Attachment 2 - Addendum to FAR 52.212-1 Instructions to Offerors

Submission of Offers: Offers shall be submitted by the due date and time provided on the SF1449 block 8 to Susan Ruggles at sruggles@usgs.gov.

The Government intends to award a single-award Indefinite Delivery Indefinite Quantity (IDIQ) contract to the responsive and responsible offeror whose offer conforms to the solicitation and has been determined to be the most advantageous and best value to the Government.

All evaluation factors other than cost or price, when combined, are significantly more important than cost or price. It should be noted that price does not have a definite adjectival rating but will be evaluated based on reasonableness.

The Contracting Officer will screen proposals to identify and eliminate any proposal that are so incomplete as to preclude a meaningful evaluation of technical/past performance/non-price factors.

One (1) electronic copy submission:

1: Offer Form (Standard Form 1449) – original signature and date; Contract Administration Data; Unique Entity ID; Acknowledgement of Solicitation Amendments if applicable.

2: Pricing

Complete the Schedule of Items, Section B. Include anticipated labor categories, with detailed description, to fulfill work requirements outlined in Section B-4 Performance Work Statement. Materials and supplies are not evaluated since they shall be priced at commercial rates without markup. Price will be evaluated based on reasonableness.

3: Technical

Technical Experience of Vendor and Qualifications of Vendor Personnel: Offerors shall submit information detailing the type of experience the vendor has in performing maintenance and repair services similar in nature to those specified herein. In addition, Offerors shall provide qualifications of service personnel that the vendor is proposing that will perform the services specified herein.

4: Past Performance

The offeror shall submit references from three (3) customers that have purchased similar maintenance and repair services within the last three (3) years. Each reference shall include: the company name, address, point(s) of contact phone number and email address(es). References provided may be contacted by the Government. Their statements will be used in the evaluation of the offeror’s proposal. In addition, any information obtained by the Government from references outside of the offeror’s quotation may also be used, in a similar manner, in the technical evaluation of the offeror’s quotation to the extent appropriate.

Past performance information provided by references, from Government records, or from other than the sources identified by the offeror may be used in the evaluation under any or all applicable evaluation factors. In addition to the foregoing, CPARS.gov will be used as a source of past performance information in the source selection process, in addition to any other past performance information required under this solicitation. All past performance information will be used for both the responsibility determination and the best value decision.

5: Representations and Certifications in Section E (FAR 52.204-24, 52.209-5, -11 and 52.212-3). If SAM registration is completed in full, clause 52.212-3 in this solicitation does not need to be completed and returned with the vendor's quote.

Questions and Answers (Q&A): Questions relating to this Solicitation shall be submitted by e-mail to sruggles@usgs.gov no later than 3 business days prior to the closing date of the solicitation. The Government will evaluate the questions, and if needed, an Amendment will be issued with the questions received and their associated answers. If further questions arise following the outcome of answers provided to initial questions, the Contracting Officer has sole discretion to determine if a new question closing date will be required. If a new question closing date is approved, an amendment will be posted.
