About this file
This is a Certificate of Compliance to Berry Amendment form (Attachment 4) for solicitation FA700024Q0005, which requires offerors to certify their compliance with DFARS 225.7002 restrictions on food, clothing, fabrics, specialty metals, and hand/measuring tools. The form requires detailed documentation of all cloth/textile components used in uniform manufacturing, including supplier names, addresses, and manufacturing facility locations.
The form consists of two main sections: 1) A certification section requiring the company official's signature and date, and 2) An identification section where offerors must list all uniform components and their manufacturing sources. The document emphasizes that both end items AND components must comply with the Berry Amendment, including items like buttons, zippers, thread, clips, fasteners, and ties. Additionally, there is a Place of Performance section where offerors must indicate if they intend to use facilities at different addresses than their main location. Non-compliance with the Berry Amendment requirements will result in disqualification from award consideration.
