The contractor must provide both scheduled preventive maintenance (semi-annually) and emergency repair services to ensure all equipment operates at 95% uptime during normal business hours (8:00 AM–5:00 PM, Monday through Friday, excluding national holidays). For emergency repairs, the Field Service Engineer must respond with a phone call within 30 minutes of notification and arrive on-site within 4 hours if the issue cannot be resolved remotely. The contractor must supply 24/7 remote technical support and priority monitoring. All scheduled maintenance must follow manufacturer specifications and comply with NFPA-99, UL, OSHA, and VA standards, with detailed PM procedures including equipment cleaning, software diagnostics review, calibration, lubrication, parts testing and replacement, electrical and mechanical inspections, and annual chamber cleaning for steam sterilizers.
The contract covers 34 distinct pieces of equipment located throughout the medical center, including ultrasonic cleaners, sterilizers, disinfectors, warming cabinets, drying cabinets, prep and pack tables, washers, transfer carriages, and software subscriptions. The contractor must furnish all required parts (meeting or exceeding OEM specifications), tools, test equipment, service manuals, and diagnostic software; rebuilt or used parts are prohibited. All service work outside normal hours requires prior COR approval and will be billed at the contractor's prevailing hourly rate. Documentation requirements include detailed Equipment Service Reports for each service call capturing contractor name, FSE name, dates/times, equipment identification, problem description, services performed, and associated costs, with reports submitted via email to the COR as verification following end-user acceptance signatures.

B.2 PERFORMANCE WORK STATEMENT
The Contractor shall provide comprehensive full-service maintenance, routine and emergency repair service for the government owned sterilizers, washers, dryers and other related equipment located at the Salt Lake City VA Medical Center.
Contract service shall ensure that the equipment functions in conformance with the original manufacturer’s specifications and the latest published edition of NFPA-99, UL, OSHA and VA manuals.
All equipment shall be operable and available for use 95% of the normal operating hours of the equipment, which are 8:00 AM-5:00 PM, Monday through Friday, excluding national holidays. Downtime will be computed from notification of the problem during normal work hours. Scheduled maintenance will be excluded from downtime. Operational uptime will be computed on a monthly basis. Repeated failure to meet this requirement may subject the contractor to default actions.
All service/repairs shall be performed during normal working hours unless requested or approved by the COR or designee. Work performed outside of normal working hours at the request of the COR or designee will be billed at the contractors prevailing hourly service rate.
Unscheduled Maintenance (Emergency Repairs) Contractor will provide emergency repair service between regularly scheduled services and calibration. Priority technical support and remote monitoring will be provided 24 hours a day, 7 days a week for the duration of the contract.
The Field Service Engineer (FSE) must respond with a return phone call to the COR or designee within 30 minutes of receipt of telephone notification of an issue, 24 hours a day. If the problem cannot be resolved over the phone, the FSE will arrive on-site and commence repairs no more than 4 hours after receipt of notification that the problem remains unresolved.
The contractor will only use parts that are equal to or exceed OEM specs.
Scheduled Maintenance The contractor shall perform scheduled PM services semi-annually on all equipment unless otherwise noted, ensuring that all equipment performs in accordance with the original manufacturer’s specifications and the latest published edition of NFPA-99, UL, OSHA and VA manuals. An outline of the PM procedures and schedule will be provided to the COR within two weeks of award of contract. Contractor will utilize manufacturer recommended procedures and checklists, documenting work performed and actual values obtained (as applicable) during each PM and will provide this documentation to the COR at completion. PM services shall include, but need not be limited to, the following:

All listed items shall be replaced as needed.
8) Annual chamber cleaning of all steam sterilizers
9) Returning equipment to operating condition in full compliance with specs at completion of PM.
10) Providing documentation of services performed.
11) PM services shall be provided in accordance with the PM schedule provided herein. Any exceptions must be arranged and approved in advance with the COR or designee.
12) All parts, services, manuals, tools or software required to successfully complete the PM schedule are included within this contract unless specifically stated in writing otherwise.

1) Name of the contractor and contract number.
2) Name of Field Service Engineer performing the service
3) Date, times (start and finish), equipment downtime, and hours on site for service call.
4) VA Purchase Order number if service is provided outside of normal working hours.
