Justification for Limited Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity: Department of Veterans Affairs, White River Junction, VA Medical Center, Surgical Service, 215 North Main Street, White River Junction, VT 05009 is responsible for this proposed contracting action. Purchase Request (2237): 405- 24-2-7922-0011 (Contracting UID:
36C241-24-AP-0896). Network Contracting Office 1 (New England) 215 North Main Street, White River Junction, VT 05009, in support of VISN 1 VA Health Care System.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The Department of Veteran Affairs intends to procure a base plus four options (firm fixed price) purchase order for 3D reconstruction modeling on CT images for VAMC, WRJ.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
This procurement requires the delivery of computerized interactive reconstructions that are unique to our Veteran patients who are being considered for abdominal aortic aneurysm (AAA) repair to help define whether they are candidates for endovascular repair. Furthermore, to determine in the most exacting possible manner, the dimensions of endovascular stent graft shall be required for optimal AAA repair. Computerized interactive reconstructions are also required to be obtained on selected post-operative endovascular AAA repair patients who may have endoleaks or other reasons for which revision of the prior repair may be required. Finally, computerized interactive reconstructions provide unique valuable clinical input for patients who may be candidates for carotid stent placement and other unusual clinical situations. In all cases, the computerized interactive reconstructions allow the provider to obtain extremely accurate centerline measurements and to create model endovascular repairs in a manner and degree of accuracy not available through routine CT scan software. Other than Astute Imaging, the VA has not considered or know of any other vendors who could supply the required items for this procurement.
4. Description of market research conducted and results or statement why it was not conducted:
The estimated value of the proposed action is $ 149,500.00 (5-year contract @$29,000/year)
Vascular Surgeons within the Vascular Surgery component of the Surgical Service at the White River Junction, Vermont, Veterans Affairs Medical Center (WRJ VAMC) require access to a computerized three-dimensional (3D) interactive vascular repair modeling and measurement tool.
Astute Imaging, LLC, located in West Lebanon, NH, currently provides the WRJ VAMC with 3D reconstruction modeling of already obtained computed tomography (CT) images. These

At this time, the VA is unsure if other vendors exist who can supply the aforementioned items.
Scope, Performance Requirements & Deliverables: The original CT datasets for VA patients are generated on station using VA CT scanners. Data will be loaded onto disks and sent to the Contractor for development of the computerized interactive reconstructions in a manner consistent with VA Patient Information Security regulations. The Contractor will utilize proprietary software to transform the routine CT data into a form that allows interactive measurements and treatment device planning ensuring the privacy, confidentiality, and security of patient data in accordance with VA regulations. The Contractor will provide VA staff access to VA Patient data and unique computerized interactive constructions via password protected Internet access. Access to any individu interactive model will be limited to only those provider(s) who originally requested the service.
Market Research conducted: Applying previous contracting knowledge, as stated above as well as applying personal knowledge in procuring items of this type from our WRJ VAMC Vascular Surgeon and Section Chief, David H. Stone. Astute Imaging is the only vendor known to the VA to provide this type of goods. The Contract Specialist posted a Sources Sought Notice on 10 January 2024. The Contract Specialist provided a written d of the requirement including a draft performance work statement. The Source Sought notice yielded no results from industry.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
