About this file
This is a First Article Test Waiver Worksheet template used by contractors to request exemptions from First Article Testing (FAT) requirements in federal procurement contracts.
The form captures current acquisition information including contractor name, part number, National Stock Number (NSN), solicitation/contract number, and Technical Data Package (TDP) references. Contractors may request either a partial waiver (exempting specific tests) or a full waiver (exempting all FAT requirements). The justification section provides five primary grounds for waiver requests: (A) successful completion of FAT on a recent contract for identical items manufactured at the same facilities using identical equipment, processes, and procedures; (B) successful FAT completion on a similar item tested to the same requirements; (C) material in continuous production with documentation of the last successful FAT approval; (D) successful FAT performance on the next higher assembly to eliminate duplicative lower-assembly testing; and (E) other justifications with supporting documentation. The form requires contractors to disclose pertinent information including any quality issues, Product Quality Deficiency Reports (PQDRs), manufacturing data changes, drawing revisions, subcontractor changes, waivers/deviations, and Material Review Board (MRB) actions. All submissions must be made on company letterhead and signed by a company agent, accompanied by supporting documentation as specified in the applicable solicitation or contract. The form includes sections for attaching supporting files and capturing submitter contact information.
