About this file
This is a Pricing Schedule attachment for a Request for Quotation (RFQ) solicitation for contracted podiatry services. The schedule identifies the line items and pricing structure that vendors must complete when submitting their quotes to the Department of Health and Human Services Indian Health Service for the Warm Springs Service Unit in Warm Springs, Oregon. The contract is structured as a Firm-Fixed-Price (FFP) purchase order for commercial services, with vendors required to provide pricing for podiatrist services that will support the facility's Podiatry Program and ensure continuity of patient care. The attachment serves as the template vendors must use to submit their cost proposals, which should align with applicable wage determinations and the scope of work outlined in the Performance Work Statement.
The specific line items and pricing details within the schedule are essential for vendors to understand the contract's cost structure and submit competitive bids. Vendors must ensure their pricing submissions address all required services, including patient examination and treatment, diagnostic testing, and specialized care for high-risk patient populations such as those with diabetes requiring amputation risk assessment. This pricing schedule is a critical component of the overall solicitation package and must be completed accurately and comprehensively by prospective contractors to be considered for award.
