PERFORMANCE WORK STATEMENT (PWS)

Alinity S Analyzer Maintenance

1. DESCRIPTION OF SERVICES:

1.1 This will be a service contract for maintenance on one Abbott Alinity S analyzer, to include all labor, tools, equipment, transportation and parts required to support 24 hours/7 days a week operation. Contract will included unlimited unit-related repair. All equipment shall be maintained in proper operating condition as specified by the manufacturer. Vendor must have access to Original Equipment Manufacturers (OEM) parts, manuals and schematics to perform the service. Vendors must have their own schematics available on site at time of service. ALL

REPAIRS SHALL BE PERFORMED BY A MANUFACTURER-TRAINED TECHNICIAN. Contractor will follow manufacturer's recommendation for PM, repairs, calibrations and modifications. Service provided shall be

Plasma Sterilizer systems located at Joint Base San Antonio, Texas.

1.2. Service shall include all travel, labor, parts, materials, manufacturer's authorized field modification instructions (FMIs), individual tools and test equipment required to maintain the software.

1.1. 100 % of the time, within 24 hours after responding during contract hours of coverage.

1.4. TELEPHONE CUSTOMER SUPPORT:

1.4.1 Telephone customer support is the service required to restore the software to an operable condition in the least amount of time. It will consist of unlimited telephone support service. A toll-free 800 line must be available for users to call to get assistance in correcting any software problems as needed.

1.4.2. The contractor shall be available from 8 A.M. to 5 P.M. , Monday through Friday, (excluding Federal holidays) to receive calls for assistance.

1.4.3. The contractor shall provide an e-mail address for non-urgent questions and other customer support correspondence.

1.5 SOFTWARE SUPPORT:

1.5.1. The contractor shall provide all operational system and data maintenance. These include the right to upgrade software, software patches, software updates, maintenance releases, firmware updates, on-site software maintenance.

1.5.2 One week advance notice to the user is required for any expected changes. Free technical support shall also be provided to assist with installation of upgrades.

1.5.3 The contractor shall provide annual on-site in-service software training to users as requested.

1.5.4. The contract shall respond to requests for software and maintenance support within eight (8) hours during contract hours of coverage.

2. GOVERNMENT FURNISHED PROPERTY: None

2.1. GOVERNMENT FURNISHED SERVICES: The government shall furnish the contractor all reasonable amounts of electric power and other utilities necessary to perform the above service.

3. GENERAL INFORMATION:

3.1 DESIGNATED REPRESENTATIVES: The names of the designated Contracting Officer Representatives

(COR) of the Contracting Officer and personnel authorized to place service calls shall be provided to the Contractor at the time of contract award.

3.2 TRANSPORTATION: No equipment shall be removed from Wilford Hall Ambulatory Service Center

(WHASC) without prior written approval of the Contracting Officer or COR. Equipment authorized for removal for repair shall be done at no additional cost to the Government.

3.3 ADMINISTRATIVE CONTROL:

3.3.1 On-site, in service training and on-site maintenance support services will be coordinated by the Contracting

Officer or COR. The contractor will be called by phone when services are needed. The Contractor's service personnel shall report to the Biomedical Engineering Flight, Building 3450, JBSA-Lackland, Texas, prior to reporting to the work-site.

3.3.2 Contractor's service technician shall prepare a service report for each service (scheduled in-service or unscheduled service call) and obtain the signature of the user within the using activity, certifying the requested service were rendered and accepted. The service report shall include the following minimum information:

(1) Government contract number.

(2) Index number (equipment identification data).

(3) System nomenclature, room number, location, model and serial number.

(4) Purpose of service (scheduled in-service or unscheduled service call).

(5) Start date/time and completion date/time.

(6) Description of defect and corrective action taken.

(7) Name of Service Technician.

(8) Signature of individual acknowledging services.

3.3.3 A copy of any service report should be emailed to tracy.wilhelm.1@us.af.mil within five (5) days of date of service. Invoices should be submitted via WAWF.

3.4 GATE CONTROL: Some travel service locations will have limited access to installations/bases with controlled gate openings and closures. Unscheduled gate closures by the Security Police may occur at any time and personnel entering or exiting the installation may experience a delay. Privately owned vehicles of contractor's personnel are subject to being searched pursuant to applicable regulations. The award of this contract does not create a right to have access to any military installation. Any violation of an installation regulation or of state or federal statute may result in the termination of the privilege to enter one or more military installations.
