2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

ObservSMART Patient Safety Technology.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

ObservSMART Patient Safety Technology validates patient observation compliance. Patients wear a wristband that syncs to a tablet used by hospital staff while doing their patient checks. Hospital staff need to be close enough to the patient to sync the tablet to the patient wristband.

ObservSMART Patient Safety Technology is proprietary technology to ObservSMART.

4. Description of market research conducted and results or statement why it was not conducted:

Market research concluded that the ObservSMART Patient Safety Technology is proprietary.
