About this file

This is a Past Performance Questionnaire Template used by the National Oceanic and Atmospheric Administration (NOAA) for Solicitation Number 1305M326Q0368 regarding a Senior C++ programmer position to modify the COMPASS model for fish ecological predictions. The template is designed to be completed jointly by offerors/major subcontractors and government assessors to evaluate contractor performance on previous relevant contracts. Offerors must complete Sections I and II, providing solicitation data and current or historical contract information, then submit these sections to appropriate government assessors for verification and validation. Assessors complete the questionnaire in its entirety and return it to the Contracting Office via email to grace.lapierre@noaa.gov and Charles.kendall@noaa.gov. Offerors must also submit copies of Sections I and II with their proposals by the solicitation closing date.

The questionnaire captures assessor information including name, title, contact details, and role during contract administration (Procuring Contracting Officer, Administrative Contracting Officer, Contract Specialist, Contracting Officer's Representative, Technical Project Lead, Quality Assurance Specialist, or DCAA Auditor). Section IV requires assessors to provide an overall performance rating based on contract requirement fulfillment, quality and timeliness of delivery, cost management, inter-party communication, proactive management, and customer satisfaction. Performance ratings range from Exceptional (exceeds most requirements with few issues) to Unsatisfactory (does not meet requirements with ineffective corrective actions), with intermediate ratings of Very Good, Satisfactory, and Marginal, or Not Applicable if information is not recent or relevant.
