E-QIP is a secure website that can be accessed from any computer system which has an Internet connecƟon. AddiƟonally, two Fingerprint Cards (FD 258) will need to be generated by the contracƟng firm by visiƟng a local police department or the request can be coordinated with a local FBI Field Office through a Chief Security Officer. The e-QIP InvesƟgaƟon Request Number, automaƟcally generated by e-QIP, is located on both the header and footer of the signature forms. These release forms (three total) and FD 258 should be mailed via Federal Express or UPS Express Mail directly to the following address: Contact CO for details. The SF 86 itself will be transmiƩed to the FBI electronically. The use of regular U.S. Mail channels may cause significant delays in processing the unescorted access request.
Completed forms should be provided to the assigned CSO at least 120 days prior to required access.
A Tier 5 invesƟgaƟon (as defined by the Federal InvesƟgaƟve Standards) will be performed for all contractors who require unescorted access under this delivery/task order.
