About this file

This document contains a certification form for small business size and socioeconomic status for proposed subcontractors not registered in the System for Award Management. The form requires subcontractors to certify their small business status under the applicable North American Industry Classification System code and size standard. Subcontractors must identify if they are a small business, small disadvantaged business, woman-owned small business, veteran-owned small business, or service-disabled veteran-owned small business. The certification form must be signed and dated by an authorized official of the subcontractor.

The related federal contract opportunity is a solicitation to provide fielding agile supplies, support and training services. Amendment 0001 revises language in section L.4.1.2, corrects administrative numbering errors, and replaces Attachment 0005. The solicitation was issued by the Department of the Army Materiel Command TACOM Life Cycle Management Command.
