CARPET

INSTALLATION

Statement of Work (SOW)

For

Removal & Carpet Installation

SCOPE OF WORK:

The contractor shall furnish all labor, tools, materials, permits, and equipment necessary to perform the work.

B1947- Work includes removing existing carpet, existing cove base, existing transitions, and installing modular carpet squares (approximately 2,000 sq ft), applying vinyl cove base (approximately 2,500 linear feet), and applicable transitions in B1947. The contractor is responsible for ensuring the actual/correct amount of carpeting required.

Color choices are as follows:

Milliken- Southern Analogue 19.7x19.7 solution-dyed nylon

Pinhole color- PH418 Zoom carpet tile

The contractor shall also furnish all labor and tools to disassemble, remove and upon carpet installation re-assemble existing furniture to include desks, wardrobes, and theater chairs. The contractor is responsible for measuring carpets to ensure the exact amount of carpet and cove base is required to complete this installation. Installation includes delivery, uncarting, set-up and debris removal. The measurements given are an estimation. Offers shall include, but not be limited to: All interior finishes, covers, carpet, and cove base.

The government requires the contractor to visit the site location and take their own measurements (dimensions, quantities, and square footages) prior to contract award all dimensions, quantities, and square footages included are approximations. We are not responsible for incorrect measures of the amount of carpet required. The extra carpet tile shall be given to the facility manager after the project is complete. The contractor shall haul away and dispose of removed carpet, base cove, and transitions.

a. Contractor requirements for carpet replacement: Carpet to be purchased and installed shall be as noted above, lifetime commercial limited warranty, from the 7 predetermined government sources.

b. Contractor is responsible for purchasing and installing transitions to be replaced in all applicable doorways where carpet tiles are installed. The transitions shall match transitions or approved equal.

c. The existing floor surface shall be suitable for installation after the old carpet is removed but if the floor needs filling of holes or cracks the contractor shall be responsible for making the carpet ready. Contractors shall furnish all cement, tape, thresholds, and cap down moldings. Carpet to be installed in accordance with the manufacturer’s instructions. The contractor shall furnish all adhesive as recommended by carpet manufacturer. The contractor shall be responsible for the preparation of floor surfaces as necessary to receive carpet. All surfaces to be free of voids and protrusions. The installed carpet shall be smooth, uniform, secure, and installed in a professional manner.

Patterned goods shall be accurately matched. Fit cutouts such as doors, jambs, columns, steps, landings, and ducts neatly and securely.

d. Extra attention is to be observed while butting seams together. Contractors shall follow manufactures instructions on in matching the seams to prevent fraying and raveling and for unity in pattern. The contractor is to provide at least a five-year warranty against fraying, raveling, separating, buckling and on his workmanship.

e. Phasing of work may be required to support the mission of the facility. The

Facility Manager will be the Point of Contact. Contract performance for each phase (if required) will start after the issuance of an NTP for that phase.

f. The Contractor is responsible for protection of existing facilities equipment from damage during removal of existing carpet and installation of new carpet.

Contractors should take into account the special treatment of hazardous waste, as well as the environmental implications related to building demolition management (i.e. fluorescent lighting tubes, t-stats, etc.). Every effort shall be made to recycle eligible materials, and those not suitable, shall be disposed of off base in accordance with all (SCDHEC) South Carolina Department of

Health and Environmental Control regulations.

g. Ensure that any demolition, utility cut and capping, and debris removal service performed at Shaw AFB, South Carolina in completed in such a manner that will restore a neat and professional appearance of base areas.

h. The facility/area shall be cleaned in its entirety, including exterior areas, before being returned to the government.

i. During Performance, the contractor shall take care not to damage existing structures, etc. that will not be replaced as part of this contract. The contractor shall be responsible for the temporary support of all the devices required to perform work. Any demolition work that needs to be accomplished to complete the contract is the responsibility of the contractor. All material shall be installed per manufacturers’ recommendations. All items listed in the SOW are to be supplied and installed by the contractor unless otherwise noted.

j. The Contractor shall take all precautions to ensure that his operations are conducted in a manner as to minimally interfere with the normal operations of the base and the safety and convenience of the base personnel. The contractor shall coordinate utility outages with the Civil Engineering Squadron

(CE) service call and the Government representative.

GENERAL INFORMATION:

The contractor shall provide all management, tools, supplies, equipment, labor, and applicable licenses and permits necessary to complete the requirements within. All work shall be done in strict accordance with this Statement of Work and subject to the terms and conditions of the contract and in accordance with United Facilities Criteria

(UFC), Unified Facilities Guide Specifications (UFGS), the International Building Code

(IBC), International plumbing code (IPC), South Carolina Department of Health and

Environmental Control (SCDHEC), National Electrical Code (NEC), National Electrical

Safety Code (NESC), National Fire Protection Association (NFPA), EPA 402-K-01-001

(2008), South Carolina Department of Transportation (SCDOT), Shaw Air Force Base

Design and Construction Standards, Change 2, 21 Aug 2018 and all other applicable codes.

Contractor is responsible for verifying all dimensions, quantities and square footages.

All dimensions, quantities, and square footages included are approximations.

Construction and demolition shall be in strict accordance with the Shaw Air Force Base

Design and Construction Standards. The information provided in this Statement of

Work (SOW), the drawings provided, and the requirements of the Shaw Air Force Base

Design and Construction Standards shall be determined to be the minimum standards required in this project.

The contractor shall video all areas in which work should occur prior to starting work to include interior and exterior of the project site. UFC’s take precedence over all standards, codes and Shaw AFB Design Standards and requirements that pertain to the work within this SOW.

During construction, the contractor shall take care not to damage existing structures, etc. that will not be replaced as part of this contract. The contractor shall be responsible for the temporary support of all the devices required to perform work. Any demolition work that needs to be accomplished to complete the contract is the responsibility of the contractor. All material shall be installed per manufactures’ recommendations. All items listed in the SOW are to be supplied and installed by the contractor unless otherwise noted. The government will periodically evaluate the contractor’s performance to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor’s quality control program and receipt of complaints from base personnel.

The contractor shall notify the equipment owner of any repairs required that are not listed in the scope of work and if any of the additional repairs would need to be completed before continuing with the maintenance outlined in the scope of work. No additional work will be approved unless coordinated through the Contracting Officer’s

Representative (COR). The contractor shall notify the COR, 20th Civil Engineer

Squadron at 895-9713 or 897-9716 upon completion of all work for this contract.

Normal working hours are 7:30 AM to 4:30 PM, Monday through Friday, excluding US

Government holidays. Work to be performed at all other times shall be approved by the

COR’s Office. Emergency contact information will be provided to the contractor.

Buildings will be occupied during all work activities. The contractor shall coordinate all work with the COR to ensure that aircraft and vehicle maintenance operations are not impaired.

The contractor shall take no longer than 15 business days from beginning of installation to completion of insulation.

Local CONS will require the Tier II Contractors (Carpet Installers or Local Dealers w/

Installers) to contact the Tier I Contractors for material quotes and orders. The Tier II contractor can input the price for the carpet and adhesives being utilized from the Tier I contractor so that the CONS can verify that the price is at or below the NTE price.

NOTE: Contractor mark-up/profit are not allowable on items purchased via the Tier I contracts.

INSTALLERS: Choose one of the 7 carpet selections and negotiate with the Tier I contractors. Once the negotiation is complete, the installer shall provide a quote to

CONS that includes the price for the carpet and adhesives per square foot, so that the

CONS can validate those prices are at or below the NTE prices set in the Tier contracts.

PERMITS:

The Contractor shall pay for and obtain any required permits. The contractor shall act in accordance with this Statement of Work, SCDHEC, Federal, and Shaw AFB guidelines.

WORKING CONDITIONS:

The contractor shall take all precautions to ensure that his operations are conducted in a manner as to minimally interface with the normal operations of the base and the safety and convenience of the base personnel. The contractor shall coordinate utility outages with the Civil Engineering Squadron (CE) service call and the user. The site must be kept neat, clean, and free of debris with a complete general clean up at the end of the project. Contractors shall restore work areas to a manner that presents a neat and professional appearance. Contractors shall clearly mark work areas using signs, reflective tape or cones.

DEBRIS HAULING AND DISPOSAL:

Contractors shall use an approved hauling route to prevent interference with base operations. In order to prevent problems with traffic, noise, wear and tear on roads, or other problems having the potential to adversely affect health, safety, or the environment, the government reserves the right to regulate routes, intervals, delivery points, and times for hauling. All debris hauled by the contractor shall be enclosed to prevent leaking, spilling, or blowing. The Contractor shall ensure all debris or dirt is promptly removed from any transportation route if such debris is spilled, blown, or leaked from hauling equipment and vehicles. The contractor shall remove all dirt and mud caused by tracks of hauling equipment within one hour of notification by the government. All demolition debris collected by the contractor shall be housed dumpsters and covered at all times and disposed of in an authorized landfill or transfer station. No disposal or burning is allowed on base. The contractor shall provide the government inspector with an electronic copy of each dump receipt on the following workday by 0800 with the daily report.

SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS:

The following security requirements are required in Shaw AFB Statement of Works

(SOW’s) and Performance Work Statements (PWS’s) when contract work does not involve access to classified information.

Listing Of Employees: The contractor shall maintain a current listing of employees.

The list shall include the employee’s name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee’s status or information changes.

Security Training: The contractor ensures all contractor employees receive initial and recurring security training from the sponsoring agencies security manager. Training must be conducted IAW DOD 5200.1-R, Information Security Program Regulation, and

AFI 31-401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air

Force Installation Security Program.

Pass And Identification Items: The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. Contractors cannot sponsor other contractors on base. Only government employees (military or civilian) are authorized to sponsor contractors.

NOTICE: All contract employees shall submit to a criminal background check accomplished by Shaw Air Force Security Forces prior to the employee being issued base access credentials. Failure to submit requested information as indicated below can result in denial or delay in issuing base access credentials.

Retrieving Identification Media: The contractor is responsible for retrieving all government-issued identification media from employees who depart for any reason before the contract expires, e.g. terminated for cause, retirement, etc. The contractor shall return retrieved items to the Security Forces Pass and Registration Office.

Traffic Laws: The contractor and its employees shall comply with base traffic regulations.

Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw Air Force Base.

For Official Use Only (FOUO): The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

Physical Security: The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

Controlled/Restricted Areas: (IF APPLICABLE) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a

Restricted Area Badge will be issued. Contractor employees must have a completed

National Agency Check, Local Agency Check, Credit Check (NACLC) investigation before receiving a Restricted Area Badge.

Key Control: The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized people. The contractor shall not duplicate any keys issued by the government.

a. The contractors shall immediately report to the COR or Program Manager any occurrences of lost or duplicated keys.

b. In the event if keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government.

The government may, however, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

c. The contractor shall prohibit the use of keys issued by the government by any person other than the contractor’s employees and the opening of locked areas by contractor employees to permit the entrance of people other than contractor employees engaged in the performance of contract work requirements in those areas.

SHAW AFB HAZARDOUS MATERIAL AND ENVIRONMENTAL POLICIES:

Air Force Manual 32-7002, Environmental Compliance and Pollution Prevention, requires that all Hazardous Materials (HM) used on the base will be tracked by the HM database, EESOH-MIS (AF Enterprise Environment, Safety, and Occupational Health

Management Information System). This includes HM used by any Contractor while working on Shaw AFB property. Prior to using any HMs on base, the Contractor shall provide the following information to HAZMART (bldg. 231, phone 803-895-

9931/9932/9933) for authorization prior to use: Base Project Name, Contract Number, HM Use Location(s), Safety Data Sheet (SDS), Material Container Size, and Usage

Data. (such as 1-gallon per week or 3-pints per month, etc.). This requirement applies to all contractors, including construction, performing studies or tests and service contractors, such as janitorial and maintenance. All SDS’s and quantities used are required to be submitted to HAZMART (bldg. 231, 895-9931/9932/9933) for authorization and approval prior to use. This shall be done PRIOR to the start of this contract. If you require assistance, please contact HAZMART at (803) 895-

9931/9932/9933. The generation of hazardous waste is not expected from this contract;

however, it is the responsibility of the contractor to determine if hazardous waste is generated from contractor operations. Shaw AFB Environmental Compliance (20

CES/CEIEC) will assist with determination if requested by the contracting officer. Proper disposal of any Hazardous or Universal waste generated by the contractor is the responsibility of the contractor. All such waste must be removed from the installation before the end of the contract period of performance. The generation of universal waste

(including but not limited to mercury containing light fixtures) is probable under this contract. The contractor shall manage and dispose of all universal waste in accordance with South Carolina Department of Health and Environmental Control (SC DHEC) and

40 CFR Part 273, Universal Waste Rules. Fluorescent and HID lamps (including CFLs), ballasts, thermostats, smoke detectors, mercury-containing equipment, emergency light batteries and other potentially hazardous waste will be removed, packaged and stored on site until ready for disposal in accordance with applicable regulations under

Resource, Conservation and Recovery Act (RCRA), Toxic Substances Control Act

(TSCA), and SC DHEC regulations. The contractor shall ensure that all personnel involved in the generation or handling of any Hazardous or Universal Waste are appropriately trained to safely and legally manage the waste in accordance with the

State of South Carolina and Federal regulations.

DAILY REPORTS:

The contractor shall provide the government with an electronic copy of each dump receipt along with the daily report by 0800 the following workday. All daily reports will be submitted in RAKEN format and shall include pictures of work performed for each day.
