1.1 DESCRIPTION OF SERVICES
The Contractor shall provide all equipment, tools, materials, transportation, personnel, labor, management, supervision, and quality control necessary to provide rental toilets, hand washing stations, and servicing of all units at the United States Air Force Academy (USAFA) in Colorado Springs CO, Farish Recreational site in Woodland Park CO, and the surrounding areas.
1.2 ORDERING
The Contractor shall provide to the government a form which allows for the simple ordering of rentals and services. The form shall at a minimum include the services requested, locations of request, frequencies of rentals and/or services, cost breakdown with total cost, and a section for any additional information.
1.3 PROVIDE RENTAL UNITS
The Contractor shall provide portable chemical toilets (regular and ADA compliant) and hand washing stations to be delivered, placed, secured, serviced, and removed, as specified by the Government. The Contractor shall use the location information from the task order in conjunction with any map, sketch or drawing provided by the Contracting Officer’s Representative (COR). At no time shall the Contractor place a unit in a location that differs from the task order, map, sketch, or drawing unless approved by the COR.
The Contractor shall ensure that each unit is secured in a manner that prevents tip overs. All units must be delivered in excellent condition and fully operational. Excellent condition is defined as clean and disinfected, no odor, no visible scratches or graffiti, no mud, dirt, dust, mold, chemical residue, fecal matter, urine, and trash or debris on the inside or outside of the toilet. Fully operational is defined as structurally sound and all toilets must a have fully functioning and securely mounted hand sanitizer dispenser, toilet paper dispenser, door handle/locking mechanism, and a vent. All hand washing stations must have a fully functioning water distribution/drain system, a fully functioning and securely mounted liquid hand soap dispenser, paper towel dispenser and trash receptacle for used paper products. All units shall be in place, secured, in excellent condition, and fully operational no later than 6:00 A.M. on the start date of the requested event. The contractor shall remove all units no later than two (2) business days after the event completion date. If units are placed prior to the event start date, the contractor shall ensure that each unit is in excellent condition and fully operational no later than 6:00 A.M. on the start date of the requested event.
1.4 PROVIDE SERVICES
All services shall be provided IAW the PWS (government toilets) or at the frequency requested on the task order (rental units), any deviation must be approved by the COR. The Contractor shall provide a legible service record depicting the date of service and initials of employee who completed the service. The service record shall be affixed to each unit in a manner which provides protection from the weather but still in a visible location. The Contractor shall accomplish an inspection/function check on all rental units before placement to ensure they are in excellent condition and fully operational IAW PWS para 11.2. After each service, the Contractor shall report to the COR any Government owned toilet that is not fully functioning IAW PWS para 1.3.
1.4.1 REGULAR AND ADA COMPLIANT TOILETS
Services shall include but are not limited to the removal of all waste and replenishing of chemically treated liquid (including antifreeze in winter months), cleaning and disinfecting the interior and exterior of the unit, replenishment of all liquid hand sanitizer and paper products, and the removal of all trash and debris in the immediate area. Resupply of toilet paper shall include a minimum of three (3) rolls of two-ply toilet paper.
1.4.2 HAND WASHING STATIONS
Services shall include but are not limited to the removal of hand washing waste water (shall not be dumped in same location) and used paper products, refilling with fresh potable water, and replenishing tri-fold paper towels and liquid hand soap.
1.5 EMERGENCY RENTALS AND SERVICES
The Contractor shall comply with the request for rental units and/or services for rental units or Government owned units within two (2) hours of notification. Approximately ten (10) times a year. The Contractor may be required to perform outside of base hours of operation (7:00 A.M. to 4:30 P.M.) and/or on weekends.
Typically one (1) to two (2) times a year.
1.6 TIP OVER RESPONSE
The Contractor shall set upright any unit(s) that have been tipped over, and perform services IAW PWS para 1.2, 1.3, 1.4, and 1.5 within two (2) hours of notification by the COR at no additional cost to the Government.
1.7 RELOCATION OF UNITS
The Contractor shall relocate any unit within two (2) hours of notification by the COR. Approximately twenty (20) times a year. Relocation service shall be IAW PWS para 1.2, 1.3, 1.4, and 1.5.
1.8 INVOICING
1.8.1 WIDE AREA WORK FLOW (WAWF)
The Contractor shall submit invoicing for rentals and services within the first five (5) business days of the following month.
2 SERVICE DELIVERY SUMMARY

The Contractor shall provide a Quality Control Plan (QCP) within five (5) business days of contract start date. Any changes to the Contractor’s QCP shall be submitted to the CO and the COR for review and approval prior to implementation.

2.2.1 Quality Control Report
Upon request from the COR, the Contractor shall provide electronically by email, all reports generated as a result of the Contractor’s quality control efforts. This shall include any summary information used to track quality control, including any charts/graphs to depict trends in inspection findings.
2.3 QUALITY ASSURANCE
The Government will evaluate the Contractor’s performance to ensure services meet contractual requirements. When a performance threshold has not been met, the COR will initiate and provide the CO a corrective action report (CAR) for issuance to the Contractor. The Contractor shall respond to the CAR IAW instructions provided.
3 GOVERNMENT PROVIDED PROPERTY AND SERVICES
3.1 DISPOSAL OF WASTE
The Contractor shall comply with all Federal, State, and local laws, regulations, and ordinances while transporting waste for disposal. The Contractor can dispose of waste off base but has the option to dispose of waste generated from services performed under this contract at a site on USAFA designated by the COR.
The ability to dispose of waste on USAFA is not guaranteed and must be terminated immediately upon notice from the Government.
3.2 STAGING AREA
The Government shall provide a staging area on USAFA for contract related equipment and materials which shall be kept free from trash and debris and in an organized, orderly manner. The Contractor shall be solely responsible for any loss or damage of Contractor equipment and materials. The Contractor has thirty (30) calendar days to have all equipment removed from the storage area once the contract period of performance end date has been met, unless otherwise stated by the CO. The storage area must be free of trash and debris before the last invoice is approved by the COR.
4 GENERAL INFORMATION
The Contractor shall perform work in a manner which will minimize disruption to USAFA operations.
4.1 PROJECT MANAGER (PM)
Upon contract award, the Contractor shall designate a primary, and at least one alternate Project Manager, who will be responsible for all performance related to the contract. The Contractor shall submit the names and contact information of the primary and alternate(s) PM to the CO and the COR. The PM or alternate(s) shall have full authority to act for the Contractor on all matters. At the request of the COR or CO, the PM or alternate(s) shall be available on-site within sixty (60) minutes.
4.2 USAGE REPORTING
The Contractor shall determine a measureable scale to track usage of each toilet serviced that is approved by the COR. The Contractor shall then digitally submit via email to the COR a usage report no later than five (5) business days after the end of the month.
4.3 DOCUMENTATION
The Contractor is responsible for creating and maintaining historical records of but not limited to, rentals and all services. When requested by the Government, the Contractor shall provide the original record, or a reproducible copy of any such record within five (5) business days of the request.
4.4 PERSONNEL
Contractor personnel shall present a neat appearance. Contractor personnel shall wear distinctive clothing, overcoats, or hats, bearing the company name or logo. The government is authorized to restrict the employment of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
4.5 CONTRACTOR VEHICLES AND EQUIPMENT TRAILERS
The contractor shall comply with all state, local and federal laws. The contractor shall maintain all vehicles and equipment trailers so as not to present a danger on USAFA roadways. All vehicles and trailers shall be in safe working order and be fully functioning. All vehicles must have company logo visible on both sides.
4.5.1 Contractor Full-Time Equivalent Reporting
The Contractor shall report all Contractor labor hours (including sub-contractor labor hours) required for performance of services provided under this contract for the United States Air Force Academy via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
4.5.2 Reporting Inputs
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the Contractor Manpower Reporting Application (CMRA) help desk. Contractors are required to input data by 31 October of each year.
4.5.3 User Manuals
Data for the Air Force service requirements must be input at the Air Force CMRA link http://www.ecmra.mil. However, user manuals for government personnel and Contractors are available at the Army CMRA link.
4.6 DAMAGE CAUSED BY THE CONTRACTOR
The Contractor shall not place any units in area which requires driving vehicles across any and all grass or artificial turf fields without the approval of the COR. If any damages occur, the Contractor shall notify the COR, repair, and return the damaged area to its previous condition.
4.7 VANDALISM AND PERSONAL INJURY
The Contractor shall report, in writing, acts of vandalism, theft, or apparent willful act of damage of Government-owned toilets to the USAFA Security Forces, CO, and the COR. The Contractor shall cooperate with Government personnel and provide a written statement of facts for the Security Forces, when requested.
4.8 CONTRACTOR IDENTIFICATION, SPECIAL CLEARANCES, BASE, AND VEHICLE PASSES
The Contractor shall comply with all requirements and procedures IAW FAR, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference. All Contractor employees are required to obtain badges for access on USAFA. The Contractor shall provide the CO a listing of all employees requiring access. Employees are required to register at the Visitor Entry Control Office.

5 USAFA HOURS OF OPERATIONS
5.1 NORMAL BASE HOURS
Base hours of operation are 7:00 A.M. through 4:30 P.M. Monday through Friday. The Contractor may be required to perform outside of base hours of operation and on weekends to ensure timely completion of work under this PWS at no additional cost to the Government.
5.2 FEDERAL HOLIDAYS
The Contractor is not required to perform on the federal holidays listed below.

If the scheduled service to Government toilets falls on a holiday, the Contractor shall coordinate with COR and arrange an alternate service date.
5.3 BASE CLOSURES
Work scheduled, but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
5.4 PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND
AUTHORITY OR OVERSEAS COMBATANT COMMANDER
According to Department of Defense Instruction (DoDI) 3020.37, Continuation of Essential DoD Contractor Services During Crises, and the Air Force implementation thereof, unless otherwise directed by an authorized Government representative, it is determined that Portable Sanitation requirements under this PWS are not essential to be performed during a crisis.
5.5 PARTICIPATION IN EXERCISES
Contractor personnel are required to participate in base exercises (e.g. fire drills, Random Anti-terrorism Measures (RAMs), or active shooter exercises) if they are working in an areas where such an exercise is implemented. Contractor personnel shall follow instructions given to them pertaining to the exercise and report to the COR any delays in performance.
5.6 SAFETY REQUIREMENTS AND REPORTS
Contractor employees shall comply with the Occupational Safety and Health Act (OSHA), USAFA Fire Protection Program, Air Force Occupational Safety, and the Fire Prevention and Health Program. The Contractor shall display the appropriate caution signs when work operations might pose a risk to injury for any personnel that are present. Provide a verbal report to the COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required accident investigation reports to the COR within five (5) calendar days of each occurrence.
5.7 SAFETY DATA SHEET (SDS)
Upon contract award, the Contractor shall submit a hazardous chemical inventory list including all SDS to the COR. This list shall include all hazardous chemicals used by the contractor on USAFA.
5.8 COMPLIANCE WITH LAWS AND REGULATIONS
The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements governing the work required under this contract. In the event the applicable laws, regulations, and requirements change during the term of this contract, the Contractor shall be required to comply with such changes when they come into effect. This PWS will NOT be revised to incorporate changes in laws, regulations, or requirements.
5.9 WEAPONS, FIREARMS, AND AMMUNITION
Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while on USAFA.
5.10 REPORT HAZARDS AND UNSAFE CONDITIONS
The Contractor shall report any fire hazards, unsafe conditions, and any items in need of repair to the COR.
