Statement of Work
For the
Install Fall Protection System in
East and West Maintenance Bays of Bldg. 1065 In accordance with O.S.H.A. and A.N.Z.I. standards

Statement of Work This proposed Statement of Work (SOW) is for the specifics listed below to the installation of four two-person passable fall protection track systems. (In accordance with attached guidelines) for Bldg. 1065. Unless otherwise specifically provided, all of the terms and conditions of this Agreement are incorporated within or by attached reference and shall specifically apply to this SOW.
Description of Services The Contractor will:
A. Engineer integrate and install four two‐person passable track system
B. Tracks will be of high carbon steel and have a baked powder coat
C. Tracks will be supported from the existing building bar joist, tracks will not interfere with existing lighting
D. Furnish eight cable style class “B” self‐retracting lanyards with four 5 point padded full body harnesses
E. Provide a training class to cover pre‐use inspection of system, inspection and operation of a self‐retracting lanyards and proper donning and inspection of full body harnesses
F. Provide one year warranty on all labor and material
G. Perform first annual inspection in the 12th month of service
Out of Scope Any other tasks not included in this Statement of Work will be considered out-of-scope and will require approval from listed Project Manager.
Project Management Tommy Branscum will be the points of contact and Project Managers for the duration of this
SOW.
The Company’s Responsibilities Throughout the Project the contractor will:
Provide a single point of contact for all project support issues and designate a backup when the primary contact is unavailable.
Coordinate the project plan and schedule with Project Manager.
Provide project status updates to project manager.
Complete the steps outlined in the Description of Services section, above.

MacDill Air Force Bases Responsibilities Throughout the Project the Air Force Project Managers will:
Designate a person, who shall be identified as the Air Force project manager and shall designate a backup for when the primary project manager is not available.
Provide e-mail and valid phone access to project managers throughout the project.
Respond within one business days to the contractors request for information required for the project.
