Statement of Work

345th Training Squadron, Fort Lee, Virginia

FORKLIFT SIMULATORS

19 May 2020

1. GENERAL

1.1 The 345TH Training Squadron, Traffic Management Course requires two (2) additional brand name forklift simulators (FL-Simulators: Model # FLS-CBS-001) in order to be utilized in accordance with the Career Field Education Training Plan.

1.2 The Government shall not exercise supervision or control over the contract service providers that will be performing delivery and setup of this purchase. Such contract service providers shall be accountable solely to the contractor, who in turn is responsible to the government.

1.3 Background and Objective:

This Statement of Work defines the requirements for the contractor to deliver, assemble and ensure proper operational use of the requested two (2) additional forklift simulators that will be added to the Traffic Management Course, which is currently using four (4) simulators as part of the curriculum.

2. SCOPE

This Statement of Work (SOW) covers requirements for the shipping, assembly, and functional training/operational use of the forklift simulators (FL-Simulators: Model # FLS-CBS-001).

3. BACKGROUND

The 354th Training Squadron, located at Fort Lee, VA, utilizes the forklift simulators (Model # FLS-CBS-001) during their Traffic Management Course. The requested forklift simulators have been an approved training device IAW with Plan of Instruction/Course Number LCABP2T031 00AD. As part of the approved training curriculum exclusively throughout Department of Defense for standardization and continuity of training, the two additional forklifts need to be identical. The two additional simulators will ensure all students receive the exact training in accordance with the Career Field Education Training Plan all utilizing the same devices.

4. REQUIREMENTS

Full and complete delivery of the forklift simulators (FL-Simulators: Model # FLS-CBS-001) to be used by instructors and students of the Traffic Management Course. Below is a listing of the

JBSA Lackland, TX 78236 ATCH 1 specific items of what is requested and information of the manufacture. See Attachment 1 at the end of the document with specifications and features of the forklift simulators that can be used as an example of the quality of the products desired by the government.

4.1 General Requirements

Specific Items:

4.2 Delivery

The forklift simulators will be delivered to Building 1540 (RM: 206), 201 C Avenue, Fort Lee, VA, 23801. The POC for the delivery driver will be Mr. Doug Riney. He can be reached at 804-765-1796.

4.3. Hours of Operation

The contractor shall have access to the site from 0730 – 1630 Monday through Friday (excluding weekends (Saturday and Sunday) and federal holidays). If the delivery and assembly process should take more than one day of work, the contractor shall report to the site PT Supply NCOIC at the beginning of each day prior to the start of any work and at the end of each day prior to departing to discuss progress.

4.4. Quality Assurance

The contractor shall be responsible for quality control for all work accomplished during the performance of the task up until receipt of delivery is confirmed with the 345th Training Squadron.

4.5. Warranty

The contractor shall provide all specifications of manufacturers’ warranty for the installed parts and provide a 1-year warranty on labor after Government acceptance of the installation

5. ACCEPTANCE

Upon completion of the delivery, the forklift simulators shall be subject to a final acceptance test performed by the contractor and the 345th Training Squadron POC.
