Invoicing Monthly invoices per contract procedures are timely and accurate.
Review & acceptance of the invoice by COR
Monthly 100% accuracy representative when a defect is identified and inform the manager of the specifics of the problem.
Quality Control Inspection Reports: The Contractor shall maintain a current record of all Quality Control Inspection reports. These reports shall be furnished to the COR/COTR each month or more often, if requested in writing. The records of inspections shall cover all work items scheduled and shall be signed and dated by the Contractor’s Quality Control Inspector.
The frequency of these inspections shall be of an adequate number to insure all contract specifications are met. The COR/COTR reserves the right to require daily inspection reports if deemed necessary.
6. INSPECTION DEFICIENCIES: The COR/COTR reserves the right to request the Contractor to return to the job site and address any items which fail to meet the technical cleaning specifications. Any deficiencies must be corrected by the contractor no less than two hours after being notified. Payment will not be made for work which was not performed or does not meet the specifications in the scope of work. After the initial two-hour notification, the Corps of Engineers reserves the rights to have a third party perform any scheduled work which was not done or has not been done satisfactorily. In such a case, the government will charge these third-party cost to the contractor.
7. MEETINGS
Performance Evaluation Meetings: The contractor shall meet with the COTR, as often determined necessary by the COR/COTR to discuss performance.
Contract Deficiency Report (CDR): A meeting shall be held no later than one business day after a CDR is issued. Written minutes of these meetings shall be prepared and signed by the COR/COTR and the Contractor. Mutual effort will be made to resolve all problems identified.
Should the Contractor not concur with any decision, etc., the Contractor shall so state, in writing and request a final decision by the Contracting Officer (Park Manager).
8. RATINGS
Metrics and methods are designed to determine if performance exceeds, meets, or does not meet a given standard and acceptable quality level. The Inspection Checklist (Enclosure 1) employs a numerical scale using the numbers 1 through 4.
1. Acceptable
2. Unacceptable
3. Work in Progress
4. Work Not Started
9. DOCUMENTING PERFORMANCE
The Government shall document all performance using the attached Inspection Checklist.
Report may become a part of the supporting documentation for fixed fee payments or other actions.
When unacceptable performance occurs, the COR shall inform the contractor. The Government Inspector will document each occurrence on the Inspector Checklist unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file.
When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR) and present it to the contractor's task manager or on-site representative. A CDR template is attached to this QASP.
The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor must present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability.
Any CDRs may become a part of the supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other actions deemed necessary by the KO.
