Project Title PRD Bunkhouse Flooring Replacement

General Information

1.0 Background and Purpose

1.1. Description of Services: This is a non-personal services contract for flooring and trim replacement in the Petersburg Bunkhouse on the Tongass National Forest. Work includes removal of approximately 2600 square feet of carpet and 600 feet of rubber base in all 8 apartments of the bunkhouse, to be replaced with vinyl plank flooring, matching the existing vinyl plank flooring in the unit and rubber baseboard trim matching the existing rubber baseboard trim in the units. The Government shall not exercise any supervision or control over the Contractor’s employees. Such contract service providers shall be accountable solely to the Contractor who, in turn, is responsible to the Government.

1.1.1. The Contractor shall provide all personnel, supplies, supervision, tools, materials, equipment, transportation and other items and non-personal services necessary to provide flooring and trim replacement in accordance with this Performance Work Statement (PWS) except those items specified as Government furnished property and services. The Contractor shall perform to the standards in the contract as well as all local, state, and federal regulations.

1.2. Background: The Tongass National Forest, located in Southeast Alaska, encompasses approximately 17 million acres spread out over the vast archipelago islands of Alaska’s pan-handle where much of the work takes place in remote locations. Tongass National Forest Facilities Engineering Program supports living quarters throughout the forest. The Petersburg Bunkhouse is a 2-story, 8,352 SF apartment building with 8 units, constructed in 1999.

1.3. Location: The project is located at the Petersburg Ranger District (PRD) on the Tongass National Forest in Petersburg, Alaska 99833. The project facility is the USDA Forest Service Petersburg Bunkhouse located at 402A Fram St. in Petersburg, Alaska.

1.4. Contract Type: The Government will award a firm fixed price purchase order.

1.5.

1.6. Period of Performance:

Date of award through March 31st, 2024.

1.7. Federal Observed Holidays: The government observes the following holidays:

1.8. Codes and Standards: The Contractor shall complete all work in accordance with applicable local, state, and federal codes and regulations, including the current family of ICC codes.

1.9. Post award conference/periodic progress meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation (FAR) Subpart 42.5. The CO and/or COR shall meet periodically with the Contractor to review the Contractor's performance. At these meetings the CO will apprise the Contractor of how the Government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced.

Appropriate action shall be taken to resolve outstanding issues.

1.10. Government Key Personnel: The following government personnel are considered key personnel: Contracting Officer and Contracting Officer’s Representative.

1.11. Contracting Officer (CO): The CO is the only person with the authority to: enter into, administer, and/or terminate contracts; make related determinations and findings on behalf of the government; and legally bind the government. Th CO shall ensure tha the contractor receives impartial, fair, and equitable treatment under this contract. The CO shall determine and document the final assessment of the contractor’s performance in the government’s past performance tracking system.

1.12. Contracting Officer’s Representative (COR): A COR may be assigned to this contract to monitor all technical aspects and assist in contract administration. The COR may be authorized to perform the following functions: ensure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communication with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements (without increasing or decreasing contract scope), including Government drawings, designs, and specifications; monitor Contractor's performance and notify both the CO and Contractor of deficiencies; and provide site entry of Contractor personnel. The COR is not authorized to change any of the terms and conditions of the contract.

1.13. Contractor’s Key Personnel: The following personnel are considered key personnel by the Government: Contract Manager and Quality Control Representative. Provide list to CO and

COR.

1.14. Qualifications of Contractor Personnel: Personnel employed by the Contractor shall be experienced and qualified to perform the work with minimal supervision.

1.14.1. Contractor’s Responsibilities: In addition to assuming complete responsibility for satisfactory performance of the requirements of this contract, the Contractor shall maintain sufficient workforce of adequately trained personnel available to perform services as specified herein throughout the duration of the contract.

1.15. Contract Manager: The Contractor shall provide a Contract Manager and designated alternate, as applicable, who has full authority to act for the Contractor on all contract matters relating to this contract. The Contract Manager or alternate shall be available during the Government's regular working hours and shall be available on site within one hour after the Government's regular working hours.

1.16. Quality Control Plan (QCP): The Contractor shall develop and maintain an effective QCP to ensure services are performed IAW this PWS. The Contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective services and addresses each discernable item of work with quality standards, measurements, and testing. The standard shall be sufficiently descriptive to be used to determine that the task has been successfully accomplished in full compliance with the requirements of this contract. The Contractor’s QCP is the means by which the Contractor assures that their work complies with the requirements of the contract. The Contractor’s QCP shall be provided to the CO and COR within 10 business days after contract award. After acceptance of the QCP, the Contractor shall receive the CO’s acceptance in writing of any proposed change to the QCP. The contractor shall maintain a full copy of all quality control records including sampling, testing, and inspection diaries. These records shall be maintained at the project site and available for inspection by the Government.

1.17. Quality Control Representative: The Contractor shall provide a Quality Control Representative who shall be responsible for the Contractor’ QCP to ensure services are performed IAW the PWS. The Quality Control Representative shall implement and maintain full compliance with the QCP throughout the contract period.

1.18. Conduct of Personnel: The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of misconduct, security, or if they are found to be or suspected to be under the influence of alcohol, drugs, or other incapacitating agents. Contractor employee shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is in the best interests of the Government. Such removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this PWS.

Technical Requirements / Tasks

3.1. General Instructions Applicable to All Tasks:

3.1.1. Description of Bunkhouse: The Petersburg Bunkhouse is a 2-story, 8,352 SF apartment building with 8 units, constructed in 1999. In general, carpeted flooring is located in all bedrooms and hallways of the 1 bedroom units of the bunkhouse. Vinyl plank flooring is located in the living rooms and hallways of the 2 bedroom units. Flooring selection shall match the existing vinyl plank flooring. A vinyl sheet product is used for the kitchens and bathrooms throughout the bunkhouse.

3.1.2. Coordination: Contractor shall coordinate with the government through all phases of this contract including demolition, flooring installation, and trim installation of the bunkhouse.

Some units may be occupied while this project is ongoing. Contractor shall start work on unoccupied units and provide 2 weeks’ notice to the COR, of which occupied units they plan to work on next, to allow tenants time to move their belongings from the unit, prior to work starting in that unit.

3.1.3. Work Schedule: Provide a written schedule showing proposed dates for accomplishing all work items. Contractor shall maintain the schedule up to date and shall provide it to the CO upon request.

3.1.4. Clean-Up: Contractor shall remove all sand, dust, grime, water, protections and other debris generated on the bunkhouse.

3.1.5. Protection: The bunkhouse shall be protected from damage to existing flooring that will remain in place. This also includes, paint, drywall, doors, or any other items that may be encountered during work on this contract.

3.2. TASK 1 – Demolition:

3.2.1. Description: This section covers the furnishing of materials, labor, and equipment required to partially or entirely remove and/or demolish those items identified on in this contract and restore any disturbed areas or components impacted by demolition.

3.2.2. Quantities: Carpet removal for the entire bunkhouse is approximately 2600 square feet.

Baseboard trim removal is approximately 600 lineal feet for the entire bunkhouse. Baseoard trim removal is only in the areas where carpet is present.

3.2.3. Protection: Ensure safe passage of persons around area of demolition. Conduct operations to prevent injury to adjacent buildings, structures, and persons.

3.2.4. Egress: Maintain two paths of egress to any occupied apartment.

3.2.5. Damages: Promptly repair damages caused to adjacent structures by demolition or construction operations at no cost to the Owner.

3.2.6. Disposal: All debris, rubbish, and other materials resulting from demolition operations and not identified as salvageable by the Owner’s Representative shall be removed and transported to a Contractor-furnished disposal site. Burning of removed materials will not be permitted on site.

3.3. TASK 2 – Installation

3.3.1. Underlayment and Vinyl Plank: Install underlayment and vinyl plank flooring to replace all carpeting in the Petersburg Bunkhouse according to manufacturer’s recommendations, including preparation of subfloor before installation. Vinyl Plank shall match existing vinyl plank flooring in the units to the best extent possible. Locations include: 1 Bedroom Units (Apt 1, 3, 5) – Carpet is found in the bedroom and hallway of these units. 2 Bedroom Units (Apt 2, 4, 6, 7, and 8) – Carpet is found in the bedrooms of these 2-bedroom units. For the 1 bedroom Units, carpeted flooring shall be replaced with vinyl plank in all closets that carpet is present. Product shall be selected to match existing vinyl plank in the bunkhouse.

3.3.2. Rubber Baseboard Trim: Install rubber baseboard trim in areas where new flooring is installed only. Match rubber baseboard trim to existing trim in the bunkhouse. Existing product is Roppe rubber baseboard trim in 1/8” thickness and 4.5” height with toe.

3.3.3. Quantities: Underlayment and Vinyl plank installation for the entire bunkhouse is approximately 2600 square feet. Rubber baseboard trim is approximately 600 lineal feet for the entire bunkhouse. Field verify before ordering materials.

3.3.4. Product Submittals: Prior to ordering materials, submit a sample of the desired vinyl plank flooring, underlayment, and rubber baseboard trim product to be used, for approval by the CO.

4. Deliverable / Schedule

4.1. The Contractor shall submit an electronic copy of all deliverables for this PWS to the CO and COR in PDF format and phrased in layperson language. Below is a list of all deliverables outlining each objective and due date.

PERFORMANCE STANDARD

6.1. Customer complaints will be substantiated by the COR for validity. Should the COR determine that the complaint is valid, the COR will inform the Contractor orally and in writing and give the Contractor additional time to correct the defect, if additional time is available. The COR will determine how much time is available.

6.2. Should the Contractor disagree with the complaint after investigation of the site, the

Contractor shall notify the COR. The COR will review the matter to re-examine the validity of the complaint.

6.3. The Government will visually inspect the services for compliance prior to acceptance and issuing any payments. Inspections will consist of direct observations.

6.4. Any qualified Government personnel may observe unacceptable services, incomplete work, or required services not performed and they may contact the COR to communicate a complaint.
