PERFORMANCE WORK STATEMENT
FOR
AIR FORCE RESERVE COMMAND
94 Mission Support Group / Bioenvironmental Engineering
WATER SAMPLING
1 February 2020
1. DESCRIPTION OF SERVICES:
1.1. Scope of work: The contractor shall provide all necessary labor, supplies, and equipment to accomplish microbiological testing of water samples for compliance with the Safe Drinking Water Act (Public Law 93-523), Georgia Environmental Protection Division (GEPD) Standards, or other methods as herein specified or as required by 94 MSG/SGPB.
The contractor shall be State certified laboratory for Microbiological testing by the GEPD.
Contractor shall use EPA-approved analytical methods as outlined in the Code of Federal Regulations 40 CFR 141.21 and contained in the most recent edition of the "Standard Methods for the Examination of Water and Wastewater" published by the American Public Health Association. This includes Escherichia coli ( e-coli), total coliform, chlorine on site analysis and Heterotrophic Plate (HPC) only if the chlorine level is below 0.2 parts per million (ppm).
1.2. Organization. The AFRC organization with primary responsibility for this requirement is 94 Mission Support Group (MSG), Bioenvironmental Engineering/Public Health Services.
1.3. Place of Performance. These services will be performed at Dobbins Air Reserve Base (DARB), Marietta, GA.
1.4. Availability. The contractor shall be available for service and provide technical advice pertaining to water sampling and analysis Monday through Friday, 0730 until 1600 with the exception of observed federal Holidays. (See paragraph 3.2 for observed holidays.)
1.5. Sampling Requirements and Supplies
1.5.1. Administrative: The contractor shall supply all business forms and labels necessary for sample collection and analysis.
1.5.1.1 Sampling Supplies: The contractor shall supply shipping containers, packing aterials, ice/refrigerants, preservatives, labels, proper marking/identification, sample collection instructions, custody seals, and chain-of custody forms for all samples to be analyzed under this contract. The contractor shall supply all U.S. Department of Transpiration (DOT) shipping labels and other labels designed to meet all regulatory shipping requirements. The contractor shall provide container labels necessary to clearly and correctly identify the type of analysis required, preservatives used, sample source, date and lime sampled, name of sample identification number.
1.5.2 Samples:
1.5.2.1 Routine Samples: Routine Samples are those samples that can be reasonably planned or anticipated, require a standard turn-around-time (TAT) and will be picked up on a regularly scheduled day. Contractor shall conduct sampling on the second Wednesday of the month.
Samples shall be taken at the following locations:

If any of these locations are not available during the sampling time, the contractor shall call Bioenvironmental Engineering Services (BES) at (678) 655-5781 for an alternate location.
1.5.2.2 Non-Routine Samples: Non-routine samples are those samples that can't be planned or anticipated, require a standard turn-around time, and may need to be picked up on other than a regularly scheduled day. This collection shall be conducted within 24-hours of request and verbal report back to BES within one hour of results finding.
1.5.2.3 Micro-Bacteriological Sample Analysis: Micro-bacteriological sample analysis shall;
at a minimum, consist of chlorine readings on site and analysis for the presence/absence of coliform and e-coli. Heterotrophic plate shall be conducted only if the chlorine level of the sample is below 0.2 ppm.
1.6 Reporting Results for Analysis
1.6.1 Positive Results and Non-Routine Sample Results: Contactor shall telephone results to Dobbins ARB BES personnel within one hour of result. The contractor shall continue to call until a Dobbins ARB BES POC is contacted. The contractor shall submit hard copy results within 48-hours (electronic means are acceptable; 94msg.bio@us.af.mil).
1.6.1.2 Duty Hours. To report during duty hours (0730 until 1600 Monday through Friday), t he Contractor shall telephone the Dobbins ARB BES point of contact (POC). The Dobbins ARB BES representative will provide the contractor with a list of primary and alternate POCs. Updates will be provided to the contractor as changes occur. The contractor shall not leave a voicemail message, fax, or electronic mail notification for these results.
1.6.2 Routine Negative Results. The contractor shall report negative results for the total coliform and e-coli to Dobbins ARB BES by hard copy format or by an agreed electronic means(ie: e-mail) within 7-10 calendar days of collection (94msg.bio@us.af.mil). The contractor shall not leave a voicemail message or fax for any results. Dobbins ARB BE will provide a list of POC and alternates. Updates will be provided to the contractor as needed.
1.7 Transportation of Water Samples
1.7.1 Routine Microbiological/Bacteriological Samples. The contractor shall collect or pick-up, package and transport samples at the second Wednesday of the month. However, if routine water sampling is not permitted due to inclement weather, holiday or unforeseen circumstances, the Government shall notify the contract lab personnel and coordinate an alternate pick-up date with the contractor, usually the following day.
1.7.2 Non-Routine Special Microbio logica l /Bacteriological Samples. The contractor shall collect or pick-up, package, and transport samples at a mutually agreed upon time between the government and contractor. Samples shall be delivered to the lab within 12 hours.

3.0 GENERAL REQUIREMENTS
3.1 Hours of Operation. Normal hours of operation for DARB is, 7:30 a.m. - 4:00 p.m., Monday through Friday, excluding federal holidays. Services performed outside of normal duty hours must be coordinated with the COR in advance, at no additional cost to the Government and prior to any schedule changes.
3.2 Federal Holidays. The contractor shall not perform routine services on the following Federal holidays, or the actual day set aside for observation.

3.3 Laboratory Requirements.
3.3.1 Validation of Qualifications: The contractor's laboratory shall maintain certification by the GEPD to perform bacteriological analysis of drinking water samples. The contractor shall notify the government within 7 calendar days of changes in certification status. Contractor shall disclose any non-compliance activity cited by the GEPD during the last two years.
Contractor shall inform the contracting officer of any non-compliance activity cited by GEPD during the life of this contract.
3.3.2 Laboratory Audits: Upon the request of the government, the laboratory will be required to allow government personnel to perform on-site laboratory audits.
3.4 Confidentiality. The contractor shall strictly observe the confidentiality of the Air Force information and records and shall comply with the laws and regulations governing disclosure, including Federal Privacy Act 5 U.S.C.553A. The contractor shall be held liable in the event of breach in confidentiality.
3.5 Continuation of Essential Departmental of Defense (DOD) Contractor Services During A Crisis. All services to be performed under the contract have been determined to be essential for performance during crisis. According to the department of Defense Instruction (DoDI) 3020.37, Continuation of Essential DoD Contractor Services During a Crisis, and Air Force implementation t hereof; it is determined that the contractor shall be required to perform during crisis situations. Service may be required to support an activation or exercise of contingency plans outside the normal duty hours. The base could be closed because of security problems or other events, which would necessitate the closing of the base. Should one of these situations occur the contractor should listen or watch one of the local television stations for notification of base closure from the government unless contacted by the Contracting Officer or BES. The contractor is responsible for notifying his/her employees.
3.6 Post Award Meeting. The Contractor shall attend a one (1) day post award meeting convened and hosted by the 94 MSG/BES COR. The post award conference will be held at Dobbins ARB within two (2) weeks of contract award. The Functional Commander, 94 CONF/PK CO and 94 MSG/BES COR will participate with Contractor Representatives. The meeting will provide an orientation and overview of the contract scope, terms and conditions. The meeting will detail the roles and responsibilities of AFRC team members, performance evaluations, and contract closeout.
3.7 Contractor Identification. The Contractor personnel shall wear contractor-provided identification at all times while in government facilities so as to distinguish themselves from Government employees. Contractor personnel may attend meetings, answer phones, and work in other situations where their status is not obvious to third parties; therefore, the Contractor personnel shall always identify themselves as Contractor support to avoid potential misrepresentation as Government personnel or to avoid situations arising where sensitive topics might be better discussed solely amongst government personnel. Electronic mail signature blocks shall identify their company affiliation. Where practicable, Contractor personnel occupying collocated space with their Government program customer shall identify their work space with their name and company affiliation.
3.8 Continuation of Mission-Essential Services during a Crisis.
3.8.1 Designation of Services as Mission-Essential. The Functional Commander or civilian equivalent has determined, by placing the decision in writing, that all of the contractor services performed under this contract are mission essential (M-E) and will continue in the event of a crisis.
3.8.2 Mission-Essential Contractor Services Plan: The Contractor shall submit a plan describing procedures for continuing performance of mission-essential services during a crisis.
The Contractor shall identify provisions for ensuring required personnel and resources are available to support continuity of operations for up to 30 calendar days or until normal operations can be resumed. The Contractor shall submit any revision to the plan for approval prior to implementation. The Contractor shall activate the plan only if authorized by the PCO acting under the direction of the FC or civilian equivalent.
3.9 Non-personal Services. The Government will neither supervise contractor personnel nor control the method by which the contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor personnel. It shall be the responsibility of the Contractor to manage its personnel and to guard against any actions that are of the nature of personal services, or give the perception of personal services. The Contractor shall notify the AFRC/PKA CO immediately if any task requires action by the Contractor personnel that constitutes, or are perceived to constitute personal services.
These services shall not be used to perform work of a policy/decision making or management nature, i.e. inherently governmental functions. All decisions relative to the [insert title of requirement] program supported by the Contractor shall be the sole responsibility of the Government.
3.10 Security. The Contractor shall comply with all security regulations and directives identified herein and other security requirements as shown elsewhere in this contract.
3.10.1 Physical Security Program.
3.10.1.1 Installation Perimeter Access Control. The requirements for installation perimeter access are detailed in Air Force Federal Acquisition Regulation Supplement (AFFARS) clause 5352.242-9000 entitled, Contractor Access to Air Force Installations, in Section I, Contract Clauses, of the basic contract. Contractors merely requiring unescorted access to the installation to perform services, that don’t require access to classified information or network access, will be vetted IAW AFM 31-113, para 4.3 and issued a DBIDS Identification Card. The Contractor is only allowed to visit the installations specified in the contract.
3.11 Health & Safety Program. The Contractor is responsible for the safety and health of their personnel and protection of the public on Government work sites (DODI 6055.1, Paragraph E5.1).
The Contractor shall maintain a health and safety program that meets OSHA standards. If the 94 CONF/PK CO notifies the Contractor of a potential OSHA violation, the Contractor is obligated to comply with the applicable OSHA regulations.
3.12 Incident or Mishap Procedures. The Contractor shall immediately call 911. The Contractor shall within one (1) hour notify the 94 CONF/PK CO, 94 MSG/BES COR and 94 AW/S of all mishaps or incidents for damage to government property. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three (3) calendar days to the 94 CONF/PK CO (with copy to 94 MSG/BES COR) who will forward to 94 AW/S. For information not available at the time of initial notification, the Contractor shall provide the remaining information not later than twenty (20) calendar days after the mishap, unless extended by the 94 CONF/PK CO.
Mishap notifications shall contain, as a minimum, the following information:
(A) Contract, contract number, name and title of person(s) reporting
(B) Date, time and exact location of accident/incident
(C) Brief narrative of accident/incident (events leading up to the accident/incident)
(D) Cause of accident/incident (if known)
(E) Estimated cost of accident/incident (material + labor to repair/replace)
(F) Nomenclature of equipment and personnel involved in the accident/incident
(G) Corrective actions (taken or proposed)
(H) Other pertinent information
The Contractor shall, in the event of an accidental incident/mishap, take reasonable action to establish control of the incident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the incident/mishap investigative authority.
3.13 Fire Emergencies. The Contractor personnel shall dial 911 to report fire related emergencies.
3.14 Department of Labor (DoL) Inspection of Contractor Operations. The Contractor is subject to DoL inspections and enforcement by OSHA health and safety officials while performing work on a Government installation. The OSHA health and safety officials may access workplaces on Government installations at any time, scheduled or unscheduled, during regular work hours. The OSHA health and safety officials must meet security requirements to enter restricted or classified areas. The Contractor shall notify the 94 CONF/PK CO and 94 MSG/BES COR upon notification of a visit.
3.15 Fire Protection and Prevention Program. All Contractor personnel performing work on properties under jurisdiction of Dobbins ARB, GA shall be responsible for fire safety and compliance with all applicable OSHA, State, Air Force, AFRC, and base regulations and directives.
3.16 Environmental Management System (EMS). Contractor/subcontractor personnel who perform work on any Government facility shall comply with the EMS requirements established by the facility.
3.17 Trafficking in Persons. The Contractor shall comply in accordance with the FAR and applicable supplements and shall be in compliance with all applicable guidance and clauses listed in the contract as it relates to Trafficking in Persons.
