Attachment 001 Performance Work Statement and QASP
Hazardous Waste Service USDA Stuttgart and Almyra Arkansas
GENERAL INFORMATION
1. Scope of Work:
1.1. Hazardous waste removal and disposal service for two USDA ARS Arkansas facilities. See
Attachment 002 for inventory to be removed.
1.2. CLIN 0001 Location: Dale Bumper National Rice Research Center (DBNRRC) located at
2890 Hwy 130 East, Stuttgart, AR 72160
1.3. CLIN 0002 Location: Stuttgart National Aquaculture Research Center (SNARC) located at
2955 Hwy 130 E, Almyra, AR 72003.
1.4. Period of Performance: 45 Days After Receipt of Order (ARO.)
2. Definitions:
2.1. CLIN – Contract Line Item, numbered. Established in the solicitation, example “0001.”
2.2. Working Hours – 8AM-4:30PM Central
2.3. Business Days – Standard business days with holiday exceptions as noted:

CONTRACTOR PERFORMANCE REQUIREMENTS
3. CLIN 0001 Stuttgart Location Technical Requirements/Tasks
3.1. The Contractor shall supply all labor, materials, safety equipment, vehicles, licenses, and special permits required for packaging, manifesting, transporting and disposal of all hazardous materials and other non-regulated materials at DBNRRC Stuttgart, AR
3.2. The list of materials for this requirement is provided in Attachment 002 Appendix A.
3.3. Requirements prior to arrival on site:
3.3.1. The contractor shall identify transportation and disposal requirements for the material listed in Attachment 002 Appendix A in accordance with State and Federal regulation and applicable law.
3.3.2. The contractor shall provide appropriate containers for the material listed in Attachment
002 Appendix in accordance with State and Federal regulation and applicable law.
3.3.3. Appropriate personal protective equipment PPE in accordance with State and Federal regulation and applicable law shall be secured prior to arrival on site. The Government is not responsible for providing PPE.
3.3.4. Manifest preparation shall occur in accordance with sections 5.1, 5.2, and 5.3 below.
3.3.5. Scheduling for pickup date and time shall be in writing with the Hazardous Waste
Coordinator provided in the contract according to the business days and working hours defined above in section 2 within three (3) business days prior to arrival on site.
3.4. Requirements for on-site pickup:
3.4.1. Appropriate PPE in accordance with State and Federal regulation and applicable law shall be worn during service delivery.
3.4.2. The Contractor shall sort waste by hazardous properties, pack, label, and transport the waste materials to a disposal site approved by the receiving State and EPA, in DOT-approved shipping vehicles in accordance with applicable Federal and State regulations for such shipments.
3.4.3. Each container for shipment shall be labeled clearly.
3.4.4. The Government will not be liable to the Contractor for the cleanup of spills, accidental releases to the environment occasioned through the fault of the Contractor
3.4.5. Once on-site service delivery begins, all work shall be completed within three (3) business days.
3.5. Requirements after completion of on-site work:
3.5.1. Certificates of Disposal shall be forwarded to the HWC within 90 days of the date the waste was received at the disposal facility unless otherwise specified see section 3.5.3.
3.5.2. The Certificate of Disposal shall list each container received at the treatment or disposal facility; document the date and method of treatment or disposal and include the appropriate
EPA Treatment Code Number if available; and list the name and location of the treatment/disposal facility.
3.5.3. The Government recognizes that Certificates of Disposal may take up to twelve (12) months to be received due to the lack of incineration capacity. The Contractor is expected to make every effort to provide certificates as quickly as possible, and shall report to the HWC, on a quarterly basis, the status of pending certificates of disposals.
3.5.4. If material is disposed of in a secured chemical landfill facility, the Certificate of
Disposal shall also indicate the location within the landfill where the waste container was buried through the use of cell numbers, coordinates or other appropriate identifiers.
4. CLIN 0002 SNARC Almyra Location Technical Requirements/Tasks
4.1. The Contractor shall supply all labor, materials, safety equipment, vehicles, licenses, and special permits required for packaging, manifesting, transporting and disposal of all hazardous materials and other non-regulated materials at SNARC, Almyra AR
4.2. The list of materials for this requirement is provided in Attachment 002 Appendix B.
4.3. Requirements prior to arrival on site:
4.3.1. The contractor shall identify transportation and disposal requirements for the material listed in Attachment 002 Appendix B in accordance with State and Federal regulation and applicable law.
4.3.2. The contractor shall provide appropriate containers for the material listed in Attachment
002 Appendix in accordance with State and Federal regulation and applicable law.
4.3.3. Appropriate personal protective equipment PPE in accordance with State and Federal regulation and applicable law shall be secured prior to arrival on site. The Government is not responsible for providing PPE.
4.3.4. Manifest preparation shall occur in accordance with sections 5.1, 5.2, and 5.3 below.
4.3.5. Scheduling for pickup date and time shall be in writing with the Hazardous Waste

5. Manifest Preparation and Shipping Data Requirements:
5.1. The Contractor shall provide all data required for waste transportation, treatment, or disposal, and for completion of annual hazardous waste generator reports if required by the regulatory agency or jurisdiction. Uniform Hazardous Waste Manifests (EPA Form 8700-22 or latest revision thereof) shall be properly completed by the Contractor for each waste and shall list each transportation container including all nonhazardous waste or hazardous materials shipped.
5.2. All manifest information shall be neatly printed or typed and minimally contain that information required by applicable Federal, State, and local hazardous chemical waste regulations.
5.3. Waste manifests shall be submitted to the Hazardous Waste Coordinator (HWC) for review and signatures during routine operating hours and prior to the shipment of wastes off-site.
5.4. The manifest shall be signed by the Contractor's transporter in the presence of the HWC.
5.5. Correction of errors in packing, labeling, or shipping shall be the responsibility of the
Contractor. Costs associated with these errors shall also be the responsibility of the Contractor.
5.6. Manifests shall be specific to the pickup location, manifests shall not be combined for both locations.
6. Contractor’s Key Personnel: The contractor shall be proficient in “Lab Pack” waste disposal procedures and familiar with all applicable Federal, State, and local regulations governing the handling, packaging, storing, transportation, and disposal of hazardous waste and other non-regulated material.
7. Security Requirements: All contractor personnel shall always be in the presence of a USDA escort while on location.
8. Quality Assurance Surveillance Plan (QASP): The Government shall establish a QASP as a means of evaluating whether the Contractor is meeting the performance standards/quality levels identified in the PWS to ensure that the government pays only for the level of services received.
