About this file

This is a Performance Risk Assessment Questionnaire used by federal agencies to evaluate contractor performance on existing or past contracts during the contract award process.

Part I requires the offeror to provide contract identification information, including the contractor/company name, address, program identification, contract number, contract type, prime contractor name (if applicable), contract award date, forecasted or actual completion date, and nature of the contractual effort or items purchased. The offeror must also identify their representative with name, title, contact information, and email address.

Part II, completed by a point of contact/respondent, evaluates contractor performance across six key criteria: (A) Compliance of Products, Services, Documents, and Related Deliverables to Specification Requirements and Standards of Good Workmanship (rated as Exceeds/Meets/Failed Contractual Requirements); (B) Effectiveness of Project Management including subcontractor control (rated as Exceptional/Satisfactory/Unsatisfactory); (C) Timeliness of Performance for Services and Product Deliverables including administrative aspects (rated as Exceeds/Meets/Failed Contractual Requirements); (D) Effectiveness in Forecasting and Controlling Project Cost (rated as Exceptional/Satisfactory/Unsatisfactory); (E) Commitment to Customer Satisfaction and Business-like Concern for Customer Interest (rated as Exceptional/Satisfactory/Unsatisfactory); and (F) Overall Satisfaction (rated as Extremely Satisfactory/Satisfactory/Unsatisfactory). The questionnaire requires detailed comments for ratings that exceed or fail contractual requirements, or are rated unsatisfactory. Additional sections request general performance comments, identification of other relevant past efforts by the contractor, and respondent identification information. Part III directs return of the completed questionnaire via email to the contracting officer within three days.
