PSYCHOLOGICAL THERAPY SERVICES FOR JOHN F. KENNEDY DAY SCHOOL
SCOPE OF WORK
BACKGROUND:
The John F. Kennedy Day school services students on the Fort Apache Indian Reservation. The scope of work calls for professional services to assist the school in implementing special education and related services as authorized under P.L. 108-446 Individuals with Disabilities
Education improvement Act (IDEIA). The purpose of IDEIA program is to provide a free appropriate public education to students who have an identified disability and by reason thereof require specialized instruction including a related service to meet their unique education needs.
John F. Kennedy Day School is located in a semi-remote area on the Fort Apache Indian
Reservation, in a community called Cedar Creek, 18 miles south of Whiteriver, Arizona.
PSYCHOLOGY THERAPY SERVICES:
The contract will provide all necessary certified and/or professional licensed psychologist(s) to provide school based mental health services within a comprehensive educational program. In addition, the contractor will provide all necessary supplies, materials, equipment and testing materials to perform the work as described below.
WORK REQUIREMENTS
After being provided a copy of the parental permission form signed by parent/guardian, and/or teacher referral for testing based on MTSS (Multi-Tiered System of Supports) or SAT
(Student Assistant Team) the contractor shall:
A. Conduct psychoeducational evaluations to identify various disabilities in accordance with Individuals with Disabilities Act of 2004 (IDEA) and Department of Education (ADE) guidelines.
B. Once the evaluation has been completed, generate a report with findings and provide recommendations and interventions appropriate to address learning and/or behavioral needs of the student.
C. Interpret assessment data and share with parent/guardian and team members during
MET meeting.
D. Results will be shared with special education teacher/coordinator to assist in the development of an Individualized Education Plan (IEP) or 504 Plan if needed.
E. Lead the multi-disciplinary evaluation team (MET) in discussion of evaluation results.
F. Maintain records of work performed for monthly invoicing
a. The contractor will be expected to sign in and keep a log regarding the actual duration of therapy session of individual or group nature.
b. The contractor will maintain mileage log for each trip.
c. The contractor will be paid for therapy services on a monthly basis. Invoices shall be submitted by the contractor directly to the contracting office and school serviced.
G. The contractor MUST sign in at the front office and get a visitor’s badge and sign out when services are completed.
H. The contractor (Psychologist) must provide a copy of their Arizona State Certification for Psychology.
REPORTS:
All evaluation reports shall contain information required to remain in compliance with all pertinent regulations mandated by Federal or State Laws including IDEA and
Educational Standards for the state of Arizona regarding Special Education Program
Students. The reports may be hand delivered or mailed with the original protocols to the school special education coordinator.
The contractor is also required to log in and out of the school site and to schedule herself/himself for each individual child’s IEP committee meeting at the school’s request. Weekly schedules shall be approved by the school’s special education coordinator.
EVALUATION REPORT OF PROGRAM EVALUATION:
All reports should contain all necessary information required to remain in compliance with any and all pertinent regulations mandated by Federal or State Laws including IDEA and the Indian School Equalization Program’s guidelines and criteria.
Evaluations should include cognitive and academic assessments, such as the Kaufman Assessment Battery for Children (KABC-2) for cognitive, and the Woodcock-Johnson Test of Achievement (WJ-4) for academics.
CONFIDENTIALITY:
All materials furnished or produced by the contractor under this agreement, including but not limited, to documents, reports, and correspondence will remain CONFIDENTIAL.
The contractor shall not release information to any persons, group of persons, firm or organizations other than the Bureau of Indian Education, John F. Kennedy Day School with information related to work performed, research or information derived from work performed under this agreement. Breach of confidentiality shall be denied substantial breach of this agreement and may be grounds for rescission.
IDENTIFIED STUDENTS:
There are an estimated 24 evaluations including re-evaluations at John F. Kennedy Day
School.
PROFESSIONAL STAFF REQUIRED:
The position of Psychologist (service provider) is required to complete this project. The
Psychologist must be licensed in the State of Arizona.
