About this file
This document is an attachment outlining Hospital Standards and Requirements for contractors at the Department of State Hospitals - Atascadero (DSH-Atascadero) in California. The document details four key policy areas: dress code, neckwear, tool control, and influenza vaccination requirements that contractors, their personnel, and subcontractors must follow while providing services at the facility.
The dress code strictly prohibits khaki clothing, requires photo ID badges to be worn properly, restricts clothing with certain logos or messages, and mandates appropriate coverage. The neckwear policy prohibits items that encircle the neck except for clip-on attachments and state-issued lanyards. Tool control requires inventory forms for a maximum of 10 tools/equipment items and strict accounting procedures. The influenza policy requires either proof of vaccination or completion of a declination form, with unvaccinated providers required to wear masks during flu season in patient areas. The facility offers free flu vaccines to contractors upon request.
