About this file

This document is a Quality Assurance Surveillance Plan (QASP) attachment that establishes performance standards and monitoring requirements for support services under a Performance Work Statement (PWS).

The QASP identifies three critical performance objectives with zero-tolerance standards. First, inspection service frequency and certification testing intervals must achieve 100% compliance with all VA Directive and regulation requirements, with no deviations allowed and 100% inspection surveillance. Second, all reports must be accurate, complete, and submitted on time with 100% compliance; no discrepancies are permitted and require 100% inspection verification. Third, all personnel must meet CETA certification requirements 100% of the time with no exceptions, also subject to 100% inspection. The document emphasizes that these thresholds are critical to mission success and that failure to meet the stated standards will result in contract termination for default. All three performance objectives employ identical surveillance methodology requiring full inspection to ensure PWS requirement achievement.

ATTACHEMENT 1

Quality Assurance Surveillance Plan (QASP)

The following Performance Requirement Services Summary identifies the performance objective and performance thresholds for critical tasks associated with providing support services for this requirement. The performance threshold describes the minimum acceptable levels of service required for each requirement. The Performance

Requirements Summary shall not take the place of the PWS. These thresholds are critical to mission success.

Failure to meet the standard will result in termination for default.
