About this file

This document is a Minimum Qualification Certification form for the Other Post Employment Benefit (OPEB) Trust Fund's Request for Proposals for Fund Custodial and Investment Services. The form requires prospective providers to certify that they have at least five years of experience as of January 1, 2024, in providing the requested services and that these services are a specialization of their business. Providers must complete the certification to be considered for further evaluation, with the understanding that incomplete information may result in immediate proposal rejection.

The certification form is designed to validate the minimum qualifications of potential service providers for the OPEB Trust Fund. It requires the prospective provider to indicate whether they meet the five-year experience requirement by checking either "Yes" or "No" and provides space for the provider's name, authorized signature, printed name, title, and date of submission. The form emphasizes the importance of providing accurate and comprehensive information to demonstrate the provider's capability and specialization in fund custodial and investment services.
