Statement of Work 737TRG Basic Military Training (BMT)
TacMed Mannequins February 2024
1. Overview: Basic Military Training (BMT) Trainees receive Tactical Combat Casualty Care (TCCC) during their appropriate week of training (WOT). During this training, they are provided a scenario where they are to administer a form of medical care on a mannequin, in lieu of placing a tourniquet or other form of medical substance on themselves or their wingman, when practicing or conducting their progress check. During the sixth WOT, trainees arrive for Primary Agile Combat Employment Range Forward Operations Readiness Generation & Employment (PACER FORGE); a simulated combat zone. During their time at PACER FORGE there are numerous scenarios where they are to apply the techniques learned during TCCC on a mannequin. In addition, they apply the carrying techniques learned during TCCC. Approximately 900 Basic Military Training (BMT) trainees and guardians arrive to PACER FORGE per week.
2. Description of Work. The Contractor shall provide all necessary labor, materials, and equipment to deliver:
2.1. Two (2) Upper Trauma Trainer with Amputated Arms
2.2. Two (2) Upper Trauma Trainer with Full Arms
2.3. Four (4) Amputation Trauma Trainer
2.4. Four (4) Two-way Communication (TWC) System
2.5. Four (4) Vital Signs Monitor
2.6. Two (2) days On-site Service and Training Support
2.7. Annual Repair Program
3. General Information:
3.1 Change Management: Any changes to the project scope, timeline, or budget must be communicated in writing and approved by both the contractor and the Contracting Officer (CO). Change requests will be evaluated based on their impact on project objectives and resources.
3.2 Support/Warranty Services: All commercial goods and services sold to the Government are warranted to the Government to be free from defects in material and workmanship for the standard manufacturer’s warranty period. Manufacturer’s warranties for individual pieces consistent with industry standards for coverage and length must be provided along with contact information and operation manuals if any.
3.3 Site Program Manager/Point of Contact (POC): The Contractor must designate an on-site project manager as the Site POC for their individual projects. The Site POC or alternate(s) must be available during normal duty hours until project completion. The Site POC must be the interface for all work site communications with the Government, including quality, safety, and discrepancy matters.
3.4 Site Maintenance and Clean Up: All shipping and packaging materials must be removed by the contractor personnel. Contractor must ensure all leftover wrappings, boxes, paper, and any other related refuse is picked up and properly disposed of off-site. All debris, trash and refuse must be removed from the site. At completion of project, area must be left clean and in a final professional appearance. All contractor generated refuse and waste must be stored in a waste container provided by the contractor and hauled from the project site to a disposal area to be selected by the contractor and must be located outside the physical boundaries of the installation. Recycle of project and packing materials is required. The contractor must protect adjacent property, offices and their contents from dust, dirt or other materials.
3.5 Occupational Safety and Health: The Contractor must establish a safety and occupational health program consistent with requirements of the Occupational Safety and Health Administration (OSHA), Title 29 CFR. The Contractor must also provide Personnel Protective Equipment (PPE) and training consistent

TACMED Mannequins with applicable standards. The Contractor must comply with all national and local fire codes and policies as needed to protect USAF resources (facilities, equipment, and USAF personnel). The Contractor must notify the Government representative(s) of any malfunctioning Government Furnished Equipment (GFE). The Contractor must immediately report all mishaps involving serious personnel injuries, fatalities, and/or Government property/equipment damage occurring on the job to the responsible Safety Office and the Government representative(s). The Contractor must fully cooperate with any AF led safety investigation and must not disturb the mishap scene until proper officials authorize movement.
3.6 Safety: The contractor must be responsible for following safety and security guidelines within the area in which work is being performed, to include securing any contractor owned tools or equipment, and any on site storage being utilized.
3.6.1. The contractor must maintain accountability and control of any keys and/or passes provided and must return them to the Government upon completion of work.
3.6.2. The contractor must prevent its personnel from entering any area other than the designated work area to complete the project herein.
3.6.3. The contractor must maintain a means of egress within all designated work areas to comply with fire codes.
3.6.4. Work must comply with applicable building standards and codes, specifically: National Electrical Code (National Fire Protection Association (NFPA 70) and National Life Safety Code (NFPA 101).
4. Base Fire Regulations. The contractor must comply with all JBSA Lackland Fire Regulations. No welding will be permitted without obtaining a welding permit issued by the JBSA Lackland Fire Department. To obtain a hot work (welding) permit, the contractor must call JBSA Lackland’s Base Fire Dispatch Center at (210) 671-2921 to request a hot work permit. All fire and safety regulations must be enforced.
5. Hours of Operation: The contractor has access to the site and may work between the hours of 0600 and 1800 LT, Monday through Friday, excluding Federal holidays. At the option of the government and with approval of the Contracting Officer, work may be permitted on Federal holidays, weekends, or after 1800 LT at no additional cost to the government.

6. Inspection/Acceptance: The Government representative(s) must inspect all work performed by the contractor.
The Government representative(s) will document all inspections. Validation between the contractor and the Government representative(s) that all products has been delivered according to manufacturer guidelines and customer specifications.
6.1. The contractor must, without charge, replace or correct work found by the Government not to conform to contract requirements, unless in the public interest the Government consents to accept the work with an appropriate adjustment in contract price. The Contractor must promptly segregate and remove rejected material from the premises.

6.2. Unless otherwise specified in the contract, the Government must accept, as promptly as practicable after completion and inspection, all work required by the contract or that portion of the work the CO determines can be accepted separately.
6.3. Acceptance must be final and conclusive except for latent defects, fraud, gross mistakes amounting to fraud, or the Government’s rights under any warranty or guarantee.
7. Security Requirements, Clearances and Badges. All installation personnel assigned to this requirement must be a United States citizen, must have a Local Area Check (LAC) conducted by the contractor and must identify possible security threats to the installation Security Forces.
8.1. The contractor must provide the Government with the names, social security numbers, driver’s license numbers and state of issue, date of birth, and country of birth of personnel performing on this task before they will be allowed on the installation. This information must be provided at a minimum of 72 hours prior to performance start date for the Government representative(s) to coordinate installation access for contract personnel. Contract personnel not meeting the minimal standards will not be issued an installation pass and will not be allowed access to the installation.
8.2. Personnel employed by the contractor must get a pass for their privately owned vehicles at the installation
Visitor Reception Center, with proof of following:
1. Liability Insurance
2. Current License Plates
3. Current State Inspection Sticker (If required)
4. Valid State Driver License
5. A phone number for sponsor on base
6. Vehicles owned by the contractor with the company name permanently printed on them are not required to obtain a pass as long as a current work order is presented at the time of entry. However, current liability insurance, state inspection sticker, and registration is required. The person driving the vehicle must have a valid operator license for the type of vehicle.
8.3. Upon completion or termination of the contract or expiration of the identification passes, the contractor must ensure that all base identification passes issued to contractor employees are returned to the issuing office. If a contractor employee has been terminated, the pass will need to be retrieved and returned to Security Forces so that employee does not have base access. If the pass is not retrieved, then SF will need to be notified so base access is not allowed. Upon completion of the contract return all issued installation access passes to the issuing authority. Failure to comply with these requirements may result in withholding of final payment.
8.4. The contractor is required to provide identification badges for its employees. All contractor personnel must wear these badges while on duty on the Government site. Badges are required to identify the individual, company name, and be clearly and distinctly marked as contractor. Size, color, style, etc. are to be mutually agreed upon by contractor and Requiring Activity.
8.5. Firearms are prohibited on all JBSA Installations.
