified to the contractor by the Government as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population.
3.3.4 Smoking – Smoking is not permitted in any government building or work area.
3.3.5 Employees – The contractor is cautioned that off-duty active military personnel hired under this contact may be subject to permanent change of station, change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt of these personnel could adversely affect the contactor’s ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.
3.3.6.1 The contactor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the US Government. Wither military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7, Joint Ethics Regulation. The contactor shall not employee any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106, Air Force Industrial Labor Relations Activities.

GOVERNMENT SERVICES
1. Government Services. The Government will provide water and electrical power when available to the contractor.
