STATEMENT OF WORK
FOR PURCHASE, DELIVERY, INSTALLATION.
JBSA-FORT SAM HOUSTON BLDG 4021 DORM ARTWORK
1. DESCRIPTION/GENERAL INFORMATION.
1.1 Contractor shall provide all management, tools, supplies, equipment, labor and transportation necessary to provide. (116) Artwork A and B 3 Single Matting Plexie Glass, Wood Frame with Security Hardware. (73) Artwork C 3 Single Matting Plexie Glass, Wood Frame with Security Hardware. All materials and accessories shall be installed in accordance with (IAW) industry standards, manufacturer’s recommendations and this Statement of Work (SOW).
1.2 The Contractor shall make adjustments without impact to the user during government critical mission training.
1.3 The Contractor shall confirm delivery 15-days prior to delivery and provide a 24-hour notification prior to truck arrival. The Contractor shall provide an email notification as to the date and time the trucks will be arriving to both Technical Representatives (TR):

2. Project Location: There is one location:
1. Building 4021, 3280 CPL Johnson, JBSA-Fort Sam Houston TX, 78234.
Building 4021 has 192 rooms and three floors. The building is new construction/ vacant and do not have an elevator and 2 Stairwell(s) to access floors. COVID policies are in place.

3. Project Description:
Artwork to be install with all the hardware included:
The building is vacant. The Contractor shall deliver and install artwork in hallways on each floor throughout the building. The contractor shall complete one floor prior to moving to the next floor. At the end of the period of performance:

HOURS OF OPERATION: The delivery and installation shall be performed during normal duty hours (7:30 A M. to 4:15 P. M., Monday thru Friday). In the event that delivery falls on a federal holiday, the delivery date shall be moved to the following workday.
5. ACCEPTANCE AND INSPECTION:
5.1. The vendor shall manage ordering, shipment, delivery and installation of all new furniture until final acceptance by the designated Government Acceptor.
5.2. The contractor shall deliver all items specified in SOW Order specification sheet deliverables will be new and not used or factory refurbished.
6. WARRANTIES: Contractor shall provide industry standard commercial warranties. A period of not less than one-year from the date of installation and acceptance.
7. WORKMANSHIP: The Contractor shall perform all operations in a professional manner in accordance to industry standards.
8. ACCESSORIES AND INCIDENTALS: Contractor shall provide all incidentals necessary to perform as required in accordance with this SOW. Cost for these items shall be included as part and parcel of the finished installation.
9. QUALITY ASSURANCE: The government shall inspect all work performed by the contractor. The Contractor and TR(s) shall verify and inspect all items to ensure that no damage has occurred to the furniture during loading, transport, unloading, staging and set up. Copies of the discrepancy reports shall be forwarded to the government.
If repair or replacement of damaged items is needed, the contractor will repair or coordinate replacement of damaged, defective, or missing items. The Government shall require repair or replacement of nonconforming supplies or re-performance of non-conforming services at no additional cost. Any disputes in this area must be brought to the attention of the TR(s) and the Contracting Officer immediately.
10. CLEANUP: The Contractor shall provide/supplied dumpster. The contractor is responsible for providing any roll-off dumpsters required and responsible for removing as necessary to complete this project; the Government will not provide nor supply any dumpsters or Government sites for this project. The Contractor shall not dispose of or discard recyclables, packing materials or trash in any recycling or trash containers or dumpsters located on the installation; except for the contractor owned containers or dumpsters. The Contractor shall collect all trash, debris, refuse, garbage, etc., generated in performance of entire process and remove and properly dispose items off-site. The contractor shall follow all Local, State, and Federal regulations for the disposal of items.
11. GOVERNMENT PROPERTY: The contractor shall take precautions to prevent damage to government buildings and property. The contractor shall repair or replace damaged items at no additional cost to the government.
12. COMPLETION OF WORK: The contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all relevant materials, finishes, equipment, and systems shall be fully operational at the completion of work for this project. In order for the project to be considered completed, work performance shall be deemed satisfactory and acceptable to the government when the TR and 802 CES/CEIH declares the project satisfactory.
13. ENVIRONMENTAL REQUIREMENTS: The contractor shall perform all work in accordance with federal, state, and local environmental regulations.
14. INSTALLATION ACCESS
14.1. The contractor and its employees shall meet minimum-security requirements in order to gain access on to Joint Base San Antonio - Fort Sam Houston, Texas a ten
(10) - day advance notice is needed for access pass, and any person with recent arrest will not be able to enter the post. A base access pass for an employee without a US Driver’s License of US State issue ID card may take longer.
14.2. The contractor shall obtain all required installation passes for contractor personnel by providing the employees full name, date of birth, Driver’s License/ID information to the Technical Representatives identified in para 1.3. Contractor personnel shall be able to produce personal identification when required by current force protection conditions or special instructions.
14.3. The contractor personnel shall wear an identifiable outer garment (company shirt) or badge which clearly identifies a contract employee. The badge will contain a name of employee, and contractor's name.
14.4. All contractor vehicles must enter and depart through JBSA -Fort Sam Houston via the Jadwin Gate Access Control Point which can be accessed via Interstate 35. Any questions regarding this route should be filtered the Government POC(s) prior to delivery.
