About this file
This is a Specialized Experience Form template used to document contractor qualifications and past project performance for federal contract proposals.
The form requires offerors to submit exactly three (3) projects demonstrating relevant experience; submissions with fewer than three projects may receive lower ratings, while more than three will not be evaluated. For each project, contractors must provide: contractor information including firm name, address, DUNS number, CAGE code, and points of contact; contract/task order number and award date; project title and location; role performed (prime contractor, subcontractor, joint venture, or other); percentage of work performed by the company; prime contractor information if applicable; and government agency/contracting officer contact details. The form also requires a detailed description of the project's scope of work, the offeror's specific roles and responsibilities, and how the work performed is relevant to the current solicitation. Additionally, offerors must document any difficulties, obstacles, or unforeseen conditions encountered and the resolutions implemented; the performance period with contract start date and original/projected completion dates; actual completion date and reasons if not completed; whether the project is at least 50% complete; any contract terminations with explanations; and original versus final contract prices with explanations of any differences. Projects that are less than 50% complete may result in lower ratings.
