About this file

This is a First Article Test (FAT) Waiver Worksheet template designed for contractors to request waivers from FAT requirements on federal contracts. The form must be submitted on company letterhead and signed by an authorized company agent, with all supporting documentation attached.

The worksheet accommodates two types of waiver requests: partial waivers (exempting specific tests only) and full waivers (exempting all FAT requirements). Contractors must specify which TDP references, specification paragraphs, and drawing notes apply to their waiver request. The justification section provides five pathways for requesting waivers: (A) successful FAT completion on a recent contract for the identical item using the same facilities, equipment, processes, and subcontractors; (B) successful FAT on a similar item tested to the same requirements; (C) material in continuous production with documented approval dates; (D) successful FAT on the next higher assembly to eliminate duplicate lower-assembly testing; and (E) other justifications with specific supporting documentation. The form requires disclosure of any quality issues, PQDRs, corrective actions, manufacturing data changes, subcontractor changes, or Material Review Board actions related to the item. The submission section requires the signatory to provide printed name, title, company name, address, and phone number, with certification that all statements are true and correct. Supporting documentation must be attached according to solicitation/contract requirements.
