STATEMENT OF WORK (SOW)

FOR

Incident Support Base (ISB) and Federal Staging Areas

(FSA)

Portable Office Unit

1.0 GENERAL

1.1 BACKGROUND

A major disaster was declared for Guam Typhoon Mawar (DR-4715-GU) on May 25, 2023.

Since declaration, Federal Emergency Management Agency (FEMA) has established an Incident Support Base (ISB)/Federal Staging Area (FSA). FEMA requires 1 Portable Office Unit to Support (ISB/FSA) operations.

1.2 SCOPE

The Contractor shall deliver and setup one (1) Portable Office Unit and provide the necessary maintenance services for continued operations during the period of performance. The Contractor shall pick up the portable office unit upon notification from the Contracting Officer or the Contracting Officer Representative within 24-hours, unless specified in writing for specific date and time after the 24-hours of notification.

Delivery Pick Up Maintenance Set Up Tear Down

1.3 OBJECTIVE

FEMA Region Nine has determined a need to establish a Portable Office Unit at the ISB/FSA to support the mission of staging commodities at Andersen AFB Site and Won Pat Site.

2.0 SPECIFIC REQUIREMENTS/TASKS

2.1 TASK ONE-SAFETY

2.1.1 The portable office unit shall meet all Local, State and Federal guidelines and regulations for safe handling and operation.

2.2 TASK TWO-FUELING

The contractor shall only provide fueling for the initial setup. The contractor will not provide refueling services for the portable office unit generator.

2.2.1 Meet current State and Federal Environmental Protection Agency (EPA) Guidelines including spill prevention, control, and countermeasures. The Contractor must keep on site spill cleanup materials with instructions for on-site personnel and communicate with the FEMA point of contact of the materials location.

2.3 TASK THREE-MAINTENANCE

2.3.1 Properly maintain portable office unit in working order to allow consistent operation (up to 24-hours/day during the period of performance) in a disaster environment.

2.3.2 Adhere to manufacturer specifications and guidelines for the intended purpose of the equipment. Follow any Local, State or Federal Guidelines, including for disposal of liquids/waste.

2.3.3 If an issue happens outside of normal maintenance schedule and is deemed an essential service or piece of equipment, the Contractor must be on site within two (2) hours addressing the issue unless other arrangements are made in writing with the Contracting Officer or Contracting Officer Representative.

2.3.4 All equipment must be set up to prevent leaks or spills onto the ground.

2.3.5 If leaks or spills occur the Contractor must perform State and Federal EPA Guidelines legal clean up services within 24-hours of notification.

2.3.6 If the portable office unit is inoperable, it must be fixed or replaced within 24-hours of notification to the Contractor.

2.4 TASK FOUR-SET UP

2.4.1 Properly set up and anchor the portable office unit to withstand routine severe weather without damage to existing asphalt or concrete. The Contractor shall be responsible for any damage and/or repairs due to the set-up of the portable office unit.

2.5 TASK FIVE-TEAR DOWN

2.5.1 Remove the portable office unit without damage to existing asphalt or concrete. The Contractor shall be responsible for any damage and/or repairs due to the tear down of the portable office unit.

2.6 TASK SIX-DELIVERY

2.6.1 Deliver to the specified location within 24-hours of award. The Contractor shall coordinate delivery with the COR and FEMA location point of contact.

2.7 TASK SEVEN-PICK UP

2.7.1 Pick-up the portable office unit within 24-hours of notification unless specified time and date is provided in writing by the Contracting Office or Contracting Officer Representative.

2.7.2 Pick up shall be coordinated with the COR and FEMA location of point of contact.

2.8 TASK EIGHT-DISASTER WEATHER PROCEDURES

2.8.1 All items should be secured and or removed within 24-hours if notified by FEMA Point of

Contact and/or COR of incoming severe weather, and then returned within 24-hours of notification of all clear from the FEMA Point of Contact and/or COR.

3.0 CONTRACTOR PERSONNEL

It is the responsibility of the Contractor to provide qualified personnel to perform all requirements specified in the SOW.

3.1 QUALIFIED PERSONNEL

The Contractor must provide qualified personnel that meet Federal, State and Local regulations to perform all requirements specified in this SOW.

3.2 CONTINUINTY OF SUPPORT

The Contractor must ensure that the contractually required level of support for this requirement is always maintained.

3.3 CONTRACTOR PERSONNEL SAFETY

3.3.1 Contractor personnel must follow all Local, State and Federal guidelines.

3.3.2 Contractor personnel on site must wear proper safety equipment for their services, as well as a reflective clothing/vest at all times.

3.3.3 Contractor personnel must immediately report to the FEMA POC if any safety incidents occur while performing duties onsite on the day of occurrence before leaving the site.

3.4 KEY PERSONNEL

3.4.1 CONTRACTOR PROJECT MANAGER

The Contractor shall provide a company representative as a liaison to the onsite FEMA

POC upon written request from COR or Contracting Officer to provide guidance and instructions for day to day operations of equipment provided. The FEMA POC’s contact information will be provided to the Contractor by the COR.

3.5 CONTRACTOR EMPLOYEE CONDUCT

3.5.1 Contractor employees must comply with all applicable Government regulations, policies and procedures (e.g., fire, safety, sanitation, environmental protection, security, “off limits” areas, wearing of parts of DHS uniforms and possession of weapons) when visiting or working at Government facilities. The Contractor must ensure Contractor employees always present a professional appearance and that their conduct must not reflect discredit on the United States or the Department of Homeland Security. The Contractor Representative must ensure Contractor employees understand and abide by Department of Homeland Security established rules, regulations and policies concerning safety and security.

3.6 CONTRACTOR EMPLOYEE IDENTIFICATION

3.6.1 Contractor employees visiting Government facilities must wear an identification badge that, at a minimum, displays the Contractor company name, the employee’s photo name, and badge expiration date. Visiting Contractor employees must comply with all Government escort rules and requirements. All Contractor employees must identify themselves as Contractor when their status is not readily apparent and always display all identification and visitor badges in plain view above the waist. 3.7 REMOVING CONTRACTOR FOR MISCONDUCT OR SECUIRTY REASONS

The Government may, at its sole discretion (via the Contracting Officer), direct the Contractor to remove any Contractor employee from DHS facilities for misconduct or security reasons. Removal does not relieve the Contractor of the responsibility to continue providing the services required under the contract. The Contracting Officer will provide the Contractor with a written explanation to support any request to remove an employee.

4.0 PERIOD OF PERFORMANCE

The period of performance for this contract is a one-month base period and one-month option period.

Base Period: June 16, 2023 through July 15, 2023 Option Period: July 16, 2023 through August 15, 2023

4.2 GENERAL REPORT REQUIREMENTS

The Contractor must provide all written reports in electronic format with read/write capability using applications that are compatible with DHS workstations (Windows 10 and Microsoft Office Applications).

The Contractor must adhere to delivery of all operation reports specified in task items Section 2.

The Contractor must provide an initiation report to include the schedule of delivery upon notification of order to include point of origin and estimated timeline of delivery of the portable office unit. Upon set up of the portable office unit on the site, Contractor shall provide reports indicated in the specified task items within Section 2 of the SOW.

If the portable office unit will be delayed past the 24-hour requirement of delivery, upon 1-hour of discovery, the Contractor must notify the CO and COR in writing of the portable office unit being delayed and the estimated time of delivery. The Contractor shall be required to update the CO and COR every eight 8 hours in writing until the portable office unit has met delivery requirements.

The Contractor must provide written acknowledgement to the CO and COR upon notification of site termination and to provide a detailed schedule of tearing down and picking up the portable office unit. Upon removal of all the portable office unit from the site, The Contractor shall provide reports indicated in the specified task items within Section 2 of the SOW.

If the portable office unit will be delayed past the 24-hour requirement of pickup, upon 1-hour of discovery, the Contractor must notify the CO and COR in writing of the portable office unit being delayed and the estimated time of pickup. Contractor shall be required to update the CO and COR every eight (8) hours in writing until the line item has met pickup requirements.

6.0 GOVERNMENT FURNISHED RESOURCE

The Government will not furnish any resources to the Contractor in support of this contract.

7.0 CONTRACTOR FURNISHED PROPERTY

The Contractor must furnish all materials, equipment and services necessary to fulfill the requirements of this contract, except for the Government Furnished Resources specified in

SOW 6.0.

8.0 GOVERNMENT ACCEPTANCE PERIOD

The COR will review deliverables, supplies, services prior to acceptance and provide the

Contractor with an e-mail that provides documented reasons for non-acceptance. If the deliverable, supply and/or service is acceptable, the COR will send an e-mail to the Contractor notifying it that the deliverable, supply and/or service has been accepted.

8.1 The CO will have the right to reject or require correction of any deficiencies found in the deliverable, supply and/or service that are contrary to the information contained in the Contractor’s accepted proposal. In the event of a rejected deliverable, supply and/or service, the Contractor will be notified in writing by the CO of the specific reasons for rejection. The Contractor may have an opportunity to correct the rejected deliverable, supply and/or service and return it per delivery instructions.

8.2 The CO will have 48 hours to review deliverable, supply and/or service and make comments. The Contractor must have 24 hours to make corrections and redeliver.

9 DELIVERABLES

9.1 DAILY MAINTENANCE REPORT

9.1.1. Provide FEMA point of contact at the site with a daily report and log which provides item number, actions taken, any issues and solutions. A report must be submitted every day even if to confirm there are no issues.

9.1.2. The status of repair or replacement must be provided in the maintenance daily report.

9.2 SET UP REPORT

9.2.1. Contractor shall provide written confirmation with pictures of each set up site for the portable office unit, prior to set up and after set up, to be reviewed and signed by the Contractor, FEMA

POC on site and COR

9.3 TEAR DOWN REPORT

9.3.1. The Contractor shall provide written confirmation with pictures of each tear down site for each line item, prior to tear down and after tear down, to be reviewed and signed by the Contractor, FEMA POC on site and COR.

9.4 DELIVERY REPORT

9.4.1. The Contractor shall provide written confirmation of delivery signed by the Contractor and

FEMA POC on site.

9.5 PICK UP REPORT

9.5.1. The Contractor shall provide written confirmation of pick up signed by the Contractor and

FEMA POC on site.
