STATEMENT OF WORK
Design-Build Services for Dibe Yazhi Habitiin Community School
Crownpoint, New Mexico Playground Project
1.0 Location
The project is located on the campus of Dibe Yazhi Habitiin Community School. The campus is located on County Road 19, 13 miles southeast of Crownpoint, New Mexico.
2.0 Background
This project will be a design-build project for Dibe Yazhi Habitiin Community School, a K (Kindergarten) through 8th grade school. The project shall consist of design and installation of playground equipment for ages 5-12. The project includes the demolition and disposal of existing playgrounds, two shelters and tables; site preparation for new playgrounds; new ADA/Division of Safety and Risk Management (DSRM) compliant safety surfacing; new shade structures and site furnishings.
3.0 Offeror Requirements
Offerors shall be Prime Contractors licensed by the State of New Mexico and/or Arizona.
Provide a copy of Contractor’s License when submitting a proposal.
Offerors are strongly encouraged to conduct a site visit during the solicitation phase at no charge to the Government. See RFP for scheduled site visit.
Offeror shall be a manufacturer certified installer and have a National Recreation and Park Association certified playground safety inspector on staff.
4.0 Scope of Work
4.1 The design-build contractor shall furnish all labor, tools, equipment, transportation, materials and services necessary for the demolition and disposal of the existing playgrounds: the design and installation of all new playground equipment; new surfacing; shade structures and associated site improvements as specified and described.
4.2 The contractor shall be responsible for the professional quality, technical accuracy, verification of measurements, timely completion, and restoration of the area.
5.0 Applicable Codes and Standards
5.1 U.S. Consumer Products Safety Commission (CPSC) “Public Playground Safety Handbook”.
5.2 ASTM International (ASTM) Standard F 1487 for Standard Consumer Safety Performance Specification for Playground Equipment for Public Use.
5.3 Americans with Disabilities Act (ADA) Accessibility Guidelines for Play Areas.
5.4 Americans with Disability Guidelines for Buildings and Facilities.
5.5 All playground and surfacing are required to be certified by the International Play Equipment Manufacturers Association.
5.6 The construction documents shall comply with current laws, codes, and guidelines contained in the “Safety, Health, and Accessibility Guidelines for Architects, Engineers, and Design Firms”, Indian Affairs Division of Safety and Risk Management, dated May 2020.
5.7 The NFPA 5000, Building Construction and Safety Code applies to this project.
5.8 Fully compliant with Americans with Disabilities Act/Architectural Barriers Act Guidelines
(ADAABAG).

5.11 Design services shall be provided in compliance with the BIA School Facilities Design Handbook and Space Templates, March 30, 2007.
6.0 Project Design Requirements
6.1 Contract awardee shall conduct a pre-work visit and attend the pre-construction meeting.
6.2 The design-build contractor shall provide comprehensive design-build construction documents. The design-build contractor shall use only licensed or registered professional architects and engineers to prepare the drawings and specifications as required for the project.
The drawings and specifications shall bear the registration stamp or seal of the registered professional responsible for the preparation.
6.3 Demolition and disposal of existing play structures, swings, (2) shelters and tables.
6.4. Playground Site Preparation
• Furnish and install compacted aggregate sub-base
• Install slabs, curbing, and sidewalk for per design.
• Provide new playground shading and seating per design.
6.5 Provide a playground for ages 5-12 (brand name or equal)
• Play equipment for the ages 3-5
• Equipment Requested:
Curva Spinner and ReviWheel Spinner
• Alpha Link Towers
• 8’ swing frame with an additional bay. Provide (5) belt seats and (1) molded bucket seat with harness.
• Tender Tuff bench with 2 armrests per design.
• Poured in place surfacing is required with ADA accessibility to the playground and the equipment.
• Provide playground shade and seating per design.
• Poured in place solid pour land surfacing replacing the dirt surface
• ADA accessibility to the play equipment.
• ADA accessible play equipment
• Shade structure or shade cloth over the new play equipment
• Shade structures require engineered and sealed drawings.
• Principal shall make final color selections and approval
• Principal shall approve final play equipment selections and layout
6.6 All shade structures require engineered and sealed drawings.
7.0 Playground Design and Equipment Requirements
7.1 Playground equipment and surfacing are required to be IPEMA Certified.
7.2 Access for maintenance is required with a minimum opening of 10’-0” clear for installation;
12’-0” is preferred.
7.3 Provide a minimum of 8’-0” between playground walls and other items for access.
7.4 Utilities are not permitted to pass through, over or under play areas. This includes water, sanitary sewer, electrical, gas, communication cables, etc.
7.5 Surfaces adjacent to play areas shall be paved or treated to control erosion and for ease of maintenance.
7.6 No site drainage or roof drainage is allowed to enter playground or play equipment containment areas. Play equipment cannot be installed within ponding areas.
7.7 All permanent playground equipment shall be installed within concrete borders with ADA compliant access.
7.8 All new play structures shall include ADA accessible play equipment. Every playground shall have an accessible path leading to the play area. ADA accessibility shall comply with the most current publication of “Guide to ADA Accessibility Guidelines for Play Areas” as published by the Architectural and Transportation Barriers Compliance Board.
7.8.1 All landings and slopes shall be ADA compliant.
7.9 Include a trash receptacle designed for exterior use.
7.10 Arch swings are preferred. If single post swings, they shall be clamp attached, and no more than 3 bays.
7.11 Provide drainage out of the play area.
7.12 Wind screens shall be installed on fencing in potential high wind areas.
7.13 Equipment with built in shade structures or shade structures over large pieces of equipment is preferred. All shade structures require sealed engineer drawings.
7.14 A poured in place surfacing for 8’ Fall Height is required.
7.15 All equipment shall be purchased from a playground equipment manufacturer with adequate product liability insurance. Manufacturers’ representatives shall include a Certified Playground Safety Inspector (CPSI).
7.16 The International Playground Equipment Manufacturers Association (IPEMA) shall certify all components of the purchased equipment.
8.0 Installation Requirements
8.1 Design drawings and specifications shall be submitted to the COR and principal for review and approval. Playground equipment colors to be selected by school representative with the COR providing approval.
8.2 After approval by the COR, design drawings and specifications shall be submitted to the COR for submission to the BIA Division of Safety and Risk Management (DSRM) for review and approval.
8.3 DSRM approved Architectural and Engineering construction drawings shall be used to construct the project. All proposed designs and drawings shall be reviewed and approved prior to ordering any materials and installation.
8.4 Five (5) sets of the STAMPED design documents, full size drawings and specifications, shall be submitted at the 100% completion for review by the school, New Mexico Navajo Central Agency, and BIA DSRM.
8.5 All equipment shall be installed using approved BIA DSRM approved drawings and according to manufacturer specifications and meet current industry standards.
8.6 The installation shall have on-site CPSI certified personnel to oversee the installation.
8.7 No other organization or persons may install playground equipment on school grounds without approval by Bureau personnel.
8.8. Installation shall include a meeting with the COR and key school personnel. The meeting should include information on conducting a team inspection and how the equipment is to be used.
8.9 Provide final as-built drawings and specifications in AutoCAD, latest release software, and submitted on electronic storage. Provide final operation manuals, operating instructions, and other closeout documents.
8.10 Prior to Final Acceptance by the Owner, the Contractor shall provide an "As-Built" drawing which documents that each piece of play equipment is constructed with the required use zone and that no use zones overlap.
8.11 Prior to Final Acceptance by the Owner the Contractor shall provide written certification that each piece of play equipment has been inspected by an independent third-party National Recreation and Park Association certified playground safety inspector, and the construction and installation of each piece of equipment fully complies with the ASTM F-1487-11 and the U.S.
Consumer Product Safety Commission (CPSC) Handbook for Public Playground Safety.
9.0 Pre-Construction Submittals
The Contractor shall submit a Health and Safety plan in accordance with 29 CFR 1910.120, schedule of values and payment and performance bonds.
10.0 Work Schedule
The work shall be performed on Monday through Friday between the hours of 8:00 AM and 4:30 PM local time. Alternate time schedule shall be preapproved by the COR.
11.0 Special Requirements
11.1 The Contractor is responsible and required to pay the Navajo Nation Business Activity Tax.

In addition, local and state gross receipt taxes shall be paid by the Contractor.
11.2 Contractor shall be licensed in the State of New Mexico. Provide a copy of Contractor’s License to the Contracting Officer.
11.3 Temporary Utilities.
Contractor shall arrange with local utility companies for temporary connections and payment arrangements.
11.4 Contractor shall be responsible to contact, schedule and pay for blue staking services for natural gas, electrical, telephone and other utilities not shown on the BIA proposed route map.
In the event a utility line is damaged by the Contractor during the construction period, the Contractor shall immediately repair or replace the damaged utility line at no cost to the Government.
11.5 Any unsafe work practices shall be reported to the COR and Agency Safety Officer, Mr.
Harrison Nez 505.786.6191 for immediate resolution.
12.0 Security Requirements
In accordance with the U.S.C. 3207, Indian Child Protection and Family Violence Prevention Act, 42 U.S.C. 13041, Crime Control Act of 1990, and Department Manual 441 (441 DM 5), Personnel Security and Suitability Requirements, the Construction Foreman and Alternate on the job site are required to undergo a background and fingerprint check.
13.0 Period of Performance
The Period of Performance shall be 190 calendar days from the date of Notice to Proceed.
14.0 Payment and Closeout
Completed work shall be approved and signed off by the COR after the following are completed:
• Initial inspection performed by the COR, Principal, School Maintenance Staff and
Contractor.
• Final inspection performed by COR and DSRM.
• Perform maintenance and operating overview to local school staff
• Warranty and any operating manuals are provided to the COR.
The final invoice shall be submitted in accordance with the contract and certified completion of the project.
