MAINTENANCE OF STERIS STERILIZERS

I. DESCRIPTION OF SERVICES

1.0 The contractor shall provide everything necessary as defined in this Performance Work Statement (PWS), except as specified herein as government furnished property or services, to provide preventive maintenance to include intervening service calls for government owned equipment, located at the 60th Medical Group, David Grant Medical Center, Travis Air Force Base, California. The contractor shall perform these services in accordance with the standards specified in this contract and in accordance with the Original Equipment Manufacturer (OEM) Specifications, the National Fire Protection Association Standard NFPA 99 (Electrical Equipment in Healthcare Facilities), the Bureau of Radiological Health (BRH), Health Insurance Portability and Accountability Act (HIPPA) IAW 45CFR 160 and 164, and the Joint Commission for Accreditation of Health Care Organizations (JCAHO). The contractor shall submit reports and documentation as identified throughout this PWS. The contractor shall make available a standard commercial quality control plan upon request by a government representative. To cover the Steris sterilizing equipment, contractor tasks include:

Service performed by OEM-certified technicians Use of only new OEM parts Preventive maintenance and unlimited repairs that include all (non-excluded) parts, labor, and travel Manufacturer telephone technical support 24/7 Telephone response time within 4 hours; on-site response within 48 hours Service Reports Exclusions -The Services do not include, and Subscriber agrees to pay STERIS at its then prevailing labor rates and parts prices for, any services not included in the Service Option selected by Subscriber for any given piece of Equipment, including, without limitation, the following:

i. Equipment overhauls, welding, rebuilds, upgrades and/or relocations, door assemblies, Light cameras, surgical monitors or recorders not purchased from STERIS, generators, water tanks, reservoirs, Surgical Table Shrouds, Sonic Tanks, SYSTEM 1e trays and monochrome display. For life science Subscribers, all pumps, transducers, or components with a sales price greater than $5,000.

ii. Consumable and expendable items including, but not limited to, filters, cleaning agents, biological and chemical indicators, recording charts, paper, ink pens, ribbons, pen arms, surgical lamps, light handles, batteries, and all hydraulic oils;

iii. Surgical table accessories; hand controls, arm boards, x-ray tops, table pads

iv. Visits made at Subscriber's request to perform Services on Equipment which was not available, which could not be located, or services outlined as Subscriber responsibility in the equipment operator manual;

v. Services requested by Subscriber due to failure of defective accessory items;

vi. Services or parts requested by anyone other than STERIS, unless authorized in writing by

STERIS;

vii. Parts, manuals and/or labor found necessary to bring Equipment up to normal operating condition during the Pre-PM Inspection, provided that if Subscriber does not wish to bring the Equipment up to normal operating condition, STERIS will delete the Equipment from the Equipment List;

viii. Repairs resulting from operator error, misuse, abuse, improper operation or installation, defective accessories, being dropped, fire, loss, theft, utility failure, negligence by any party or other acts beyond the reasonable control of STERIS, or where STERIS determines that in fact no repair is actually required;

ix. Parts or labor required for recall or safety oriented modifications to non-STERIS Equipment;

x. Any repairs which are estimated by STERIS to cost in excess of 50% of the unit's current value (STERIS will provide Subscriber with a written estimate in such cases);

xi. Services performed outside of Normal Working Hours, unless work outside Normal Working Hours is provided for in the Equipment List; and

xii. Service lines, shut off valves, disconnect switches or other components not a part of the Equipment unless otherwise specified.

xiii. Any parts for Equipment that is not designated as parts inclusive on the Equipment List;

and

xiv. Unless the 1st Year Preventive Maintenance Option listed on Attachment A is selected by the Subscriber, preventive maintenance during the Warranty Period for capital equipment is not covered by this Agreement and is available through a separate agreement with STERIS.

xv. All oil changes in excess of the quantity of (2) for all V-PRO units with a standard maintenance agreement are billable events to be charged at then current parts and labor rates.

Oil changes above the standard quantity of (2) may be included in the maintenance agreement for an additional fee.

xvi. STERIS’s obligation to provide the Services will be relieved during the term if access to parts are no longer available to support product or the product is obsolete from a Parts and Labor coverage level. Subscriber will be given the option to reduce the coverage to Labor Only for the remainder of the term.

1.1 Preventive Maintenance Inspections and Intervening Repair Service

The contractor shall provide scheduled preventive maintenance inspections and repair or replacement services as needed on the Steris sterilizing equipment in accordance with OEM specifications. Inspections shall include but are not limited to the following:

Review of system operation.

Lubrication, cleaning, functional checks Completion of minor operational and reliability field engineering change notices.

PM to current published OEM specifications.

Any other remedial maintenance of non-emergency nature including but not limited to adjustments, calibrations, lubrications, cleaning, testing, replacement of work or defective non-consumable parts, etc. required to keep the equipment in good operating condition.

Contractor shall provide maintenance necessary to preserve system performance and safety testing. Contractor travel to perform covered service shall be included at no additional cost to the government. Contractor shall arrange the date and time of preventive inspections with the Medical Equipment Repair Center (MERC) at least one week in advance. The contractor shall provide the customer with initial phone contact within 4 hours and provide on-site response within 48 hours. All routine preventive maintenance service shall be accomplished during normal duty hours 8:00 AM to 5:00 PM, Monday through Friday, (excluding Federal Holidays).

1.2 Service Reports

A service report is required for any maintenance service performed at any time. Contractor shall sign in/out on the log sheet and report to the POC in the Medical Equipment Repair Center both upon arrival on-site and upon completion of the work. The original service report, signed off by the department using the equipment, must be presented to the Medical Equipment Repair Center (60mdg.merc@us.af.mil) and a copy sent to the Contracting Officer upon completion of the work prior to departure. The service report must include the following as a minimum:

Date and time of service.

Company and service representative names.

Contract number.

A detailed description of any services or repairs performed, documenting labor and any parts used.

Any contractor recommendations necessary to maintain the equipment in the best operating condition.

For PM/Calibration service calls, the PM/Calibration checklist with results will serve as the service report.

Contractor shall ensure compliance with the National Fire Protection Association

Standard NFPA 99 (Electrical Equipment in Healthcare Facilities), and shall list electrical leakage current and ground resistance readings for equipment and provide this information as part of the service report.

1.3 OEM Replacement Parts

Contractor shall provide repair parts meeting or exceeding current OEM specifications. All parts shall be only new standard parts. Replaced parts shall become the property of the contractor and shall be removed from the hospital and disposed of at no additional cost to the government.

1.4 Modifications/Software Upgrades

All software and hardware modifications and upgrades recommended or normally provided by the OEM to keep the system operational capability current shall be provided and accomplished by the contractor during PM inspections at no additional cost to the government.

1.5 Operator Training

Contractor shall provide instructions and training to DGMC equipment operators on the proper care and operation of the equipment through telephone support between 0800 and 1700, Monday through Friday.

1.6 Initial Inspection

All equipment will be inspected and approved by the Medical Equipment Repair Center prior to use in the Medical Treatment Facility. Contact a MERC Representative to schedule inspection.

1.7 Removal of Equipment

Should the equipment or any component listed herein require repair at the contractor’s plant, contractor shall obtain a signed authorization from the MERC Representative authorizing removal of government equipment. Contractor shall not remove any equipment until loaner/replacement is installed on-site. Contractor shall be responsible for damage or loss of equipment while in contractor’s possession.

III. GOVERNMENT FURNISHED PROPERTY

The government will furnish utilities (electrical power, compressed air, water) that are available and required for any service provided under this contract. The government facility engineers, to ensure compatibility with DGMC’s electrical wiring and equipment, must approve contractor electrical equipment. The MERC is responsible for inspection of all electrical wiring and equipment that is brought into the facility for use. This will occur when the technician checks in at MERC.

IV. GENERAL INFORMATION

4.6 Warranty

Repairs made during the term of the contract are warranted for the period of the contract or for ninety (90) days, whichever is longer.
