About this file
This is a Statement of Objectives (SOO) for the Defense Health Agency (DHA) Single Enterprise Solution for Maternal Fetal Medicine (MFM) Ultrasound imaging. The DHA seeks to transition from fragmented local and HUB Picture Archiving and Communications Systems (PACS) solutions to a single enterprise MFM ultrasound imaging solution that integrates with current PACS vendors and future enterprise PACS systems. The current environment operates with 12 strategically located HUB PACS sites and 50 spoke sites with downtime servers. The primary objectives include providing operational information for MFM ultrasound performance, capturing clinical information in visual and discrete data formats, standardizing fetal and maternal assessments with serial scan archival capabilities, distributing information for downstream clinical care, and supporting evidence-based decision-making. The solution must resolve current challenges including inability to perform serial scan comparisons, lack of comprehensive records for unexpected outcome reviews, data storage inefficiencies across disparate systems, and inconsistent documentation metrics.
The specific business requirements mandate that the solution legally document MFM ultrasound (MFMUS) images to the electronic health record (EHR), incorporate established clinical practice guidelines and National Protocols, import patient demographics from the system of record, meet enterprise equipment standardization requirements, archive DICOM images and files to the enterprise archiving system, and comply with National Institute of Standards and Technology (NIST) risk management framework standards for any artificial intelligence capabilities. The functional requirements emphasize DICOM standard support for image acquisition, transfer, and storage; comprehensive data security audit functions with HIPAA compliance; seamless PACS integration with query/retrieve functionalities; structured reporting with standardized terminologies; user-friendly interface design; and AI-enhanced integration capabilities including automated information extraction, anomaly detection, image analysis and indexing, patient record matching, and anatomical landmark recognition to optimize workflow and ensure measurement completeness.

Attachment 1: Defense Health Agency (DHA) Single Enterprise Solution for Maternal Fetal Medicine Ultrasound
1. Background
The Defense Health Agency (DHA) is exploring a Single Enterprise Picture Archiving Communications Systems (PACS) Maternal Fetal Medicine ultrasound imaging solution which will operate seamlessly with a DHA single PACS solution (TBD).
The primary goal is to provide a solution and services necessary for DHA to transition from local and HUB PACS solution(s) to a single enterprise Maternal Fetal Medicine ultrasound imaging solution with an integrated application Graphical User Interface to the current PACS vendors as well as a future enterprise PACS vendor.
The new Maternal Fetal Medicine (MFM) imaging solution will meet industry performance standards, offer the necessary redundancy and contingency features to meet DHA needs, and provide state-of-the-art technology enhancements to improve user experience and minimize service disruption.
DHA is finalizing the consolidation of site local PACS to a HUB and Spoke model throughout enterprise. Currently, DHA has 12 HUB PACS strategically located within the DHA PACS enterprise. Additionally, there are 50 spoke sites with downtime servers.
2. Current Environment
Currently, maternal fetal medicine ultrasound examinations performed across the enterprise, including clinics, inpatient units, and MFM departments, are not integrated into a centralized radiology information system. This fragmented approach creates significant challenges for:
• Serial Scan Comparison: Providers cannot easily access and compare prior ultrasound images and reports for the same patient, hindering longitudinal assessment of fetal development and identification of potential issues.
• Unexpected Outcome Reviews: When unexpected pregnancy complications arise, healthcare providers lack a readily available comprehensive record of past ultrasound data for informed decision-making.
• Data Storage and Retrieval Inefficiency: Disparate systems create administrative burdens, inefficiencies and potential data loss in the complexity of multiple systems involved in storing, managing, and retrieving past ultrasound exams across different locations.
• Inconsistent Metrics: Consistent template for documentation is important to ensure all components are addressed.
3. Objectives
This standardization, modernization, and integration initiative will empower Maternal-fetal medicine (MFM) subspecialists who rely on ultrasound technology to provide high-risk pregnancy diagnostic ultrasounds, consultations, procedures, and provide necessary support to obstetrics (OB) and neonatology services. The high-level objectives of the requirement will aid the capability to:
1. Provide operational information pertinent to the performance of an MFM ultrasound, then provides the ability to display images and structured report data (e.g., ultrasound measurements, etc.), generates and communicates the final report, and conveys coding and billing information.
2. Obtain clinical information regarding the pregnancy in both visual and discrete data formats. Conduct standardized, comprehensive fetal assessment, and maternal assessment with capacity to archive and review serial scans.
3. Refine and collate operational and clinical information into a clinically useful product, and support evidence-based decisions with standardized content, calculations and metrics using single source for reproducible images. Management and interventions could be more precisely targeted for optimal outcome for pregnant persons and neonates(s).
4. Distribute the information for downstream clinical care in collaboration with other providers and subspecialist to optimize care of the infant(s) or pregnancy conditions.
5. Support informed decision making with timely and evidence-based information on diagnosis and prognosis.
