SCOPE OF WORK

REMEDIAL REPAIR, PREVENTATIVE MAINTENANCE, AND CALIBRATION OF

SUPER MICROMETERS

1. GENERAL:

1.1. Purpose: The purpose of this Scope of Work (SOW) is to provide remedial repair, preventative maintenance, and calibration services on Rock Island Arsenal Joint Manufacturing and Technology Center (RIA-JMTC) Super Micrometers. The work identified in this Contract will be performed IAW the most current update of ANSI Z540 or ISO 17025 and ensure compliance with RIA’s ISO 9001 registration.

1.2. Equipment: Remedial repair, preventative maintenance, and calibration services, materials, parts and costs required by this SOW shall be applicable to RIA- JMTC’s Super Micrometers:

1.3. Performance Period: Period of performance is one (1) year from the date of contract award with four (4) option years.

2. SAFETY AND ENVIRONMENTAL:

2.1. Safety: The contractor is responsible for complying with all federal, state, and local safety and environmental regulations, including, but not limited to Occupational Safety and Health Act (OSHA), Environmental Protection Agency (EPA), Illinois Environmental Protection Agency (IEPA), and Army regulations. Contractor must adhere to all Rock Island Arsenal Safety requirements, policies and statutes. The contractor shall be responsible for obtaining any hot work/open flame permits required and coordinating with the Rock Island Arsenal Fire Department in accordance with (IAW) RIAR 420-2, Chapter #7 – Special Operations, Welding, Cutting and Tar Kettles, specifically Section 7-2, pages 27 and 28. To contact Fire Department for flame permit, contractor shall call (309) 782-5948 or 2911.

2.2. Environmental: All hazardous wastes, acid, and contaminated material generated by the execution of this project will be disposed of IAW all applicable federal, state, and local laws and regulations. All transport and decontamination will be performed IAW all applicable interstate, federal, state, and local laws and regulations.

Any materials and/or wastes generated from this effort shall be labeled with appropriate transportation/disposal documentation for off-site disposal to include but not limited to Hazardous waste manifests and associated Land Disposal Restriction. Prior to discussing any environmental issues with the environmental agency of primacy, the contractor shall contact the Contractor Officer Representative (COR).

2.3. Ozone Depleting Chemicals (ODCs): The contractor shall assure during execution of the tasks outlined in this SOW that none of the materials used contain Class I ODCs as defined by Public Law 102-484, Section 326.

3. REPORTING REQUIREMENTS:

3.1. Reporting requirements are required as part of the Service Acceptance Report (SAR), further detailed in paragraphs 5.7 and 5.9, below.

3.2. Additional Reporting Requirements are required as part of the SAR, further detailed in paragraphs 5.3 (for Calibration certification), and 5.8 (for Service Reports), below.

4. Detailed Specifications: This SOW is a performance-based specification. The objective of this project is to perform remedial repair, preventative maintenance and calibration services on the Super Micrometers, owned by and located at RIA-JMTC.

Work shall be performed on an “as-needed” basis. The Contractor shall provide services and materials to comply with manufacturer suggested preventative maintenance and part replacement. Adjustments to hardware shall include simple mechanical adjustments. Below are detailed specifications pertaining to the work included in this service contract.

4.1. General: The contractor shall be responsible for performing all remedial repair, preventative maintenance and calibration services for the Super Micrometers. All Contractor work shall be performed during normal business hours (Monday through Friday, 6:00 AM – 2:30 PM CST), except for Government holidays unless otherwise authorized and agreed to by the COR and the Contracting Officer. The Contractor shall provide and install repair parts from RIA stock if available or provide necessary parts upon request from the COR. Repair parts and installation shall carry the manufacturer's standard commercial warranty. The Contractor shall warrant that services performed are in accordance with original equipment specifications and shall carry the standard commercial warranty.

4.2. Preventative Maintenance Services: Preventative maintenance for the Super Micrometers shall be performed during the calibration as well as when on-call repair is required. Tasks performed by the project may include (but is not limited to) the following services:

4.2.1. Clean all surfaces of the machine.

4.2.2. Check and adjust machine level.

4.2.3. Check and clean scales.

4.2.4. Check friction screw; clean and lubricate.

4.2.5. Check all ways clean and lubricate.

4.2.6. Check for proper seating on circuit boards.

4.2.7. Check all cable connections.

4.2.8. Check all belts.

4.2.9. Check for proper corrections and parameters.

4.2.10. Check for proper hand wheel operation.

4.2.11. Make specific recommendations as to any part or parts that need to be replaced.

4.3. Issue written certification that machine is in good working order and that machine conforms to original equipment specifications (Government COR must be notified). All certifications must be performed prior to the current certification expires, and in accordance with the most current update of ANSI Z540 Compliance or ISO 17025. The contractor must complete the certification process on each machine within ten (10) working days after commencing work on that machine.

4.4. Remedial Repair Efforts: The Contractor shall furnish all labor and repair tools required to perform on-call repair, or remedial repair efforts which are discovered by the contractor during preventative maintenance task execution. These repair tasks shall include (but is not limited to) machine disassembly, equipment repair, mechanical component overhaul/machining, minor or major component replacement, control repair, electrical system replacement and repair, etc. Notification of remedial repairs efforts shall be accomplished as specified below.

4.4.1. Contractor Point of Contact: The contractor shall identify a Primary and Alternate notification Point of Contact (POC). The Government will assign a COR upon contract award. The COR will be responsible for the general oversight of the contractors performance under this SOW. The COR is not authorized to make changes to this SOW or contract terms and conditions. Rock Island Arsenal shall notify the contractor’s Primary or Alternate POC for all requirements.

4.4.2. For remedial repair services, the contractor shall be notified, and after physical inspection of the machine, the contractor shall prepare a cost estimate outlining machine deficiencies, and estimated price to correct machine deficiencies. The COR shall review the costs estimate then forward to the Contracting Officer who will determine if it’s fair and reasonable.

4.4.3. Remedial Repair Response: For remedial repair, the contractor shall be required to schedule service within 48 hours of telephonic, email, or written notification of remedial repair service requirement. Service must be initiated within five (5) working days of initial request unless the COR approves or requests an extension thereof.

Service must be completed within four (4) working days after commencing work. The COR may allow the Contractor to exceed the time limit if the situation is justified and will document it in his/her file.

4.4.4. Remedial Repair Discovery: During calibration and preventative maintenance activities, repair issues may be discovered by the contractor. The COR shall be notified immediately of any service requirements which are estimated to exceed the amount of this purchase order.

4.4.5. The COR is authorized to place calls for service and parts up to the amount stated in the purchase order. In no event is the Contractor authorized to exceed any cost limit set forth elsewhere in the purchase order.

4.5. The COR shall be notified immediately of any service requirements, which are identified as extraordinary repairs.

4.6. All boxes, parts and components which require shipping to RIA shall be accomplished by the servicing technician. Shipment of repair parts can be made to:

4.7. Contractor shall provide documented evidence that:

4.7.1. The service technician performing the work is factory trained to service the Super Micrometer. On-the-job training conducted by a factory trained individual designed to provide technical training equivalent to that provided by the factory may be substituted for factory training.

4.7.2. Service technicians have at least three (3) years’ experience servicing the Super Micrometer.

4.7.3. Contractor possesses the necessary equipment to make the required adjustments.

4.8. Final Service Acceptance Report (SAR): Upon completion of services, the contractor shall provide a final SAR to the COR. This report shall be submitted within 24 hours of completion of services. The SAR report shall contain detailed itemization of all parts and labor, the actual man-hours, a detailed description of what repairs were completed and the name and contact information of the technician who performed the repairs. The SAR report shall include the machine model, serial number and Rock Island Arsenal asset numbers. SAR’s shall be legible, either handwritten or electronic (preferred) in a Microsoft Word or Excel format. SAR’s shall be sent to the COR.

4.9. Allowable Costs: All travel, lodging, and meal costs associated with performance under this SOW shall be in accordance with the Joint Travel Regulation (JTR). All allowable costs will be figured using the listed standard rate for Illinois-Rock Island. The JTR can be found at https://www.defensetravel.dod.mil/perdiem/. Costs associated with any required service call shall be limited to:

4.9.1. Labor Costs.

4.9.2. Travel time from Contractor facility to RIA and return, and mileage (if driving technician’s personal vehicle).

4.9.3. Hotel @ per diem rate/technician.

4.9.4. Meals @ per diem rate/technician.

4.9.5. Car Rental and gas @ actual cost (if renting a vehicle).

4.9.6. Shipping/Freight @ actual cost.

4.9.7. Parts, tool or laser rental @ actual cost.

5. INSPECTION/FINAL ACCEPTANCE:

5.1. The COR will monitor contractor performance on this Purchase Order in accordance with the Quality Assurance Surveillance Plan (QASP) and the terms and conditions set forth in this SOW.

5.2. Upon completion of remedial repair, preventative maintenance, and/or calibration, the COR will inspect the services provided. If acceptable, the COR will provide their written acceptance to the Contracting Officer for final inspection of the services.

5.3. The COR will perform acceptance of service in Wide Area Workflow (WAWF) once the contractor has submitted their invoice and all work is completed.

6. FOOT NOTES

Foot note #1 AT Level I Training. This provision/contract text is for contractor employees with an area of performance within an Army-controlled installation, facility, or area. All contractor employees, including subcontractor employees, requiring access to Army installations, facilities, or controlled access areas shall complete AT Level I awareness training within 60 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever applies. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee to the COR (or to the contracting officer, if a COR is not assigned) within 60 calendar days after completion of training by all employees and subcontractor personnel. Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within an Army-controlled installation, facility, or area. The contractor and all associated subcontractors’ employees shall provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services, or Security office. The contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DoD, HQDA, and/or local policy. In addition to the changes otherwise authorized by the change clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

For contractors requiring Common Access Card (CAC). Before CAC issuance, the contractor employee requires, at a minimum, a favorably adjudicated National Agency Check with Inquiries (NACI) or an equivalent or higher investigation in accordance with Army Directive 2014-05. The contractor employee will be issued a CAC only if duties involve one of the following: (1) Both physical access to a DoD facility and access, via logon, to DoD networks on-site or remotely; (2) Remote access, via logon, to a DoD network using DoD-approved remote access procedures; or (3) Physical access to multiple DoD facilities or multiple non-DoD Federally controlled facilities on behalf of the DoD on a recurring basis for a period of 6 months or more. At the discretion of the sponsoring activity, an initial CAC may be issued based on a favorable review of the FBI fingerprint check and a successfully scheduled NACI at the Office of Personnel Management.

For contractors that do not require CAC but require access to a DoD facility or installation. Contractor and all associated subcontractor employees shall comply with adjudication standards and procedures using the National Crime Information Center interstate identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

Foot note #4 IWATCH (See Something, Say Something) Training. This standard language is for contractor employees with an area of performance within an Army-controlled installation, facility, or area. The contractor and all associated subcontractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR.

This training shall be completed within 60 calendar days of contract award and within 60 calendar days of new employees commencing performance, with the results reported to the COR no later than 60 calendar days after contract award.

Footnote #5 Impact on contractor performance during increased FPCON during periods of increased threat (contractor personnel working on an installation). During FPCONs Charlie and Delta, services may be discontinued/postponed due to higher threat.

Services will resume when FPCON level is reduced to level Bravo or lower.

Footnote #6 Random Antiterrorism Measures Program (RAMP) participation. Contractor personnel working on an installation are subject to participation in installation RAMP security program (e.g. vehicles searches, wearing of ID badges, etc.).
