STATEMENT OF WORK

Ingersoll Gantry Machining Center Maintenance

1.0 SCOPE: The government has a need for on-call emergency repairs and annual inspection/alignment for the Government owned Ingersoll Masterline Gantry Machining Center equipped with the following Auxiliary Spindle Units: Nutator Part No. 990-3901- 19, Heavy Duty Vertical, Part No. 990-3895-19, Extended Horizontal, Part No. 990-3907- 19, 1400 mm Extension, Part No. 990-3902-19, and Double Right Angle, Part No. 990- 5009-19. Machine control is a GE Fanuc 15i-MB with a high-speed serial bus connected to a PC. This equipment is located in Bldg. 400 at Anniston Army Depot, Anniston, AL 36201-4199.

2.0 REQUIREMENTS: The contractor shall cover the cost of providing all things necessary to perform emergency repairs and annual inspection/alignment for the machining center, including labor, tools, diagnostic equipment, transportation, schematics, documentation, and any other services to repair the Machining Center. All repairs and annual inspection/alignment shall be performed by Contractor personnel that are experienced and highly trained in operating, maintaining, and servicing the equipment in accordance with the original equipment manufacturer's (OEM) published procedures.

3.0 PRINCIPAL PERIOD OF MAINTENANCE (PPM): PPM shall consist of 9-1/2 hours on weekdays, Monday - Thursday, 0700 AM through 1630 PM; and 8-1/2 hours on Fridays, 0700 AM through 1530 PM. No hours that will result in premium charges on Weekdays, Saturdays or Sundays, or Holidays will be worked without prior approval.

4.0 EMERGENCY REPAIRS: The contractor shall provide onsite service support within three (3) working days after notification that services are required for an emergency repair. Emergency repairs consist of any work that is necessary to restore the equipment to a 100% performance level.

4.1 The contractor shall respond to the Government's request for technical support within twenty-four (24) clock hours after notification that services are needed.

4.2 The contractor shall use only new OEM repair parts or components, or certified replacements of equal quality, to repair the equipment. If repair parts are not available on-site, contractor shall provide all repair parts and maintenance supplies and invoice separately to the Government.

4.3 All quoted prices shall be consistent with the Contractor's published prices that are in effect at the time of services. Installation of any part or parts, which exceeds $500.00, must be approved by Contracting Officer (KO) prior to installation.

4.4 All requests for service and replacement parts placed against this contract will be initiated by the Contracting Officer Representative (COR) located at the Anniston Army Depot.

4.5 All replaced parts shall remain property of the Government unless a credit is given with the replacement with a new part and is approved by the KO.

5.0 SEMI ANNUAL MACHINE ASSESSMENT: The Contractor shall perform a semi-annual machine assessment to inspect the mechanical and electrical / control systems on the machine.

5.1 One mechanical technician and one systems engineer will perform a machine audit twice a year.

5.2 At the conclusion of the audit, the contractor shall provide a report to ANAD within 10 working days. The report shall include all areas of the machine audited and will identify any areas of the machine which are considered in need of repair.

5.3 The contractor shall provide upon request from ANAD quotes for service to repair the machine and or replacement parts for any or all items identified in the report.

5.4 The contractor shall, upon approval from the KO, order the required replacement parts and schedule the service and installation of the parts.

6.0 ANNUAL INSPECTION AND ALIGNMENT: On an annual basis, the contractor shall make all adjustments to the Machining Center to insure it meets the enclosed alignment and tolerance requirements and provide comparisons of original Data Acquisition System (DAS) map and DAS map upon completion of all adjustments.

6.1 The Contractor shall level runways with DAS, check runway parallelism, check machine squareness, laser align all axes, check head tracking, check spindle unit offsets/sweeps, machine inspection, and make all required software changes and document these changes. Printouts of all measurements must be given to the Government point of contact (POC) as soon as possible after the alignment has been completed.

6.2 The Contractor shall make all reasonable effort to align and adjust the Machining Center within the original machine design specifications (SEE TABLE 1).

6.3 Contractor shall provide recommendations, in writing, for the Government to maintain the Machining Center within proper alignment and accuracy requirements.

6.4 Contractor shall provide all tools, labor, transportation, repair parts, documentation, maintenance supplies, test equipment, and any other related services, to inspect, align, and adjust the Machining Center.

6.5 The contractor shall perform work in accordance with the best commercial practices without unnecessary delays. Contractor personnel shall have the expertise to perform all repairs and adjustments for this equipment.

6.6 The Government reserves the rights to have Contractor perform random sample tolerance checks at various points to ensure that test results are repeatable. The random sampling shall be performed at no additional cost to the Government.

6.7 Technical surveillance of services will be performed by the Government's designated POC. Correspondence or communications of contract matters will be strictly between the Contractor and the Contracting Officer, Anniston Army Depot, ATTN:

AMSTA-AN-CT, 7 Frankford Avenue, Anniston, AL 36201-4199.

6.8 REPORTS: The contractor shall, at the end of each service visit, provide the COR with a written report within 10 days. The written report shall contain the following.

(a) General condition of equipment.

(b) Corrective Action Statement: Status of system and description of malfunction, repair time, parts used, actual cost of required repair parts and labor to complete the job.

7. DOCUMENTING MALFUNCTIONS AND SERVICE CALLS: The contractor shall record service calls on the SIOAN Form 698 to include the number of labor hours, labor rate, list of parts, travel expenses, quantity, part number, and the price. The Government will provide the Maintenance Call Record, SlOAN form 698, which will also be used by the Government to record date and time of notification, call number, and date and time of arrival/departure of Contractor personnel.

8. TRAVEL EXPENSES: The contractor shall directly charge travel expenses for each individual service call under the terms of this contract and shall be based on reasonable actual costs incurred in accordance with Public Law (PL) 99-234.

9. NON-CHARGEABLE ITEMS: The contractor shall not charge to the Government for:

(a) Remedial maintenance performed during the PPM or scheduled extension thereof.

(b) Remedial maintenance required within forty-eight (48) hour period due to a recurrence of the same malfunction.

(c) Time spent by Contractor personnel awaiting the arrival of additional technical support or delivery of parts, equipment, tools, etc. after a service call has commenced.

10. CONTRACTOR'S POINT OF CONTACT: The Contractor shall provide the Government with the name and telephone number of a designated POC and shall make arrangements to enable service personnel to receive notifications of service request.

11. GOVERNMENT POINT OF CONTACT (POC): The Government will appoint a knowledgeable technician as the POC with authority to schedule and coordinate emergency repairs through the Contracting Officer.

12.0 GOVERNMENT RESPONSIBILITIES:

12.1 The Government will provide adequate storage space for spare parts, adequate working space, including telephone, heat, light, ventilation, electric current and outlets for the use of Contractor personnel. These facilities will be within a reasonable distance of Machining Center and will be provided at no extra charge to the Contractor.

12.2 The Government POC will be present at all times during equipment services.

12.3 Subject to security regulations, the government will provide the Contractor access to perform equipment services. If access cannot be provided due to security requirements, the Contractor shall have a reasonable period of time to provide personnel who can be granted access to Government facilities.

12.4 TECHNICAL/CONTRACT LIAISON: The Government POC will be the technical liaison responsible for scheduling and surveying emergency repairs. Correspondence of contract matters will be between the Contractor and the Contracting Officer, Anniston Army Depot, AMSTA-AN-CT (Bldg. 221), 7 Frankford, Avenue, Anniston, AL 36201-4199.

12.5 HAZARDOUS WASTE: The Government will provide containers for the collection and disposal of all waste to be removed from Government owned equipment.

The COR or ANAD Maintenance POC will be notified to coordinate the removal of any coolant, oils and fluids. The contractor shall make every effort to prevent spills.

13.0 FACILITY ACCESS: The contractor shall, upon notification of contract award, complete the Request for Visitor Access Form 380-2. The contractor shall maintain as a minimum one (1) certified technician cleared for access to ANAD at all times during the performance period of the contact. The Contractor’s Personnel shall complete any and all Forms and training required for access to Anniston Army Depot that is deemed necessary during the contract period of performance. The contractor’s technician shall coordinate the planned arrival date with the Contracting Officers Representative (COR) or ANAD Maintenance POC located in the Directorate of Public Works, Equipment Support Division located in Building 133.

14. SCOPE CHANGES: The Contractor shall not proceed with any verbal or written change(s) to the scope of work from sources other than the Contracting Officer, who will provide changes in writing.

15. Access and General Protection/Security Policy and Procedures:

A. This standard language is for contractor employees with an area of performance within an Army-controlled installation, facility, or area. The contractor and all associated subcontractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services, or Security office. The contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA, and/or local policy. In addition to the changes otherwise authorized by the change clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

B. For contractors that do not require a Common Access Card (CAC), but require access to a DOD facility or installation. Contractor and all associated subcontractor employees shall comply with adjudication standards and procedures using the National Crime Information Center interstate identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

C. If necessary, Contractor shall acquire and return all issued U.S. Government Common Access Cards, installation badges, and/or access passes.
